User :   hi, my wife had UTI a couple of months ago and was hospitalised for three days. It was severe and went up to Kidney infection. Also her Homoglobin ount was just 6.8 even after blood given to her 350 ml... she also got this BP...This is the four th time she got the same disease since our marriage in last april.. Still she seems to be getting ill and there is no increase in her Blood count as we did her test again yesterday. Also went through all the Urine related tests and all aren't normal condition. Please advise
Doctor :   Hi
User :   hi
Doctor :   give me couple of mins so that I could read your query.
Doctor :   ok
User :   sure
Doctor :   can you please give me the detilas of her last urine test reports?
Doctor :   can i know her age?
User :   her age is 22
User :   I don't have any reports with me presently
Doctor :   ok
Doctor :   do you remeber the final diagnosis with regard to kidney infection
User :   also, we have been trying... and she is not able to...
Doctor :   was it anything like pyelo nephritis?
Doctor :   ok
User :   it was starts with C
Doctor :   apart from urine tests have you done an ultrasound scan or other tests?
User :   yep, also done ultra scan
Doctor :   what does the scan say?
Doctor :   do you recall anything?
User :   lower part of her stomach was little fat
Doctor :   ok
Doctor :   what is her present hemoglobin now?
User :   also she experiances severe pain when we meet
Doctor :   ok
Doctor :   was it 6.8 yesterday?
User :   let me confirm
Doctor :   is she on any other medicines apart from the one for blood count
Doctor :   ok
User :   her latest homoglobin count is 8.0
Doctor :   ok
Doctor :   there is some improvement
User :   yep
Doctor :   she may need to continue another blood transfusion and then take iron supplements for 3 months
User :   my family doctor advised me to consult a neprologist
Doctor :   but we must evaluate why is her hb low
Doctor :   yes, i agree with the doctor
User :   should I consult one?
Doctor :   a chronic UTI will cause a kidney infection
User :   I am against another blood transfusion
Doctor :   and in such cases , the functions of the kidney may come down
Doctor :   ok
User :   should I also consult a Urologist?
Doctor :   no a nephrologist will be enough
User :   ok
Doctor :   to guide you further.
Doctor :   i would suggest a few investigations to get the clear picture if they have not been done before
Doctor :   with respect to kidneys
Doctor :   1. ultrasound scan
Doctor :   2. GFR
Doctor :   3.blood urea
Doctor :   4 Ser. creatnine
Doctor :   5. ser. electrolytes
Doctor :   these roughly give an idea of the kidney function
User :   1,3 ,4,5 are already done, but will do it again when I see a neprologist
Doctor :   was she anemic before the UTI too?
Doctor :   no
Doctor :   if you have them done recently
Doctor :   you could take the same reports
User :   it was done only a couple of months ago
Doctor :   since i couldn get any details , i suggested them to you
User :   only blood test was done yesterday
Doctor :   ok
Doctor :   you may need to repeat them.
User :   is it true that untill her disease is total cured we can't have a child?
Doctor :   since her Hb is 8 now
Doctor :   yes, it is not that she cannot conceive, but since her Hb is low and has infection, better to delay
Doctor :   till she recovers complately
Doctor :   completely.
Doctor :   as anemia is again as risk factor
User :   we are not doing anything to delay but it's simply not happing
Doctor :   once you meet a nephrologist,
Doctor :   her kidney problem will be evaluated
Doctor :   and then we could take up other tests for not conceiving
Doctor :   like tube tests, some scans, etc
User :   ok
Doctor :   since when have you been trying?
User :   do you recomend any good neprologist in Bangalore?
User :   since april 2010 we have been trying
Doctor :   its not been an year
User :   yeah
Doctor :   generally in many couple we wait for one to one and years
Doctor :   only after that we evaluate both of them.
User :   ok
User :   ok
Doctor :   in this time frame 80 percent conceive naturally.
Doctor :   i will give you doctor details in a minute
User :   k
Doctor :   meanwhile you have any other questions
User :   I don't want to hurt her hence hardly have physical relation just once in fortnight
User :   as said before she encounters serious pain
User :   what could be the resason for this?
Doctor :   many females have this problem
Doctor :   especially if the foreplay has not been long enough
Doctor :   or if she has a dry vagina
User :   okie
Doctor :   it could hurt her
Doctor :   a good lubrication needs time
Doctor :   sometimes a small urine infection could also hurt her
User :   should I avoid physical relationship for now?
Doctor :   even the blood reports, her scans, previous reports and the case sumary of hospitalisation
Doctor :   if she is free from fever and symptoms
Doctor :   no need to avoid
User :   she doesn't have fever
Doctor :   but if she feels emaciated and weak
Doctor :   better to give time for her recovery
User :   k
Doctor :   did i answer all your questions?
Doctor :   are there any more queries that ican help you with.?
User :   how long will this take to get cured after having consulted the neprologist?
Doctor :   It depends on the condition
Doctor :   if it is a chronic UTI
Doctor :   it maytake 3 weeks
Doctor :   if it is Chronic nephritis
Doctor :   it may take beyond
User :   major concern is this is coming back frequently
Doctor :   but with treatment you will notice a step by step recovery.
Doctor :   yes.
Doctor :   generally UTI recurr
Doctor :   we need to properly study it further
Doctor :   and give her the suitable antibiotics.
User :   when she was hospitalised she had all the treatments.. so wondering if the treatment wasn't good last time?
Doctor :   No, it recurrs even with best treatments
Doctor :   she must follow a good diet
User :   any tips to stop from recurring?
Doctor :   and first get her Hb also normal
Doctor :   yes
User :   my family doc says not to eat fruits at all
Doctor :   1. even after recovery, she must drink plenty of fluids if it is UTI
Doctor :   2. she must report any chills, fever, body ache to doctor and check immediately with urine tests
Doctor :   3. since she has anemia, this reduces her imunity, she can pick up infections easily.
Doctor :   so she has to eat good iron containig food and take iron supplements till her Hb gets normal
Doctor :   generally given for 3 months in the form of tabs or injections.
User :   she has been having Globac Z capsules from three months
Doctor :   4. also personal hygiene is very important
User :   also Stamlo B for her BP
Doctor :   ok
User :   should she continue these tabs?
Doctor :   since she has BP
Doctor :   she has to continue Stamlo
Doctor :   never stop Bp medicines even if it is under control
User :   won't she get used to stamlo then
Doctor :   let her continue globac also
User :   after all her age is just 22
Doctor :   No, Stamlo is a very esential drug , it is a must ,if advised that she is on this.
Doctor :   she should not stop it at any cost
Doctor :   it could be only after consultation with the physician that any changes with the dose or medicine can be done.
User :   okie...
Doctor :   any other questions for me?
User :   your advise has been more than HELP for me
Doctor :   pleasure here.
Doctor :   thanks for consulting me.
Doctor :   bye for now
