User :   Hello, i am a 31 year old male. I had never really had a problem with premature ejaculation before. I've had multiple partners and as little as 2 months ago i was able to last as long as i wanted. Just recently, about 3 weeks ago, I started have a problem with premature ejaculation as well as not keeping a full erection. What i mean is if i try to stop the ejaculation and do, sometimes i will go soft. Just yesterday I pulled out and tried to delay the orgasm. When i did, I went soft but still ejaculated. I am very embarrased by this and dont know what to do. The same thing has happened 3 times with the girl i have been seeing, and i didnt have the premature problem with another girl just 2 days prior to my last encounter with the girl that i am having the problem with. Should I see a doctor?
Doctor :   hi.
User :   hello
Doctor :   Dr. XXXXXXXX here.
Doctor :   let me read what you have typed
User :   hello , ok
Doctor :   the premature ejaculation causes can be divided into two groups psychological and organic
Doctor :   do you have problem with erection or the early ejaculation?
Doctor :   please provide your age, height, weight, details of past medical illness and treatment if any.
User :   both really, if she is on top it is really hard for me to keep the erection lately
User :   even if it goes, the erection, i still feel the need to ejaculate
Doctor :   ok. do you feel better when you are on top?
User :   31, 6'2, 181, no past medical issues
Doctor :   i will need to ask you few more questions to understand your problem better, this will take sometime. shall I proceed?
User :   yes it does
User :   yes
User :   go ahead
Doctor :   do you have problem in achieving an erection?
User :   no
Doctor :   do you have problem in maintaining an erection?
User :   sometimes
Doctor :   do you have night time erections?
User :   i think so
Doctor :   do you have early morning erections?
User :   sometimes
Doctor :   if yes what is the frequency per week of both?
User :   nighttime 5, morning 2 or 3
Doctor :   ok. that is good so the possibility of organic causes is less
Doctor :   do you have diabetes or hypertension in your family?
User :   no
Doctor :   when you started seeing this new girl, were you worried about your performance? were worried of your capability of satisfying her?
User :   not really, after the first time yes
User :   just because of my performance the first time
Doctor :   are you preoccupied with thoughts about your performance in bed each time you approach for intercourse?
User :   somewhat but i try to forget about it
User :   it just seems that my penis is very very sensitive of late even masturbating
Doctor :   ok. if you are preoccupied with thoughts about your performance in bed each time you approach for intercourse, it will affect the erection process and also lead to premature ejaculation
Doctor :   have you tried masturbating after these problems started?
User :   yes
Doctor :   if yes, are you able to hold the erection better during masturbation?
User :   at first it was the same, now its a little better
Doctor :   so your problem could be a psychological one due to failure in your performance once, and is getting repeated
Doctor :   continue t o masturbate and see if the erection time improves
Doctor :   and if it does, then all you will need is counseling sessions on how to overcome the negative thoughts
Doctor :   and perform better
User :   ok, if not see a dr?
User :   well times up, thanks
Doctor :   yes, if the erection time does not imporve
Doctor :   then you will need to be examined
Doctor :   by an urologist
