User:I am 23 weeeks pregnant I am using medicine (thyronorm 25mg) for hypo thyroidism since 1 month. i need to review it every month as prescribed by my endocrinologist. tomorrow i need to take a blood test for free t4 and tsh levels.for that do i need to go with empty stomach? and i am taking thyronorm every morning on empty stomach .can i take the tablet and go for test or without taking the tablet should i take the test?will the values differ?
Doctor:There is no need for a fasting for the test
Doctor:you can also take the tablet and go for the test , as usual
User:does this alter the valyes?
Doctor:there will be no change in the values
Doctor:the TSH is based on a long term feedback, due to hormone levels in the blood
Doctor:and not based on the regular intake of the tablets
Doctor:the tablets, you take contain T4 hormones
Doctor:if the levels are adequate, then the TSH levels will be stabilised and be normal
Doctor:if the levels of T4 , is lower, than is needed, then the TSH levels, will be elevated, indicating need to icnrease the dose of your medications
Doctor:if the dose of T4 (Your medication dose), is higher, then the TSH will be lowered, which would indicate the need to lower your dose of your medication

User:one more thing last time when i met my endocrinologist he asked to go for another test ANTI THYROID PEROXIDASE ANTIBODY. do i need to take it again tomorrow along with free t4 and tsh?
Doctor:yes, this is a test, to know the cause of your low thyroid condition
Doctor:you should definitely undergo the test

Doctor:in that case, your doctor, wants to check, if the treatment being provided is helping to lower the titres or not , in other words, it is to check , if the medications are helping to correct the underlying condition or not

User:does this hypothyroidism in any way effect my sugar levels?
User:actualy last time random sugar level test came to be normal
User:and now i am taking thyronorm 25mg daily will that affect the sugar levels?
Doctor:no, the thyroid hormones, do not affect the glucose levels directly
Doctor:but, when severe hypothyroidism, develops and is not treated, the cholesterol levels can become elevated
Doctor:As i said, the thyroid hormones, do not cause any change in your sugar levels
Doctor:the hormones are being given, to you, as your body is not producing them properly
Doctor:and those hormones are needed for the welling being , growth and development of all tissues and cells in your body
User:actualy my tsh was 5.99 last month.will this affect the baby?
Doctor:lack of them, will cause cell metabolism and growth to be affected , and in case of pregnancy, such lack of hormones can cause mental regardation, in the fetus
Doctor:SINCE medications have been started to fix the condition, you need not worry
Doctor:the TSH at that point, is just slightly higher, and it will not cause any major issues or problems
User:actualy this is my second pregnancy and i didnt have this thyroid problem during my first pregnancy.will this continue lifeterm or tsh levels revert back to normal after delivery?
Doctor:yes, it is possible, for the condition to subside after pregnancy
Doctor:and in such case, the thyroid medication, may be stopped
