User :   Hello Doctor...i am facing a problem of running fever from last 5-6 days. Temparature ranges from 100 to 103.

User :   temparature is not coming to normal
Doctor :   do you have any other symptoms?
User :   only fever and but yesterday i got shivering with fever in the morning and night
Doctor :   Ok.
User :   i have done all preliminary tests. 1. Malarial Prasite test; 2. Complete Bod Count test; 3. BioChemistry ; 4. Widal Test; 5. Urine Routine Analysis
Doctor :   May I know if you have cough,cold,diarrhea, vomitings ?
User :   everything was normal. "Nil" positives or major findings
Doctor :   Ok.
Doctor :   So all the tests were negative is it ?
User :   no i dont have cough, cold, vomitings..
User :   Yes all tests were Negative
Doctor :   Ok.
Doctor :   Do you have continuous fever ?
Doctor :   Or does it come down at any point of time ?
User :   I am currently taking the following medicines 1. Azee [Antibiotic] 2. Meftal - for fever 3. Riconia
User :   No it is continuous fever. it starts with may be 100 and by evening it touches 103 types
Doctor :   Is the fever always high or does it become low and high ?

User :   i faced a similar problem last month. Same problem with same type of fever
User :   i was given treatment by IV, because i was not responding to medicines

User :   Medicines used that time was 1. Tazacef 2. Emal
User :   i might have spelt the first medicine wrong

User :   they could not finally diagnose what was the problem but by IV treatment my temperature was brought to normal
User :   now again after 20-25 days i am getting the same problem

User :   Mumbai

Doctor :   Have you travelled to any places before the first bout of fever ?
User :   No i have not
User :   i suspect i would have had something outside

Doctor :   Have you been tested for Dengue ?
User :   No
User :   could you tell me what are the symtoms of dengue
Doctor :   Have you been tested with CBC? How are the neutrophils, esenophils , and other cells ?
Doctor :   Do you have any previous history of surgery ?
Doctor :   Have you been tested for T.B. ?
Doctor :   Any x-ray lungs ?
User :   Yes i have also got TB and Chest X-Ray done
Doctor :   Ok.
User :   Both the tests are negative
User :   No previous history of Suregery
Doctor :   ok.
Doctor :   What are the tests done for Malaria ?
User :   Neutrophils - 54; Lymphocytes - 30 ; Monocytes - 15 ; Eosinophils - 01 ; Basophils-00
User :   Malarial Parasite test
Doctor :   Ok.
Doctor :   Do you have any pet animals or birds with you ?
User :   no
User :   we have mosquitos problem at home....but we normally take precautions
Doctor :   Ok.
Doctor :   May I know if you have repeated the malarial parasite test ?
User :   I have got it done on Saturday morning
Doctor :   ok
User :   However on Sunday morning and evening i got shivering. So today again i have given the test. Awaiting report
Doctor :   Ok.
Doctor :   Malaria PCR assay and microscopic examination of Giemsa-stained blood films for detection and identification of Plasmodium spp. in blood specimens. PCR was more sensitive than microscopy and capable of identifying malaria parasites at the species level when microscopy was equivocal.
Doctor :   I would suggest as most of the malaria dont show up as classical posstive in tests
Doctor :   and need malarila treatment
Doctor :   So i would recommend this test
Doctor :   and it would rule out anything else ?
Doctor :   1. Mlaria PCR
Doctor :   2. Dengue tets
Doctor :   If that doesnt get positive, there is a huge list of possibilities
Doctor :   and we shall take it forward.
