User :   I have mild heart failure. I am seeing one doctor after another with no continuity and getting to the point where I won't go. My friend who is a nurse has given me a stethoscope to check my heart rate and rhythm. Is there a technique? I have a fast heartbeat but all I can hear is a blur of sound, my friend said she could hear all the valves and easily count my beats...
Doctor :   Hi
Doctor :   let me read
Doctor :   one cannot asses self with a stethocope, it requires the help of other person
Doctor :   what symptoms do you have at present
User :   but I'm sick. I am short of breath I have a resting heart rate of 140 I get breathless going up the stairs to the point when I have to stop and just breathe. When my heart slows down in beats irregularly...I have no ongoing care
Doctor :   yes definitely you have the symptoms of heart failure.
Doctor :   what treatment are you on now
User :   What do I do? I have to wait 3 weeks to see a cardiologist. I'm scared I might die by then
Doctor :   relax nothing happens
User :   I am sitting here with my heart like an engine...I'm breathing hard just at rest. I have no heart medications, just for astham
Doctor :   ok
Doctor :   do the worry heart has god reserve
Doctor :   it wont give away that easily
Doctor :   the anxiety will definitely put some extra pressure on the heart
Doctor :   so keep your self relaxed till you consult a cardiologist
Doctor :   also in the self monotiring keep therecord of the following daily
User :   I was hospitalised two weeks ago for ventricular tachycardia...
Doctor :   ok
Doctor :   what medication was you advised following this
User :   my heart rate right now is 160bpm
Doctor :   do you have a bp monitor machine at home ?
User :   given nothing just asthma meds...
User :   no machine just fingers on pulse
Doctor :   ok
Doctor :   may i know your weight ?
User :   60kilos
Doctor :   you are on asthama medications from how long ?
User :   20 years but bettter treatment from 18 months
Doctor :   ok
Doctor :   when you were hospitalised for the ventricular tachycardia the doctors have physically examined and have come to conclusion that a heart medication is not necessary, so only no heart medication
Doctor :   you need to control the anxiety as it is the most common cause for the heart failure to worsen
Doctor :   every day at the same time you record the pulse and body weight and the intake of salt
Doctor :   if possible blood pressure also
Doctor :   and make a note of it
Doctor :   see after a week how you have progressed and also the symptoms
User :   Have a pacemaker fitted but it doesn't seem to control fast heartbeats. Feel really bad right now
Doctor :   pace makers do controll the heart beat they are programmed to check the heart rate and allow certain limits
User :   My pulse is very faint and fast it feels uncountable at my wrist
User :   Is it normal for a carotid pulse to be very faint...I can barely feel mine
Doctor :   carotid pulse is difficult to feel and yes it feels faint
User :   another question: I lie down in bed and my heart is racing. I can't distinguish between asthma and my heart but I am breathless all night - chest working hard
Doctor :   in asthama you have the classical wheeze and in heart failure you will get breathless and if you sit up or take rest the difficulty in breathing will gome down
User :   I think it is getting worse. I feel really short of breath right now and my heart is racing but I'm not doing anything...
Doctor :   you sit up lean forward with yorr legs hanging down and near to a window and stay calm
Doctor :   in few minutes you will get better
User :   no better, I am gasping for breath, should I go to the ER I hate the repitition
Doctor :   if you are not getting better
Doctor :   rush to ER
User :   I can't rush I am hours away...what in between
Doctor :   do not waste time by thinking of possibilites, you need to reach to the ER at the earliest
Doctor :   give a call for the ambulance, call ur family members and inform them
User :   I can't and I am scared
Doctor :   do not panic, more you stay cool better it is
Doctor :   is there anybody at home
User :   No empty house
Doctor :   take the help of your neighbour
User :   Am calling emergency 20 minutes with oxygen - good and bad reports - good for me I guess
Doctor :   good for you
Doctor :   now stay calm and be in sitting position.
User :   I am so pleased for your support...sorry I am lying down and feeling very short of breatg
Doctor :   if you are comfortable lying be in that position. which ever position you are comfortable be in that position
Doctor :   keep your cool till the emergency crew reaches
User :   I am very breathless no mattter what position, but 20 minutes and I will get 02...I will demand O2 but should I breathe it for long?
Doctor :   yes u need to breath O2 till ur breathlessness comes down
User :   I'm feeling the worst ever-right now...can I talk to you tomorrow? Email? O2 need...
Doctor :   yes you can talk to me or mail me at
Doctor :   now you take rest, keep your cool

User :   wonderful men arrived am breathing o2 but heart still racing
Doctor :   it take some time to settle
User :   this is my worst nightmare. breathe 02 but still gasp heart is racing
Doctor :   are they taking you to ER now
User :   Have plugged in EKG giving 190 on my bed giving that lovely o2 but heart racingn lovely mann with stethoscope lets me listen to chest chaos
Doctor :   ok
User :   can i talk to you on spike's behalf?
Doctor :   yes please
User :   she's been sent by ambuance to hospital she has heart failure but no consistency of treatment so she is lost and alone in between acute attacks like now. Like me I am a mild CHD case but right now I have to focus t breathe as i in out etc
Doctor :   ok
User :   I hate to have to say this but I have a vwery
Doctor :   you have what ?
User :   all to do with one can imagine real lack of breatg....
Doctor :   ok
User :   have you been short of breath? Have you lived SOB? It is not good...
Doctor :   i have treated many patients in emergency ward who present with breathlesness, i do understand the difficulty of it
User :   OK they have let me type last words - very slowly because I C
Doctor :   ok
User :   get my breath. The o2 is wonderful and i am wandering around my own house with my mask on- good or bad? I'm still short of breath
Doctor :   with O2 you feel better now ?
User :   Immediately I take the mask off my chest feels like there is a lead weight on it. I have to be honext
User :   I have a pain in my chest s
Doctor :   ok
Doctor :   where are you now ?
User :   at home
Doctor :   your not been taken to hospital
User :   no they gave me o2 and asthma meds
Doctor :   ok
Doctor :   are you feling better now ?
User :   No, not reallly
User :   No not really as soon as I stop breathing o2 i fight for breatrh
Doctor :   are they planning you to shift to ER
User :   I have no consisttent doctors so I have no plan. It is reactive...
Doctor :   you need to have one consistent doctor
Doctor :   for the present acute eposide u might require an ER visit
User :   I don't and that is thet scariest thing doctor is here and makes me breathe oxygen he didn't listen to my heart or look at or touch my chest
Doctor :   wt is ekg telling
User :   i'm not good on this but have asked my friend...right axis shift the main finding but also evidence of LV enlargement...alert of sustained HR of +150
Doctor :   now you should take rest, onc you are out of this acute episode we shall continue to talk.
User :   But when I lie in bed my heart beats faster and I can't breathe . I would like to go to an ER tonight - what should I say to be believed....
Doctor :   yes u should visit ER instead of suffering breathing difficulty
Doctor :   sitting in a chair will be the most comfortable position
Doctor :   alternatively u can raise the head end of the bed
User :   But I was sent hme last do I D
Doctor :   you need to get medical help when required like the situation now irespective of whether you were sent home last week or 2 days back
