User :   hi, i was doin a bit of stretching last night and a little while after i noticed i was getting some pain in my right shoulder as time went on the pain got a bit worse and moved down my arm and back, when i was coughing it felt like i had pulled muscles in my back, i am a 25 year old female and have no medical conditions. the pain eased when i lay down for a while, i got up this morning and i still had pain in my shoulder when i lifted my arm or bent down and stretched my arm to pick something up, it is now 3.16 am and the pain in my shoulder has got worse as the day/night has gone on, i can now feel heat in my shoulder, back and arm, this is on my left side

User :   i spoke to my local accident and emergancy last night and they said it sounds as though i have pulled a muscle also the night before i was plkayin with a strong dog and it was tugging my arm while i was holdin its toy

User :   i have tried paracetamol 300mg which had 300mg asprin, they take pain and heat away after a while

User :   when i take a deep breath i feel a nipping pain in shoulder and back near shoulder blade

Doctor :   I agree with your doctor. The symptoms that you describe are very consistent with the features of muscle strain.
Doctor :   The pain and the heat you perceive over the area of shoulder, shoulderblade and the arm are as a result of inflammation secondary to muscle strain.
Doctor :   If the extent of muscle involvement and the amount of inflammation is larger, the symptoms can last much longer.
Doctor :   What you need is
Doctor :   1. Adequate rest to the affected shoulder.
Doctor :   2. Avoid large scale movement of the shoulder and arm.
Doctor :   3. Local heat fomentation
Doctor :   4. Antiinflammatory creams such as volini cream/spray can be used on the affected areas
User :   is it normal to have hardly any pain during the day, then it come on again quite bad? and can these be symptom of any heart disease/problems?
Doctor :   5. You can try Tab Ibuprofen 400mg twice a day after food for the next 2-3 days.
Doctor :   No, your symptoms aren't suggesting a heart disease.
Doctor :   The above measure should make you feel better, if the pain persists inspite of the following measures, you can visit ER for an shot - analgesics.

User :   i have, i will take my painkillers and try some gel, i have a sling for my arm, should i maybe use that for a couple of days to keep my arm rested?
Doctor :   Since your symptoms are located on the shoulder blades and not the elbow, arm pouch may not be very useful.
Doctor :   Instead, a good rest will be much more useful.
