User:i am pregnant my last month period date is 18 april 2013
Doctor:Proceed with your query.
User:i am having rubella infection. IgG andIgM both are positive
User:ultrasound report shows heartbeat 118bpm and there is subchorionic volume 0.1cc.
Doctor:Are you there?
Doctor:Do you have any symptoms now?
User:not yet
Doctor:Have you seen your gynecologist?
User:but i m still confuse
Doctor:Active rubella infection is slightly concerning during pregnancy. Since you are under 12 weeks of pregnancy, there is always a risk of fetal developmental defect. To help you understand better, I would like to know what your gynecologist's suggestions were?
User:as ultrasound shows on 11 XXXXXXX i was 6.3 week pregnant
User:According to report rubella IgG avidity 77%
User:so dr. says it shows infection is old
User:and also saying consult with foetal medicine specialist
Doctor:Yes, IgG positivity indicates old infection and IgM indicates new infection
Doctor:Recent active infection causes health risk to the fetus and therefore to assess the fetal risks, your doctor suggested to see a fetal medicine specialist.
User:on 26 XXXXXXX IgG 500 and IgM 1.03 IU/mL
User:and on another report 11 XXXXXXX it was 127 and 1.02 respesctively
Doctor:The standard reference ranges varies according to laboratory centres, so may I know the reference values of both IgG and IgM?
User:for IgG >= 10IU/mL reactive,and IgM>=1.0COI reactive
Doctor:Thanks for providing the details. Since two reports revealed positive IgM, it indicates a recent infection either before pregnancy or early pregnancy.
Doctor:Since the levels of IgM is rising, the good news is that your body is fighting the infection and you will be fine.
Doctor:However like your doctor said, it would be better to assess fetal growth and act accordingly.
User:sorry, no there is mistake on 26 may igm was 1.37 and on 11 XXXXXXX it was 1.02
Doctor:I understand. As the levels of IgG is on the rise, this is an indicator that your body resistance is actively fighting the infection.
User:on 26 may igg 500 and 11 XXXXXXX 127. is it increasing or decreasing?
Doctor:Rising IgG is a good indicator that your body resistance is good enough to fight the infection since we do not have specific medication to treat rubella.
User:whether i shoulld go for abortion or not
User:how do i now whether baby will be harmed or not ?
Doctor:That being said, since the infection is detected during the time of pregnancy, it is essential that we check the status of fetus and take a decision whether or not continue the pregnancy.
User:there is one more issue as i have asked before regarding ultrasound
Doctor:Anomaly scan or fetal doppler may be suggested to you to check for fetal abnormalities.
User:at 6.3 weeks heart beat 118 bpm and there is subchorionic collection 0.1cc
User:whether medication for this is essential or not
Doctor:The scan here doesn't reveal organ pathology.
Doctor:As I said, we do not have specific medication to treat rubella infection.
Doctor:We rely on body resistance, which seem to be good in your case.
Doctor:The priority now would be to know the fetal risks and well being.
Doctor:If fetus is not affected, you can continue pregnancy.
Doctor:This can be best judged by fetal medicine specialist.
Doctor:So I agree with your doctor.
User:dr has written one medicine sustan it safe to take this
Doctor:Sustan 200mg is a tablet given to prevent miscarriage.
Doctor:Since rubella infection can increase risk of miscarriage, your doctor's suggested it.
Doctor:Please continue the tablet. However it doesn't treat the infection per se.
User:during first trimester transvaginal ultrasound is harmful or not
Doctor:It doesn't cause major effects. Most radiologist are very careful while doing TVS
Doctor:Have you followed?
Doctor:Do you have any other query?
User:no thanks a lot
User:thank you
Doctor:Thanks for consulting me.
Doctor:Bye for now.
