User :   I have a gquestion about ovarian cancer

User :   Hello
Doctor :   Hi. How can I help you ?
Doctor :
Please go ahead with your query
User :   I had an ultrasound this week and the results did not sound good.
User :   They haved been following an ovarian cyst for over a year now. My doctor and radiologist said it was nothing to worry about to leave it alone but now
Doctor :
Ok
Doctor :
What happened?
User :   It has alway been clear with a small septation. this week it shows it has grown from 2.4cmx2cm to 3.3cx2.8cm and now it has 3 daughter cyst with one papillary protrustion of 9mm
User :   Does this mean malingcy
Doctor :
Ok I shall answer your question, let me ask you few questions.
User :   OK
Doctor :
What is your age?
User :   61
Doctor :
WHen was the first time they detected it?
User :   November of 2008
Doctor :
Why did you see the DOctor then?
User :   Yes they have been following it.
Doctor :
Did you have some problems then?
Doctor :
in Nov 2008?
User :   I had four ultrasounds that didnot change in size.
User :   I had a abdominal scan because of abdominal pain and was found to have diverticulitus. the cyst for found at that time.
User :   I had a D&C in July of last year for hyperplasia and it was find
Doctor :
Ok
Doctor :
So it was an incidental finding
User :   yes
Doctor :
and you still do not have symptom
User :   I am a registered nurse
User :   no symptoms really
User :   an occassional sharp pain but that is all
User :   mostly I feel good
User :   except for being scared to death at this point
Doctor :
Do you have a family history of ovarian cancer?
User :   no
User :   CA125 in Jan last year was 24
Doctor :
Ok, any follow up titres done after that?
User :   It has been repeated this week but do not have the results
Doctor :
Ok
User :   Ultrasounds we done ever 3 monts but they wanted to wait 6 months for this last one.
Doctor :
How many children do you have?
User :   No pregnancies
User :   I know that is a risk
Doctor :
Yes
Doctor :
What is your weight and height?
User :   5'5" 175lbs
Doctor :
Let me check your BMI
User :   Saw my gynochologist in Sept and he said everything was fine.
Doctor :
It is in over weight range
User :   Yes I know
Doctor :
The increase in size of the cyst is more to panic
User :   But isn't is still relatively small
User :   So should I panic?
Doctor :
Yes it is small
Doctor :   I am trying to evaluate risk factors though the cyst is not fearsome or trouble some
User :   It is the papillary protrusion that concerns me..should it ?
User :   Most of the cyst is anachocic
Doctor :   No

User :   So that does not mean papillary adenocarcenoma
Doctor :
I think you are overtly thinking about cancer
Doctor :
Septations and papillations does not mean it is a papillary adenocarcinoma
User :   Oh I see. Yes I have to admit, being a nurse and researching it does scare me excessively
User :   What about the daughter cysts?
Doctor :
The same reason
Doctor :
Cysts can happen in ovary being a menopausal women
Doctor :
However a weight reduction is important for you
User :   Yes that is what my doctor said and that is why he was not concerned. I have not seen him after this ultrasound yet however. Saw the nurse practitioner and I have appt with MD on monday. She said take it out.
User :   So will weight reduction change anythings as far as the cyst at this point?
Doctor :
The cyst may not regress
Doctor :
but the hormonal production depends on the fat that you have
User :   Well I have alway had a fat belly even when I was young and more slender
Doctor :
Yes that is a valid reason to have cysts
User :   My fear is that the cyst was benign and now it has turned cancerous and I have waited too long.
Doctor :
It has not turned cancerous, it is not confirmed yet
User :   But is that a valid reason to have cancerous cyst? Weight I mean.
Doctor :
It is not the cause
Doctor :
It is only a risk facor
Doctor :
Most important are the Genetic tendency
Doctor :
with BRCA genes
Doctor :
and then family history
Doctor :
then age
Doctor :   SInce you do not have the most important ones
User :   Yes I know.....I guess what I am asking for is your very honest opinion about the chances that it is cancer......a very honest opinion and I realize you don't have a biopsy to go by.
Doctor :
except age and no pregnancy
Doctor :
I opine only 10 to 20% chances for it to be a cancer
Doctor :   and might be less than that
Doctor :
Take a second opinion from a Gynecologist or an Oncologist
User :   Well I want to have it removed immediately.....cyst, ovaries but I don't want hysterectomy....does that make sense.
User :   May I ask your speciality?
Doctor :
IT is a preventive surgery
Doctor :
I am Family Practitioner
User :   Which the hysterectomy?
Doctor :
No the Oopherectomy
Doctor :
sorry, the ovary removal
User :   Yes, I want it out....if this is benign, I don't want to ever have to worry about this again.
Doctor :   I cannot get into those details of the benefits and risks of preventive ovary removal
User :   Oh I realize that. I tend to be very conservative when it come to surgery but I really want this thing gone.
Doctor :   WWW.WWWW.WW Doctor :
I think you can put across the same question to our Gynecologist here and take a second opinion
User :   Will I be able to chat with her?
Doctor :
I promise you an answer within 24 hours
Doctor :
Not the live chat though
User :   But it is hard when you can't talk to her directly.....if there are questions
Doctor :
YOu can put all the questions you have
Doctor :
and she can reply
Doctor :
and that is simple
Doctor :
My opinion is you can go for the surgical removal of the ovaries.
Doctor :   As physicians always weigh the benefits and risks
User :   OK....thank your for your help. OH is it possible to talk to the specialist by phone?
Doctor :
Yes
Doctor :
You can talk to the specialist over phone
Doctor :
So a specialist will be able to tell you the best benefits and risks
Doctor :
with that surgery
Doctor :
you wanted to undertake
User :   Thank you so much for your honest opinion.....I guess I just wish it had alleviated my fears. I just read the report , then research, and get scared. That is the way we nurses are.
Doctor :
Pleasure
Doctor :
chatting with you
User :   Goodbye
Doctor :
Hope your fears are allayed
