User :   My 16-year old had an LP on Thursday and I found out last night she has increased intercranial pressure. Although this is the least of my concerns, she wants to know if she can still go to a school trip since they are flying?
History - Sudden onset of non-migrainal headache on August 13, 2010. Misdiagnosed with cluster headaches. Got into a headache specialist who said it wasn't clusters. To date, she's had an MRI, MRV, 4 CT scans, LP on Thursday. She did have sphenoid sinus disease so had sphenoidectomy & ethmoidectomy on Dec. 3. Did not take care of the headache. We were in the ER all weekend (2 x) because she ended up with the residual headache & severe back pain. We are now at home forcing caffeine fluids and taking pain meds. We do not have back the LP labs but the report noted increased pressure even though she has been taking 4 diomox per day for 4 weeks. On a scale from 1-10, her pain level from August to November was a 5 - 6 and would peak (a period where it feels like a drill is going from her top of her head to the base of her neck repeatedly OR if feels like intense pressure). Since Nov 17, she is living on a pain level of 10 daily. She has not had one moment of relief since August 13. She is a junior in high school and since Nov. 17, really has just slept her life away.
Here's what her August looked like, she had flown to the Dominican Rep, NYC and w/in 6 weeks. We flew to while she had swimmers ear. We went to Disney on a very hot day and didn't stay hydrated as we should have. The next day, the headache hit and hasn't left.
Doctor :   Hi
Doctor :   let me read ur query
User :   thanks...
User :   currently taking 4 neurotin, 4 diomox and 2 vit c daily
Doctor :   may i knoe whats the duration of the flight ?
Doctor :   and where is she flying to ?
User :   to DR, 4 hours...NYC 2-1/ only 1-1/ hours...
User :   oh sorry....flying to boston....I would guess about 3 to 4 hours
Doctor :   ok
Doctor :   flying for 3-4 hrs will not be an issue
Doctor :   but is here raised ICT levels high ?
User :   ?
User :   i don't understand
Doctor :   i meant high intracranial pressure levels ?
User :   yes....our neuologist called last night when we were back in ER that her pressure is high still don't know why but we are hoping this could be the cause of the headaches...
Doctor :   high ICT usually causes persistant headahce
User :   on top of the head?
Doctor :   yes
Doctor :   do u have the MRI reports ?
User :   MRI showed a small pineal gland cyst (with contrast btw).....MRV showed assymetrical venous flow
User :   oh....and sphenoid sinus disease that led to the surgery
Doctor :   so i guess there must be some block in the CSF flow in the brain
User :   i think there is a block of something BECAUSE she is not responding correctly to meds in my opinion...when she had her wisdom teeth out, i would give her 1/2 a phenegryn and she was out for 12 not even perocet or torodol or neurtin on phenegryn touch the pain or makes her sleepy....seems odd
User :   she is set to go to Duke's private diagnostic clinic Feb 7....just have to get the insurance company to approve her (currently denying).
Doctor :   OK
User :   thanks....
Doctor :   For the relief of the pain completely the block may need to be addressed
Doctor :   as and when the block, is corrected her symptoms will reduce
User :   okay. i'll ask dr. sacco about that!!! thanks....last question....anything else you think is important to tell me.....anything jump out at you?
Doctor :   the headache is due to excess pressure int eh cranial cavity
Doctor :   this pain will be reduced only once the pressure is reduced
User : have been helpful. we will continue to be hopeful and know God has to use this whole thing for His good!! Thanks again!
Doctor :   you are welcome
