User :   retroperitoneal, mesenteric, bilateral iliac chain and inguinal lymph nodes. what does this mean?
User :   good day to you doctor
Doctor :   please describe your symptoms in detail
Doctor :   is this a report that you want to be interpreted?
User :   well its been very long now since my right lower pelvic has pain
User :   yes from MRI
Doctor :   ok. i will explain.
Doctor :   these are the names of the groups of lymph nodes that are affected or inflamed as seen on the MRI. their names suggest their area of location
Doctor :   retroperineal means -behind teh peritoneum
User :   ok what may be the cause of its inflammation?
Doctor :   mesenteric - means in the mesentry that is to say the lining of fatty tissue covering the intestines
Doctor :   iliac and inguinal chain means groups of lymph nodes in your lower abdomen and groin area
Doctor :   the cause for inflammation could be anything like infection, growth, infiltration of other cells into lymph nodes etc
Doctor :   the findings should be correlated with your symptoms and examination findings to be able to tell you the probable diagnosis
User :   that is just one of my problem because they wont explain very much to me
User :   dr. follicles with rim enhancement what do this mean?
Doctor :   in your last chat discussion you have said that "histopathology of the left ovary was serous cystadenoma"
User :   yes
User :   thats with my left oveary
Doctor :   so teh lymph nodes could be affected due to spread of this too
User :   will there be any connection?
Doctor :   ask for biopsy or FNAC to be done on these to see if the cell type is similar
Doctor :   if they show similar cell type in the nodes
Doctor :   then these nodes need to be removed soon
User :   may that be possible for cancer?
Doctor :   follicles with rim enhancement mean that follicles with the outer rim thickened.
Doctor :   serous cystadenoma is a benign tumour and present as cystic masses so the rim enhancement could be due to this
User :   i do not understand? is that bad?
Doctor :   if these cysts are increasing in size or still causing you pain, they can turn cancerous anytime
Doctor :   with lymph node infiltration
Doctor :   discuss with your treating gynecologist about these: Doctor :   1) getting FNAC or biopsy done on lymph nodes
User :   with the size of the cyst in the right ovary on 2007 1.4cm up to yesterdays MRI still 1.4cm
Doctor :   2) getting repeat scans done to see if cysts are increasing in size
Doctor :   ok
User :   but only my right lower pelvic feels dysmenorrhea like pain
User :   but i am already hysterectomize
Doctor :   ok. was endometriosis ruled out? one of the reasons for chronic pain is endometriosis.
User :   no endometriosis
Doctor :   ok
User :   they cant even look for what was causing my pain
User :   yesterday I was talking to the doctor that did my MRI
User :   she said something that a metalic something was left near my naval area
User :   when we ask she said it was particles from bomping surgical instruments
User :   is this possible?
Doctor :   well I am not sure about it. if something was left behind then it can be possible
Doctor :   usually particles get cleaned with the final wash that is given before closing of abdomen after surgery
Doctor :   to remove all blood clots and stuff that could get dislodged while doing the surgery and float around
User :   oh so that really happens? that when instruments bump with each other particles can fall and be left behind?
Doctor :   so if something metallic has been found then it might need further investigations to know what it exactly could be
User :   when you take a look in the MRI its round very black
Doctor :   it can hapen, but not always, and as I said, post surgery, a wash is given with medications to make sure you do not have anything left behind
Doctor :   if in doubt get MRI report by another radiologist to see if findings stated are same
User :   unfortunately some not all are negligent in their work
User :   yes you are right that is a great idea
Doctor :   if you are still faced with pelvic pain even after surgery, then take a second opinion from another gynecologist to see if anything else is needed to be done.
User :   dr. lets say that there were particles that was left behind can you tell me the complications, risk
User :   no gynecologist wants to accept me anymore
Doctor :   complications could be pain, discomfort, adhesions and fibrous scars formation, depending type of metal cancerous changes (rare cases).
User :   exactly
User :   it that very area i feel 1st like inside sopmething is tearing then right after that it feels very hot like fluid hot
Doctor :   so a repeat surgery or a laparoscopy is needed to see what is going on and remove the particles and the adhesions etc
User :   doctor i dont think i can take another surgerty i already had 9
Doctor :   then discuss about a diagnostic laparoscopy done to see what is going on and remove the particles and the adhesions etc
User :   thats great
User :   doctor ill go bach with my right ovary
User :   so when follicvles already has rim enhancement that that certain cyst do not go away can that be a sign of cancer?
User :   ill type to you the hystopathology report
User :   so you could see if that can be related
Doctor :   ok
User :   for a while im just getting the result
User :   left ovary serous cyst with hemorrhage and infarction
User :   left fallopian tube congestion
Doctor :   is it left ovary or right ovary?
User :   left doctor
User :   my right is still here
Doctor :   was the entire lefty ovary with infarction changes?
Doctor :   *left
User :   but we just learned yesterday thet my right fallopian tube is not anymore here
User :   yes left
Doctor :   was it removed? if yes what was the cause?
User :   2007 jan.13 they removed my left ovary and fallopian tube
Doctor :   ok
User :   my ob gyn said that becuase it twisted and ruptured
User :   but only the left
User :   but just hours before ny surgery they did an KUB xray
User :   it says
User :   segmental ileus, right lumbar region (reflex ileus?)
Doctor :   ok
User :   so i was confused now that i have read it because i cant see the connection with thius report with my left ovary
User :   what does the KUB report means?
Doctor :   KUB is kidney ureter bladder x-ray. it will not show ovaries
User :   oh ok
Doctor :   or any soft tissue like uterus, if there is any calcification present, it might pick it
Doctor :   or something like problemin bowels due to fecal matter in them it can show boels
Doctor :   do not get confused with the two reports, they have no connections
Doctor :   *bowels
User :   ok but with the bowel report what does that mean?
User :   and with the histopath what does it say?>
Doctor :   it means that there is changes in bowels seen that is segmental movements
