Doctor :   Hi
Doctor :   How can I help you?
User :   hELLO HOW ARE YOU
Doctor :   Good thanks
User :   I HAVE BEEN HAVING MULTIPLE PVC'S HOLTER MONITOR THURSDAY NIGHT SHOWED 5000 IN 24 HOURS, ALL UNIFOCAL
Doctor :   ok
User :   THE NIGHT BEFORE IT WAS MUCH WORSE BUT I WAS NOT ON MONITOR, FRIDAY NIGHT WAS NOT AS BAD, THIS MORNING IT IS BAD AGAIN LIKE 5 TO 10 A MINUTE SOMETIMES AVERAING 15 EVERY FIVE MINTUS JUST TAKING MY PLUSE
User :   I AM ON SOLATOL 40 MG TWICE A DAY.............THIS STUFF IS MAKING ME CRAZY I AM TOLD NOT DANGEROUS IS THIS CORRECT AND DO YOU HAVE ANY SUGGESTION, IT WAS AROUND 3000 IN DEC
Doctor :   Ok I can guide you and suggest you in that
User :   I DO HAVE SLEEP APNEA OXY LEVELS AT NIGHT ON MACHINE AROUND 90 WHEN ASLEEP COULD THIS BE CAUSING PROBLEM TO BE SWORSE
Doctor :   But I want to know few things
User :   THANKS ANY INPUT WOULD BE APPRECIATED
Doctor :   Are you a medical personnel
Doctor :   and how could you know that it is unifocal PVC's
Doctor :   Why is the Holter monitor
User :   BECAUSE I HAD FELT ALL OF THE SKIPPED BEATS THEY WANTED TO CHECK AND SEE HOW BAD
User :   THE MONITOR WAS JUST FOR 24 HOURS
Doctor :   Ok is that the doctor asked you to monitor the holter
User :   MY CARDOLOGIST PUT THE HOLTER MONITOR ON ME
Doctor :   That is fine
Doctor :   Now I want to ask you if you have measured the pulse rate
Doctor :   during the PVC's
User :   YES IT IS AROUND 60-72 BPM
User :   THE HOLTER MONTITER ARE YOU FAMILAR WITH THEM
Doctor :   Is there any change in Pulse during these erratic episodes?
Doctor :   Yes
User :   FIRST IN ANSWER TO YOUR QUESTION WHEN I AM FEELING MY WRIST TO SEE IF THEY ARE SKPPING AND TO TAKE ME PULSE THE RATE RUNS IN THE LEVELS I HAVE STATED, ACTUALLY AS YOU KNOW WHENYOU THROW A PVC IF FEELS LIKE YOU HAVE LESS BEATS BECASUE THE PREMATURE BEAT IS SILENT SO TO SPEAK YOU CANT FEEL IT WHEN FEELING YOUR WRIST
User :   DURING THE HOLTER MONITOR THE RATE RAN FROM A HIGH AT ONE TIME DURING THE DAY OF 128 AT 10 O'CLOCK TO A LOW AT FOUR IN THE MORINNG WHILE I WAS ASLEEP AT 45 BPM,
User :   I HAD AN ECOH AND CHEMICAL STRESS TEST FOUR MONTHS AGO BOTH WERE GOOD
User :   MY EF IS AROUND 55
Doctor :   Good
User :   I DO HAVE TWO STINTS IN MY LAD FROM 7 YEARS BACK
Doctor :   Wonderful
Doctor :   Ok
User :   MY QUESTION IS HOW DANGEROUS ARE THESE PVC'S WHEN THEY GET UP INTO SEVERAL THOUSAND PER 24 HOURS OTHER THAN SCARING ONE TO DEATH
Doctor :   Most of the times the Unifocal PVC's are as sae
Doctor :   safe
User :   AND ANY SUGGESTIONS REGARDING WHAT MGIHT BE MAKING THEM WORSE
Doctor :   PVC are most of the time safe as long as the ECHO, EF % are within normal limits
User :   SO EVEN IN THE 10'S FO THOUSANDS RANGE THEY ARE NOT GENERALLY DANGEROUS AS LONG AS THE ARE UNIFOCAL AND NO RUNS
Doctor :   And they are not to be concerned at all in a normal structure of heart
Doctor :   Yes
User :   I SEE SO BASICALLY ITS JUST THE TRAUMA PHYCOLOGICALLY TO ONE FROM HAVING MANY AND FEELING THEM
User :   SORRY ABOUT SPELLING
Doctor :   PVCs have little to no diagnosticDiagnostic laparoscopy significance outside heart
Doctor :   Sorry
Doctor :   for the last line
Doctor :   PVC's have little diagnostic importance
Doctor :   outside the heart disease
Doctor :   and even in heart disease they are taken to be dangerous
Doctor :   only when there is a compromise in heart's ejection
User :   OK
Doctor :   but in your case the EF is 55%
Doctor :   which is near normal
Doctor :   More over the unifocal are the safer to multifocal
User :   AT ONE TIME I HAD EP DOC TELL ME AS LONG AS EF WAS OVER 40 THEY WERE NOT PARTICUARLY DANGEROUS UNLESS ON HAD AN EPISODE AS A MI OR SOMETHING
Doctor :   Ofcourse there is a concern of multifocals more prone to leading into arrythmias
Doctor :   exactly got my point
User :   OK SO I SHOULD QUIT WORRYING AND WAIT AND SEE MY EP DOCTOR WHEN I HAVE AN APPOINTMENT?
Doctor :   Yes
Doctor :   May I know what is the dose of Sotalol
Doctor :   that you are on and dosing instructions?
User :   40MG TWICE A DAY I SPLIT A PILL
Doctor :   DO you mean 20 mg you take twice
User :   I USE TO BE ON 80 MG TWICE A DAY BUT IT CAUSED BRADYCARDUA
Doctor :   Ok ok
Doctor :   I got it
User :   NO 40 MG THE PILLS ARE 80MG AND I SPLIT THEM
Doctor :   Yes
Doctor :   So what is the heart rate when you were on 80 mg twice daily
Doctor :   *was
User :   I AM ALSO ON NORVASC WHICH TENDS TO SLOW HEART RATE I AM TOLD
User :   IT GOT DOWN INTO THE 40'S DURING THE DAY
User :   SO CARDIOLOGIST CUT DOSE IN HALF ABOUT THREE YEARS AGO
Doctor :   Yes
Doctor :   It is not good to put the HR less than 40
User :   BROUGHT RATE BACK UP TO AROUND 60
Doctor :   Good
User :   THAT IS WHAT I HEARD
User :   DO YOU THINK THE SLEEP APNEA IS PLAYING A PART IN THIS ISSUE
User :   I ALSO WAS TAKING NASCORT NASAL SPRAY AND ASSTELING, (AZELASTINE HCL) ALONG WITH CLARITINE FOR SEVERAL DAYS HAD STOPPED THE CLARITIN ABOUT THREE DAYS BEFORE THE PVC'S GOT BAD
Doctor :   There is a study that says that sleep apnoea can cause increase in PVC
User :   WHAT DO YOU THINK ABOUT THE TWO NEW MEDS I WAS ON
Doctor :   NASCORT is an inhalational steroid is it?
User :   YES
User :   THE OHTER IS AN ANTIHISTIMINE
User :   NO DECONGESTANT
Doctor :   Yes
Doctor :   There is no per se direct effect on PVC
Doctor :   Sleep anpoea does not cause PVC's remember
Doctor :   They may or may not increase the number of PVCs
User :   OK THANKS VERY MUCH I WILL TRY NOT TO LET THESE THING MAKE ME TO CARZY :)
User :   HAVE A GREAT DAY I APPRECIATE YOU HELP
