User :   I have two concerns that I would like to inquire about today, I have consulted doctors in the past about this, but would like to know more and provide the latest information.

User :   First, I am 26 / male with healthy habits of eating and don't smoke

User :   yes, my first concern is urological
User :   my symptoms include a strange feeling after urination

User :   after urination I often feel like I have some dribbling

User :   sometimes this feels like a slight amount of ejaculatino
User :   and often leaves a small stain on my pants
User :   so I need to spend excess time shaking off excess after urination

User :   but often I will have this feeling of small ejaculation sometimes with a short stinging sensation at the tip if I sit down a minute after urination

User :   I have what looks to be a very large superficial dorsal vein

User :   I have drawn an image to roughly illustrate what this looks like

User :   I have noticed first an increase in urinary frequency maybe 2+ years ago
User :   the dribbling and sensations after urination are maybe more recent but probably has been a year now

User :   the urinary frequency has perhaps improved somewhat as I have trained my body to stay calm and urinate less
User :   but sometimes I still have sudden urges

I have seen the image
User :   especially with alchohol even in small amounts
Doctor :  
It is not of major concern
User :   the vein is not painfull
User :   but swells to very large size, especially in warm water
Doctor :   that is a normal phenomena, the veins dilate when exposed to heat and can constrict when exposed to cold.

so do not worry on this,

Now coming to your urological problems
User :   great, thank you this was causing me great concern

May i know the present urological problem you have

You had mentioned few of the symptoms have improved

Do you still have ejaculation after every time when you pass urine

or does it happen once in a while?
User :   well I think I may have improved upon the physcological part of urinary frequency, I have tought myself that when I feel stinging and such that it does not neccessarily mean I need to urinate
User :   but aside from that I do feel the issues
User :   I still have the feeling of ejaculation after urination on most occasionas
User :   it varies from not feeling it much to feeling it more noticably
User :   but I think I feel some degree of this 90% of the time
User :   I also feel a burning urge to urinate during bowel movements
User :   even if I don't have to

since the urethra and the ejacualatory ducts is inter linked there can be semen in the urine on some occasions.

User :   yes
User :   I did see a urologist a few months ago
User :   this is what was done
User :   He did a urinary flow test
User :   this came back normal
User :   exam of the genitals - normal

User :   no prostate exam because I just had a thrombosed pile removed a week or so before seeing the urologist
User :   and urinalysis
User :   all normal
User :   he gave me a medication to try if I was concerned about urinary frequency

has any ultra sound examination been done.
User :   but I have not tried it because of the possible side effect of constipation
User :   I had a ultrasound that measures urine in the bladder after the flow test
User :   this came back empty - normal
User :   but no ultrasound imaging

User :   I did have an abdominal / pelvic CT scan recently
User :   with contrast

User :   this was for GI problems

and may i know the results.\
User :   yes, I have an appointment scheduled with my Gastro enterologist for the 8th to discuss in more detail but he called me to say they found an umbillical hernia and lots of stool in the bowels

User :   but no other major issues

I shall explain about your urogical prolems.

The ejacualatory duct and the urethra are linked
Doctor :  
and occasionsly you can find the semen in the urine.
Doctor :   If it is happening very frequently you need to have a complete urological examination to exactly pinpoint the cause of the problems which should also include a prostrate examination.

User :   yes, well I have had my prostate checked more than once in the past year, I have had plenty of GI and rectal problems so I have been examined and whenever the Dr. mentioned the prostate he has said normal
User :   At least in terms of size

User :   and my urine cultures come back okay
User :   but all I can think of is prostate issues or the urinary sphincter spasming or something
Doctor :   has your spine examination been done.
User :   spine?, not recently
Doctor :  
Primarily it is a problem of the splincter.
Doctor :  
Some time there can be congestive prostratitis which can also cause such symptoms
Doctor :  
You may not have prostrate enlargement in congestive prostratitis.

User :   yes, could you tell me more about congestive prostratitis?
User :   what are symptoms, causes, diagnosis, treatment?
Doctor :   I meant a neurological examination of the lower spinal nerves is what i meant.
User :   oh, I have not had this but I can tell you that I do have a fractured vertebre
User :   I fractured this over 10 years ago
User :   but they said it was not going to heal back as strong most likely
Doctor :   May i know where exactly was the fracture, i meant
User :   L5
User :   two fractures
Doctor :   I would suggest you to have the spinal examination done

SInce the lower spinal ( specifically the lumbar spine) has to be checked.

User :   how does this relate? you suspect there could be nerve issues in my spine causing sphincter issues?
Doctor :   Splincter disturbances can happen in a spinal nerve pathology and hence i am suggesting it.

User :   ok, if this is the case is the prognosis good? i.e. is this treatable?
Doctor :   It depends on the type of the pathology.

I meant they can be treated in a few occasions, some may need surgery, and others are treatable by medications similar to those you are presently on.
Doctor :   All this can be commented only after an examination.

Hence i would suggest you to have a neurological examination done too.

Too much alcohol can also be a cause,
User :   I do not drink much alchohol
User :   only on special ocassions these days

User :   yes, still a few questions: I want to ask what a neurological examination consists of but then also ask more about congestive prostatitis

You can mention about your vertebral fracture to your urologist in your next meeting to him and he may do the examination himself or may refer you to a neurologist.

It is a condition of inflammation of the prostratic gland

it mainly happens because of overload.

when ever there is incomplete ejaculation or when there is a sexual arousal and no ejacualtions all the secretion of the prostrate and ejaculatory duct can get stored

they are associated some times with pain in the lower abdominal region or the testicles.
User :   I have lower abdominal pain
User :   not much in the testes though

Meaning you need to sit in the warm tub every day for 10 to 15 minutes

this will help in reducing the congestion.

User :   I think that is mostly it, in terms of the veins while not anything to worry about I have some anxiety about the appearance

Doctor :   Yes the veins are not to worry.
Doctor :   shrinkage of these veins can some time lead to complications which can include losing potency.
Doctor :   So do not worry on trying to find a treatment on this.

User :   yes the other problem relates to my GI problems and lower abdominal burning
User :   I had a CT scan done

User :   2 years of lower abdominal pain started July of 07
User :   I believe

User :   I don't know it is not visible externally and not felt by a physical exam
User :   only on CT scan
User :   so I suspect minor
User :   but I do have pain
User :   from chest to pelvic region
User :   in different spots

Does it feel like the burning sensation all over?

User :   I have a bruise type pain sometimes above and below the belly button, comes and goes, but worse with a full stomach
User :   in addition I have pain that is lower
User :   and more severe

User :   this pain is about 3-4 inches below the belly button
User :   under where my belt buckle is
User :   this pain is burning

User :   it comes and goes but when it is there it is very sensative even if I glide my hand over the area
User :   this is more like a burning sensation while the pain around the belly button is bruise like

does the pain has any relation with the food consumption?
User :   yes
User :   some soups that cause gas irritate it

does it increase or decrease after consumption of food?
User :   increase
User :   and generally subsides after 1 or 2 bowel movements
User :   sometimes subsides with rest

And how long do these pain last?
User :   few hours generally

Have you tried any medications/
User :   I am on one medication now called Amitiza
User :   are you familiar with this?

And has it helped you?
User :   this medication is not a laxative but is taken on a regular basis to add water to the intestines
User :   It has not had a major effect on the pain
User :   but it may have made a minor improvement
User :   not enough to know for sure

This medication is basically given to increase the motility of the git.
User :   I will see my GI dr in a week but what I am trying to understand is where umbillical hernias are located
User :   so I can determine if the pain I am having is from the hernia or something else
Doctor :   Umbilical hernia can some time cause pain.

but the pain is usually located over the site of hernia.

SInce you say the pain is a diffuse variety
Doctor :   I do not think the cause for your pain is umilical hernia
User :   yes
User :   if the hernia is the cause of pain I will ask for surgery
Doctor :   I do not think the hernia is not the cause for the pain.
Doctor :   How ever if the content of the hernia contain the loops of the intestine you can go ahead with the surgery.

Doctor :   umbilical hernia are often located around the umbilicus, you need to know the size of the hernia and the contents of the hernia.
