User :   I've been having constant, moderate pain in my right kidney, sometimes a stinging-like pain. My epididymis on my right testicle is also swollen. I've also been getting some stinging pain in my abdomen area.
Doctor :   hi
User :   hello
Doctor :   since when are you having these symptoms?
User :   Alright, there's a bit of history behind this, so I'll fill you in
Doctor :   please provide your age, gender, location, occupation, height , weight, details of past medical illness if any.
Doctor :   sure. go ahead
User :   Way back at about November 2009, I had a swelling in my right testicle. This turned out to be epididymitis. I took an antibiotic (doxycycline hyclate), and after that it seemed to ahve subsided
User :   My age is 24, male, XXX, mac'10", 145 lbs.
User :   Anyway, I also had an ultrasound done, and they also found a cyst on my right kidney
User :   The cyst was benign, and only filled with fluid
Doctor :   how big is the cyst? can you tell the size and description of of the cyst ?
Doctor :   ok
Doctor :   do you have the copies of ultrasound report with you?
User :   I'm actually not sure how big the cyst is. They only told me that I had one, and not the actual size.
Doctor :   if yes, could you type the result?
User :   Unfortunately, I do not. They never provided me with those.
Doctor :   ok
User :   Is that something you have to ask for?
Doctor :   the pain in the right kidney could be due to the cyst,
Doctor :   especially if it has grown or expanddeed, putting pressure on the capsule covering kidney
User :   I think they might have said something about the cyst being too small to cause any pain
Doctor :   yes, it is always better to have a copy of your reports with you.
User :   It could have grown. I had the ultrasound done back in February
Doctor :   i would recommend you to repeat the ultrasouond again
Doctor :   what other symptoms are you haveing?
Doctor :   *having?
Doctor :   is there pain/discomfort while passing urine?
User :   Well, now my epididymis is swelling up again on my right testicle
Doctor :   do you have pain in suprapubic region?
User :   See, that's what's so weird. I have normal colored urine, and it passes normally and without any pain
Doctor :   is there pain/discomfort in erection or ejaculation?
Doctor :   when was the last urine analysis and ultrasound done?
User :   I feel no pain during an erection or ejaculation. But I do feel tenderness depending on how the testicle is handled
User :   I had a urine analysis done last year in November (which is how it was diagnosed), and the ultrasound I believe was arounf February
User :   *around
Doctor :   ok
User :   I also have swelling above my right thigh
User :   And if I press down on it, I feel discomfort
Doctor :   could you describe the swelling above right thigh?
Doctor :   is it a single swelling or a group of small nodule like swelling?
User :   Basically, I feel discomfort from my kidney all the way to my bladder area on my right side
User :   One moment, I'll see what it's like
Doctor :   do you feel pain when you touch or press on epididymis?
User :   When the testicle is pressed up against something, I'll feel pain
Doctor :   ok
User :   But when I examine or feel my epididymis, I only feel how swollen it is
Doctor :   ok
User :   As for the swelling above my right thigh, that feels like a single swelling
User :   I think that's where my ureter is...?
User :   Would that be correct?
Doctor :   no, your ureter will be inside the pelvic and abdominal cavity
Doctor :   you canot touch it as a superficial structure
User :   Ah, my mistake
Doctor :   as it is deep seated
User :   But, overall, I'm just uncomfortable all of the time and I just want to figure out how to get rid of it. Moreover, I want to know what's going on inside there, you know?
Doctor :   i suggest you to get examined and repeat ultrasound along with a urine analysis
Doctor :   and a doppler scan for testis and epididymis
User :   Quick question, will this transcript be sent to my e-mail after our conversation is oveR?
Doctor :   this will enable you to find the cause
Doctor :   you can go through this transcript by going to 'account and settings' page, and click on 'chat history'
Doctor :   you wil see it there
User :   Alright, cool
Doctor :   and you can export to your e-mail ID and go through it anytime you want
Doctor :   have you followed what i have said?
Doctor :   once you get the tests done, and the examination
User :   I have
Doctor :   you can also discuss with me your reports
Doctor :   by sending me copies at YYYY@YYYY
User :   Ok
Doctor :   addressing it to Dr. XXX
User :   Also, I forgot to mention something else
Doctor :   yes, go ahead
User :   The second time the epididymitis came back, I was given a single dose of Zithromax
User :   And that was surprisingly effective
Doctor :   ok
User :   But I kind of wish I had a few more doses, since that felt like it only got rid of 75% of it. It's hard to explain.
User :   I went to a urologist about this condition, and he said the epididymitis was caused by bacteria
Doctor :   a doppler scan here will help you to identify if the cause is due to infection or viens around it or some other pathology
User :   I don't think I've ever had a doppler scan done before
Doctor :   and once it is known, effective treatment can be done.
User :   Also, I wanted to explain the stinging pain in my abdomen area a little more
Doctor :   go ahead, where is it located?
Doctor :   does the pain radiate?
User :   At the end of March, for about 2 weeks, I was having mild nausea almost all of the time, especially in-between meals
Doctor :   ok
User :   After a while I took some acid reducers for a few days, and that seemed to have gotten rid of it
Doctor :   ok
User :   but now it's happening again, especially in the morning
Doctor :   were the 'acid reducers' that you took given by your doctor? was any investigation done at that time?
User :   And it'll sting about 4" diagonally left from my navel, and about 2" across from it
Doctor :   the nausea can be present due to stress, medication use, gastritis due to ulcers
User :   I see
User :   I didn't see a doctor about it yet
Doctor :   ok
User :   I keep thinking that all three of these conditions are related to each other somehow
User :   Due to the same bacteria or something
User :   But I really don't know
Doctor :   yes, they could be related, stress due to the kidney pain and epidydimal swelling, leading to gastritis and nausea
Doctor :   the cause for th eright kidney pain could be attributed to the cyst
User :   I asked the urologist about surgery for the cyst
Doctor :   but the cause of epidydimal swelling needs to be infvestigated more
User :   But he said due to the size of it, and my age, there was more risk than benefit
User :   Yea, the epididymal swelling is different this time
User :   Only the epididymis is swelling up, and not the entire testicle like last time
User :   Also what bothers me about all this is that I'm not sexually active
Doctor :   a thorough examination and further investigations wil help you
Doctor :   and then definitive treatment should be done
User :   Alright so, I'll need a doppler scan
User :   and, basically, further examination on everything
Doctor :   yes.
User :   Thank you for your time
Doctor :   Is there any thing else I can help you with?
Doctor :   Please fill the feedback form say done when done
