User :   I was talking to another doctor here before about kidney stones. Have been suffering since october with two stones. One in each kidney. One was 7mm and the other was 5mm. Finally found out today that they are breaking up on their own. The pain is still pretty bad and constant. Pain in the flanks. Lower abdomen and in the back. Some times radiating to the scrotum.
User :   Also am still peeing blood once in awhile. The doctor and I figure that the pieces have been cutting me up on the inside.
Doctor :   can u mention the exact location of the stones ?
User :   They are just on there way out of the kidney
Doctor :   yes blood in urine and cramping pain could be due to obstruction within the ureter and this can cause hematuria
User :   cutting the ureter wall also?
Doctor :   you mean to say they are in the ureter ?
Doctor :   yes
Doctor :   any damage to the wall of the ureter can cause hematuria
User :   ok.....some pieces are in the ureter...some in the bladder and some still in the kidney

Doctor :   have u underwent any procedures like cystoscopy ?
User :   blood in the urine = hematuria

Doctor :   its a endoscopic examination of the urinary tract
User :   no...nothing yet...
Doctor :   so what were the investigations done so far ?
User :   The doctor was talking about nerve damage that is the main reason for my coming on here

User :   I have had two ultrasounds, a ct scan, and some xrays.....

User :   I am not sure.....I am going back to see the doctor in two weeks....
Doctor :   so any medications or mode of therapy that has been advised in the mean time ?
User :   yes starting on Lyrica
Doctor :   how often do u have the pain episodes as of now ?
User :   The pain is pretty constant....The more I move around the more it hurts...It feels like kidney stone pain as I have had them 6 times now.
Doctor :   do u have any neuropathy or fibromyalgia , and how long have u been on lyrica ?
User :   Sometimes the pain is so bad that I almost pass out. Have never been diagnosed with any of those. Just started the lyrica today
Doctor :   has this medication prescribed by a urologist ?
User :   No prescribed by my GP
User :   I can't see a urologist until Feb. 16th. I have been in pain since the first of October
User :   I passed one stone in October and there was two more. One in each kidney

Doctor :   any other medication for relief of pain ?
User :   was on oxycocet but the doctor wants me off of it which is understandable. Also on indomethacin and now just started the lyrica

User :   can the renal colic cause the nerve pain?
Doctor :   no
User :   thats what I thought
Doctor :   the renal colic is due to distension of the renal pelvis and ureter
User :   so I am back to not knowing what is causing the pain
Doctor :   and is not a neuropathic pain
Doctor :   the pressure of the stones on the blocked ureter causes the pain in the renal stones
User :   and the sharp pieces?
Doctor :   and can be releieved when the obstruction is relieved , and yes the sharp pieces can cause ureter damage and hence hematuria
Doctor :   the usual mode of therapy is posting you for a cystoscopy and then removing the stones and relieving the blockage

User :   yes\

User :   The doctor says there is no blockage. But there is still pain. and the pain is just like kidney stone pain.
Doctor :   but the history given mentions 2 urineay stone fragments ., how is he sure that the stones have passed ?
User :   says he saw that they were broken up with an ultrasound that I had a week ago

User :   any ideas as to the cause of the pain?
Doctor :   the pain is due to remnants of the urinary stones
Doctor :   causng obstruction
User :   thats what I tried to tell the doctor.

User :   I don't think the lyrica is going to help
Doctor :   i am not sure why the doctor s treating with lyrica, as it is not indicated in any form of renal colic
User :   and I have not seen any time that it has been prescribed either in my looking either.....what should I do?
Doctor :   you should talk to ur doctor regarding why the medication is being prescribed
Doctor :   ask him that u checked out the medication and it it is not indicated for urinary stones
User :   that is hard to do as they do not like to be second guessed. And it is almost impossible to get a second opinion as other doctors don't want to step on other doctors toes.

Doctor :   but the US legal system does help the patient and the doctor is supposed to disclose anything and everything
User :   thats the thing I am from Canada............and it doesn't really work that way

Doctor :   socialised medicine has some drawbacks
User :   ya definitely....always turns into a fight...hahahha... anyways thank you.... any idea for relieving the pain if he will not put me on pain killers again?
Doctor :   you need to take medications like tylenol PM or tramadol
User :   tramadol makes me sick...tried it already.....ummmmmmm.....the only thing that worked was oxycocet....but even it wasn't effective anymore.....
Doctor :   but its a good pain killer and good for renal colic
Doctor :   you can take it with medications to reduce nausea
User :   ya....I had a reaction to the tramadol....difficulty urinating(decreased flow) heart racing.....vomiting.....worse abdominal pain
User :   I only have two minutes left here.....I just want to thank you again for the advice.....

User :   I guess I will just ride it out and see how it goes
User :   I just hope I don't pass out from the pain and end up in the ER again.....
Doctor :   you can ask him for patches of pains of tramadol or morphine
User :   maybe I will have to move to the states to get better health care.....A morphine patch would be good right now maybe I could sleep.....
