User :   I've been having soft chest twinges and light pain on the left side of my chest three nights in a row. It starts to occur at night when I sleep. It is not crushing or heavy and there is no radiating symptoms but I can feel it enough to have it keep me from going to sleep and cause anxiety that makes my heart race. Whenever I feel it and check my pulse it feels normal. I exercise everyday at a high heart rate and never feel pain or twinges during or after exercise.

User :   I am an animator - I sit 99% of the work day at a computer.
Doctor :   do you smoke or consume alcohol?
User :   I don't smoke, and I drink 1-2 drinks every few weeks.
Doctor :   ok
Doctor :   have you checked for diabetes or high blood pressure?
User :   Yes. I had a general checkup 2 months ago. Blood pressure was 2-3 points elevated but I've been taking lisinopril to counter that. Diabetic markers were normal

User :   No.

Doctor :   may I know if you consume coffee/tea, if so how many times in a day?
User :   I used to consume several cups of coffee a day but I've reduced it to less than a cup a day for the last month.
User :   I don't consume any at night anymore.

Doctor :   is this the first time your having this kind of pain?
User :   Yes, I believe so.
User :   It may have happend once or twice before but never for consecutive nights and it always went away quickly.
Doctor :   apart from this , are you worried or anxious about anything else?
User :   Yes, I have stress and take 5-htp (100mg) for anxiety. It is a more general feeling of stress however. when this occurs it feels much closer to panic.

Doctor :   I would want to suggest these things to you.
Doctor :   Since you are having a history of high blood pressure and medications for the same, it is worth to get these tests done
Doctor :   1. an ECG being the base line investigation to look for heart changes
Doctor :   2. a 2D echo to check the chambers of the heart.
Doctor :   If these turn out normal , I feel that your chest discomfort is related to the anxiety .
Doctor :   I would suggest you to do these tests, as well as keep checking the blood pressure often and keep it in control.
User :   Alright, I will.
Doctor :   and I hope you do not change the dose of the medicines without consulting the doctor
Doctor :   sometimes, this may cause symptoms
Doctor :   make sure you avoid coffee/ tea at least 6 hours before going to bed.
Doctor :   If your able to exercise at high heart rate and no discomfort, continue the same once your ECG is done.
Doctor :   since your chest pain is not very classical, I feel it is better to start with an ECG test and only if needed take it further.
User :   Alright.
Doctor :   Did you follow my suggestions?
Doctor :   other than this do you have any history of asthma or breathing difficulties?
User :   Yes. I understand. Avoid stimulants for 6 hours before bed, and get an ECG to start with to see if my heart is normal. Continue monitoring my blood pressure, and don't change my medication dosages.
User :   No.
Doctor :   yes, exactly, please follow the same.
Doctor :   since you do not have breathing difficulties or shortness of breath , and able to exercise as well, I feel an ECG is helpful first.
