User :   I have a situation. My mother had a total knee replacement done 2 months back, but she is still in tremendous pain. The doctor who did it says resurfacing of the patella was not done, and it's not state-of-the-art. Last time when my mother had her other knee replaced (6 years back) by a different Doctor, patella resurfacing was done, and that knee was much better within a few days. Should re-surfacing be done, or not done?
Doctor :   hi
Doctor : here
Doctor :   please give me few moments to read what you have typed.
User :   Hello Dr. xxx
Doctor :   I am a general practitioner. I will try to answer your query with my expertise.
Doctor :   patella resurfacing is usually needed to be done if the knee replacement was done for severe arthritic changes
Doctor :   how old is she now?
Doctor :   what were her symptoms before the surgery?
Doctor :   do all movements of knee cause her pain?
User :   This 2nd doctor who did it this time says it is not done any longer, and patients just need to bear the pain, until it subsides. That should not be the answer.
User :   yes, pretty much. And first knee replacement did help her a lot, so she went for the 2nd one, but unfortunately to a diffrent dr.
Doctor :   yes, I agree that it should not be the answer. since she is having pain still, (2 months), I would recommend you to go ahead with taking a second opinion for her condition
User :   may I ask where you are located?
Doctor :   you can take her back to her first doctor or meet another orthopedic surgeon along with her reports
Doctor :   I am based in Banagalore
Doctor :   she would need an x-ray or repeat MRI to see what is causing he pain to persist for so long
User :   Yes, she did go for a 2nd opinion, and they took x-rays for both knees, and it is clear why movement in the first one is much better than the 2nd one. This Dr wrote it shoudl have been done, My father took it back to the dr who did the surgery this time, and he says no it shoudl not..
Doctor :   such that based on the finding decision can be taken whether she needs re-surfacing or not
User :   ok, my mother is in bangalore too. first time it was done by Dr Chandy.
User :   she will need another surgery to fix the problem.
Doctor :   ok. if there is recent x-rays done, then it may not be required to be done again
Doctor :   Dr. Chandy is well known and has good hands
User :   yes, but this time they went to Manipal instaed of Hosmat.
Doctor :   you can take back to him with the recent reports and x-rays done prior to surgery
User :   Do you think this could be medical malpractice if it should have been done and it was not done, and the Dr. could be sued.
Doctor :   for orthopedic problems Hosmat is the best
User :   yes, dur to some problems they could not go there, and got it done by Dr xxxx
Doctor :   ok
Doctor :   since she is in pain, I would strongly suggest you not to delay further, and go ahead with meeting Dr. Chandy (her previous doctor) soon
User :   Dr C will defintely not accept that he made a mistake. what do we do in this case. of course my mother will get the next surgery done by dr chandy, but what can be done about the possible mistake that was made?
User :   I mean Dr xxxx
User :   Do you know of any orthopadic surgeon in Indian medical association? how can we prove a mistake was made or not made?
Doctor :   once she is fine, after meeting Dr. Chandy,
Doctor :   you will also come to know if Dr. xxx made mistake
Doctor :   when Dr. Chandy examines her
User :   so there is no straight answer as to if the resurfacing is generally done or not done?
Doctor :   and if it proves that mistake was on the doctors side as revealed by Dr. Chandy who is well known and senior consultant
Doctor :   then you can approach consumer curt or medical complaints if you wish
Doctor :   yes, there is no straight answer as to if the resurfacing is generally done or not done
Doctor :   as it depends from case tocase
Doctor :   and from surgeon to surgeon
User :   hmm. ok.
Doctor :   but if arthritic changes are severe, it should be done to ensure patient recovers well
Doctor :   and faster
