Doctor :   Hi
Doctor :   How may I help you?
Doctor :   Are you there?
User :   hi
User :   yes
Doctor :   Please go ahead with your query
User :   I am suffering from Multiple fibrocystic disease of the breast. In 2007 they were only in my one breast and now they are in both the breast. Recently I went to the Gynae and she refered me to the GS as I have severe pain and its tender. The surgeon told me to go for Aspiration under GS.
Doctor :   Are there nodues in both breats?
User :   and I consulted the second Surgeon for the opioin
Doctor :   go ahead
User :   yes both the breast
User :   he told me to get it removed surgically which are bigger in size
Doctor :   Are they soft are hard?
User :   dont know abt this
User :   they are tender and painfull
Doctor :   You need to examine your breast your self, ofcourse it is done by Gynecology.
User :   the first surgeon also used the term auxillary LH
User :   and the second surgeon used Lesion
Doctor :   So you are asking me an opinion whether to go for surgical removal or not?
User :   yes and is there anything for worry
User :   ultra sound confirmed the cysts
User :   they are hard
Doctor :   OK.
User :   in 2007 it was in one
User :   mamagraphy in that year for one breast confirmed multiple fibrocystic diseases of the breast
User :   and FNAC also confirmed the same
Doctor :   You can go for Ultrasound guided aspiration of the cysts?
Doctor :   Ok
Doctor :   Then aspiration tests were negative for any cancer cells.
User :   after 2007 only recently I got the ultra done
User :   yes
Doctor :   Did you for Ultrasound guided aspiration of the cysts recently?
User :   but the surgeon why they have used lesion and auxillary LH
Doctor :   LH?
Doctor :   Long standing Fibrocystic disease can lead to Cancer esspecially after 40 years.
Doctor :   Do you mean lymphnode for LH?
Doctor :   Did he biopsied?
User :   auxillary lyph node
User :   yeah
Doctor :   Did he biopsied?
User :   no
Doctor :   So what he did?
User :   physical examination
Doctor :   So after physical examination he told you surgery for removal big lumps, right.
User :   yes
User :   but first surgeon told for aspiration under general anesthesia
Doctor :   I want to know was the FNAC result negative for Cancer.
User :   in 2007 its negative
User :   but now no FNAC done
Doctor :   ok
Doctor :   Now you can go for aspiration under Ultra sound.
User :   ok
User :   is it painfull
Doctor :   It need not be done under general anesthesia.
User :   ok
Doctor :   They give local injection , local anesthesiia adn put the nmeedle and drain it.
User :   but there are too many cysts
User :   2.1 x 3.6 is the biggest one at 9 O clock position on right breast
Doctor :   If they want to test for diagnostic purpose they do it under local anesthesia.
Doctor :   But if they want to remove all the painful, big cysts, they do it under general anesthesias.
User :   diagnostic purpose means?
User :   r u there
Doctor :   I think because you have very painful cysts and nodes, he want to remove the cysts and big nodes, he want to do surgery under general anesthesia
Doctor :   And that is appropriate procedure.
User :   but one surgeon is saying that he will do aspiration under ultrasound with General anesthesia
User :   but the second surgeon is saying he will remove the cysts which are bigger
Doctor :   OK
User :   so shud I go Apiration or removal
Doctor :   I prefer you go for Aspiration
User :   ok
Doctor :   From here onwards, have the annual Mammogram every year.
User :   ok
User :   how effective is MRI
User :   should i go for that also
Doctor :   If the cysts keep coming and bothering you, and if they are big , again they need aspiration.
User :   so aspiration will be on going process?
Doctor :   Mammogram and MRI are done to increase chances to detect cancer\
User :   ok
Doctor :   First we attempt to drain , if it re accumulate it needs re aspiration
Doctor :   I can understand you feelings. It is a chronic disease.
User :   yes very tensed
Doctor :   Gettng lump -nodes and cystsa are common.
User :   what is auxillary lymph node and lesion as mentioned by these two surgeon
Doctor :   Surgery will help you . it will relieve your pain
Doctor :   Feel relaxed.
User :   ok
Doctor :   There are lymph nodes all around the body.
User :   yeah
Doctor :   They are nothing but collection of white cells in nets.
Doctor :   When there is infection or cancer, thet get bigger.
User :   ok
Doctor :   So if there is cancer in Breast, the cells may go into lymphnodes in the arm pits
Doctor :   lymphnodes in arm pit are called auxillary lymphnodes
User :   ok
User :   there is swealling in the auxiallry Lymph nodes
Doctor :   so surgeon , if they suspect cancer, if there are big nodes, they cut the nodes and cut the one or two lymphnodes and see for cancer cells.
Doctor :   that is upto the surgeons discretion.
User :   shud I suspect this?
Doctor :   what?
Doctor :   Cancer
User :   am I having cancer
Doctor :   You may not have cancer.
User :   Ceftum 500 mg, combiflam, A to Z, evion 400 mg
User :   and primosa
User :   these medicines they have prescribed to me
Doctor :   But surgeon will take a sample from lyphnodes.
User :   ok
Doctor :   then only we can definitely diagnose cancer.
Doctor :   So suspecting cancer is good.
Doctor :   if it is found earlier it can be treated only with surgery
User :   ok
Doctor :   I recommend you to go for removal of both nodes and lymph nodes
User :   ok
User :   so i shud not go for apsiration
User :   last year the doctor has mentioned while they were examining me that no Lymphnodes
Doctor :   Please go for removal of both nodes.
Doctor :   Nodes=lumps
User :   ok
Doctor :   I am I cleared yout queries
User :   yes
Doctor :   Go for surgical removal of breats nodes and lyphnoed.
User :   the medicines which i am taking is usefull
Doctor :   Continie your previous medicine.
User :   ok
Doctor :   It is surgical problem
Doctor :   Surgery will fix the problem.
Doctor :   Did I answer your queries
User :   so surgery is final, right
Doctor :   YES
User :   ok
User :   but please tell the problem is not serious?
User :   shud I go for 3rd opioion also
Doctor :   No
Doctor :   Please think logically not with fear.
User :   ok
Doctor :   Doctors have some logic.
Doctor :   Simple fear for you,me and surgeon is , you may have cancer
User :   ok
Doctor :   So the only solution is ..
Doctor :   cut the nodes and lumps to identify for cancer
User :   ok
Doctor :   if the nodes are negative, your mind clean
Doctor :   Doctor feel happy.
User :   ok
Doctor :   But if it is not done , it is problem for u and me, surgeon.
User :   ok
Doctor :   So for surgical removal of breast lumps and lymphnodeas
Doctor :   You dont need 3rd opinion
Doctor :   that is waste of time
Doctor :   Did you follow What I said
User :   yes followed
Doctor :   OK
User :   surgeon have called me on saturday and other one on Monday

Doctor :   Thank you for consulting.
User :   ok
Doctor :   Get back to me after the procedure and the results of biopsy.
User :   ok
