User :   Hi, I am curious to know if I have oral herpes or canker sores. I was recently tested for all other STDs, but I don't know if my blood test included testing for HSV-1 or -2. I have never had a blister on my lips, but I do get ulcers in my mouth. I am unsure if these are herpes or canker sores. Recently, my boyfriend mentioned that he has a sore in his mouth similar to mine, so I'm afraid I may indeed have oral herpes I spread to him; otherwise, he happens to have canker sores as well. My biggest concern would be not to spread this, if it is herpes, to his genitals or mine.

User :   I've had them since I was a child, always on the soft tissue of my mouth (inner lips, cheeks, tongue), usually when I am stressed out or sick or something like this.
Doctor :   were there any fever, pain for 1-2 days before the blisters arose and then broke into ulcers ?
User :   Um, no. What usually happens is I notice a small bump in my mouth that is smooth and round, then the covering will burst at some point and the sore will be open. I'm not aware of any fever or pain until after the sore is open, then it can be a bit painful to eat.
Doctor :   these are aptous ulcers induced due to stress
User :   If I bite my lip or something, I also develop a sore at that site often.
Doctor :   they are not related to herpes or any STD
Doctor :   they usually subside within a week
User :   Do you think it is a coincidence that my new boyfriend mentioned having a mouth sore?
Doctor :   but they are very painful in the initial period of 3-5 days
Doctor :   if he has the same presentation, he also might be having apthous ulcers

User :   We have been sexually active approximately 6 weeks, so when he mentioned this, I was concerned that I might have transmitted something. I know that apthous ulcers are not contageous, is this correct?
Doctor :   yes aptous ulcers are not contagious
Doctor :   they ocur due to stress
Doctor :   not due to any infection
Doctor :   but if any secondary infection occurs, it can make them more severe and more painful
User :   Hmm. What is the difference between herpes sores in the mouth and an apathous sore in the mouth? Do you know? Because I have seen some pictures online, and they are very similar, which was giving me some cause for alarm...
User :   Or, I suppose a better way to phrase it would be, IS there a difference in the appearance of these two conditions inside the mouth?
Doctor :   the appearnce is the same
Doctor :   but the presentation is differanet
Doctor :   fever, blisters , and multiple small ulcers
User :   Oh. So, what about my presentation of symptoms points to ulcers and not herpes?
User :   I am just trying to keep things straight in my mind, thank you for your kind attention...

User :   How can you be sure? Specifically, I mean?
Doctor :   100 % sure
Doctor :   I as a child used to have it too
User :   If they look the same, then how do we know without a blood test? (Believe me, I'm glad you gave me that answer, I just wanted to know why...)
Doctor :   coz, the association with severe stress, like missing food, severe illness, etc will be there or any injury in the mouth due to self biting or dentures
Doctor :   these are associated with apthous ulcers
Doctor :   but not with herpes
User :   I've read that herpes outbreaks can be exacerbated by stress as well, is this not true?
Doctor :   the presentation of herpes is classical ,
Doctor :   you mean herpes zoster
Doctor :   not herpes simplex that we are taking about as of now
User :   I believe so. The HSV-1 and HSV-2 viruses was what I was concerned about. What is the classical presentation of those?
User :   I've heard that herpes can be asymptomatic, too, which is why it can be so scary to have sex at all nowadays, no matter how careful we try to be...
Doctor :   fever, pain in the region for a few days, followed by blisters, whch turn to pustules after a few days and then rupture to form ulcers
Doctor :   this is classical herpes simplex 1 or 2
User :   Ah. Interesting. I see. I guess I was concerned that the pre-stages of my canker sores (the smooth, round surface which later ruptures) might be a blister.

Doctor :   thats not a blister
