User :   Hi - I've had periodic sharp vaginal pain and constant burning/irritation for 6 months. In the beginning, there wasn't anything to see. Now, there is a black spot about a quarter of an inch long and irregular in shape. I will make another appt with my gyn, but can you give me some ideas? Thank you

User :   Constantly!
Doctor :   Ok, Do you have any discharge form the vagina ?
User :   Sometimes. I periodically have yeast infections.
Doctor :   ok, May i know your age ?
User :   50 (almost 51), have not gone through menupause yet, but possibly close.
Doctor :   ok,it is probably a fungal infection super added to yeast infection
Doctor :   yeast infection makes the vangial and vulval mucosa very vulnerable to other fungal infections like Pytiriaisis versicolor
Doctor :   its a very common presentation
User :   OK - the gyn treated me for a fungal infectation at first and that did not help. She did a genital herpes test and I tested positive for Herpes simplex. At the time, there was no black spot, and I do have cold sores in my mouth.
Doctor :   ok, i do not consider a lesion being itchy to be herpes or related complication

User :   So when I visit the gyn, should I ask for any special tests?
Doctor :   yes, there is a entity called Herpes zoster which has to be considered in people with history of herpes simplex
User :   The zoster was negative.
Doctor :   yes, KOH mount test for fungal infection and rule out herpes zooster by clinical examination
Doctor :   oh, good
Doctor :   then you may need KOH or woods lamp test which can confirm the presence of fungal infection
Doctor :   that will suffice
Doctor :   the test gives a good idea as in which antifungal to be chosen this time
User :   OK. that sounds very doable. You have been much more communicative with me than the doctor. She said it was herpes and that was that.
