User :   I have a question regarding my mother, who based on recent tests has been prescribed to take ZOMETA.
Doctor :   Hi
Doctor :   How can I help you?
User :   Hello
User :   thank you for taking the time to talk to me
User :   should i give you quick background on my mom's health so you can better understand my question?
Doctor :   I do understand that Zometa is a chemotherapy drug,
Doctor :   So if you can give me more details of what you are looking from on this live chat, I can help you better.
User :   I wanted to give you some of her recent result for a test she did and get your opinion on a right course of treatment (here in the us or somewhere else), or whether she should even do the Zometa treatment at all
Doctor :   Can you describe her symptoms?
User :   She is 59, she was treated with breast cancer at the age of 33. since then she has been cancer free, but the doctors gave her Tamoxifen as an extra precaution. Recently, she had an eye nerve infection that she is currently battling that prevents her from seeing in her left eye, with headaches and poor sleep. last week she did a scitigraphy and i have translated the result from that, based on which they recommend she does the Zometa
User :   Here is a translation (hopefully an ok one) of her results and conclusion: User :   Dispersed fixation osteotrophical radiopharmaceutical increased mineral metabolism in the vertebrae of the spine, joints of the upper limbs and stacked clavicular synchondrosis. Against this background appears abnormal focal hyperfixation in the field of flat skull, left shoulder joint, L4, Th5, L5, S1, sakroiliyachnite synchondrosis, head and great trochanter of right femur, left acetabulum and the horizontal arm of the left pubic bone, as and in the course of the left ribs 4-6 and 7-8 ribs on the right. Conclusion: Stsintigrafski data for bone lesions associated with cancer in the presence of paraneoplastic syndrome. There is an increase of the process in the bones compared to the previous scintigraphy. Details of degenerative arthritis, and arthritic changes in the skeleton.
User :   One of my questions is do you think based on the exam, that the heavy osteoporosis is a result from the cancer treatments in the past, or something that is going on now
Doctor :   Osteoporosis changes occur due to demineralisation process.
Doctor :   It is difficult to predict the cause of demineralization based on the scintgraphy techniques alone.
Doctor :   A MRI scans which looks at the structural aspects of the spine and bones can be able to differentiate well between them.
Doctor :   Zometa which is a biphosphonate helps in remineralization of the bones,
Doctor :   thus preventing osteoporosis and skeletal fractures.
Doctor :   I do agree that depending on the severity of osteoporosis and the severity of her symptoms, that zometa is indicated,
Doctor :   however she need further evaluation that includes diagnostic MRI scans, tumour markers such as CA 15-3, CEA and so on.
Doctor :   Discuss these issues with the treating doctor and if your doctors concur, then you can go ahead in getting these tests.
Doctor :   Have you followed what I have suggested?
User :   Yes i have
User :   She currently lives in xxxxxx, do you think it might make a difference her coming to the xxxxx or is the care and treatments pretty much the same everywhere
Doctor :   The chances of recovery if treated in xxxxx is marginal
Doctor :   The treatment will remain much similar at all places.
User :   Is this a dangerous condition?
Doctor :   Since the cause is not proved,
Doctor :   I cannot comment present condition.
Doctor :   The treatment is depends on the cause.
User :   i understand, but in most cases is it a treatable condition (maybe not full recovery of the bones, etc.) but at least getting it to be stable
Doctor :   Osteoporosis most often remains a chronic one.
Doctor :   and treatment options presently available are mainly oriented in preventing further bone damage,
Doctor :   along with treating the cause.
Doctor :   She is on the right track with regards to preventing further osteoporosis - zometa,
Doctor :   Further evaluation will help in diagnosing the cause.
Doctor :   It will be equally important if the cause when found is treated also.
Doctor :   Have you followed?
User :   yes
Doctor :   Do you have any other query?
User :   Since she doesn't live here, do you know if there are clinics that might accept to see her for consultation?
Doctor :   I am sorry, I am unaware of clinics at xxxxxx.
Doctor :   I shall try researching for a centre,
Doctor :   and if I can find one, I shall mail you back with the details.
Doctor :   Do you have any other query?
User :   i am sorry, i meant clinics here in the xxxxx that will accept to see a patient that is not a citizen
Doctor :   Since this involves legal formalities, as a general physician, I am not aware of those administration formalities.
Doctor :   It seems you have moved out of the chat window. Please log in back if you have medical queries
