User :   hello,I just got one of those dx that ladies don't hope to get. I saw the surgeon today who felt the same under arm area as two nurse practitioners and 1 MD felt within the last 2-3 weeks who found nothing there. The surgeon now wants to do an axillary node resection as part of a mastectomy (that, frankly, I wonder might be done another way). However I had a DPT injection about a week ago, and I am wondering if that might be the cause of the swollen node.Do you know how long these immunizations take to clear some people's systems?
Doctor :   hi
Doctor :   Dr...
Doctor :   let me read what you have typed.
Doctor :   p-lease describe your symptom in detail.
Doctor :   lymph nodes can get swollen due to vaccination if you have taken in the arm
Doctor :   it might take 1 week to 1 months time to get the inflammation cleared
User :   It's a swollen node under the left arm, the arm where I had the vaccination. I'd like to know how long they can last. I see you just answer my question.
Doctor :   do you have breast pain/tenderness/breast lumps?
User :   I have a hardened area in the breast that was negative by mammo, sonogram, and positive in 1 sample of a 6-8 sample biopsy for lobular carcinoma. But I have not had swollen nodes previously, even as recently as last week (per female MD exam). I don't want a resection when I do not need to have the nodes removed.
Doctor :   the lymph nodes should be investigated first before opting for resection. i would recommend you to get a mammogram done first along with FNAC (fine needle aspiration /cytologybiopsy) of the lymph node.
Doctor :   if the nodes show the lobular carcinoma, then you will need to get the resection done
Doctor :   along with the mastectomy for the lobular carcinoma
Doctor :   are you on any treatment at present for the carcinoma?
User :   I mean....if this swelling is a reaction to the vaccination. And thank you for the info about the node findings. You seem very knowledgeable. Do you know any surgeons who do laparoscopic breast surgeries? Also, what about getting an MRI before deciding to do a mastectomy vs. a lumpectomy. In answer to your question, I JUST got the dx a week ago, so there only treated Kaiser has offered is mastectomy, although reluctantly the surgeon I saw was willing to do a lumpectomy. I like your ideas.
Doctor :   getting MRI done would be helpful to know the exact extent of the carcinoma, and also help to see if any other structures are affected due to it.
Doctor :   it will definitely help in deciding between a mastectomy vs. a lumpectomy
Doctor :   but it should be combined with an FNAC
Doctor :   and if the FNAC shows cancer cells in lymph nodes
User :   I have fibrocystic breasts, so although the left one is hard, it is more diffuse than what I would otherwise imagine. Wow, I appreciate your support. The surgeon didn't seem to think the MRI would be that useful. FNAC is a term I do not understand. Also, can you tell me of your background?
Doctor :   then even a lumpectomy should be combined with a proper lymph node resection
Doctor :   i have explained it before. FNAC is fine needle aspiration cytology/biopsy.
Doctor :   it helps to see what i sthe cell type in the lymph node
Doctor :   and helps to identify the cancer cells if present
User :   Yes, you did. I'm not tracking all that well today, I guess. :)
Doctor :   helping in differentiating between inflammation and any other serious causes for the lymph node swellings
Doctor :   have you followed what I have said?
User :   Do you know anyone who does laparoscopie breast surgery or how to find out? Also, what would be your opinion of the surgery that leaves the skin and the nipple but removes all the breast tissue? Yes, I have followed what you've said, actually.
Doctor :   you can discuss with a cosmetic surgeon about doing a savage surgery
User :   ok...
Doctor :   in which only the affected breast tissue is taken away
Doctor :   laproscopic surgery may not be that useful in breast surgery
Doctor :   as the cancer tissues should be removed extensively
Doctor :   once the tumor is removed, depending on the extent of damage
Doctor :   you can decide upon breast reconstructive surgery
Doctor :   to get the skin and nipple back intact (if nipple is not removed in the first surgery).
