Doctor:Dr. XXXXXXX here
Doctor:how can I help you?
User:needed a advise
User:am undergoing follicular monitoring as am planning conception
User:wanted to check wid u
User:is progynova tabs not to be taken during this time
Doctor:Progynova is not routinely used during follicular studies.
Doctor:Some patients with infertility issues are prescribed gonadotropin releasing hormones to help follicles grow.
Doctor:Other preparations that is used is hCG infections.
Doctor:I haven't heard of people using progynova tablets which are estrogens during this time.
Doctor:However you can discuss with your gynecologist before using it.
Doctor:Have you followed?
Doctor:Do you have any more questions?
User:my lining is 4mm on 11th day, and follicle 12.4
User:hence i was recommeded to take progynova to increase the lining
Doctor:Progynova helps in increasing endometrial size. Your follicle to me looks ok on the 11 th day
Doctor:That being said, it still needs to grow further for a healthy ovum to be released.
User:u mean it helps increasing the lining
Doctor:Estrogen acts on endometrium to increase the size of it for active implantation.
Doctor:I mean, yes it will help in increasing the endometrial lining.
User:k, but i was reading on google that progynova shud not be used whn planning pregnancy
User:is tht true
User:u thr
Doctor:Like I said, they are not routinely used by patients. Doctors commonly use hCG infections, gonadotropins
Doctor:to help in improving ovum production.
Doctor:But from your follicular scan, i am ok with the follicle development
Doctor:but there seems to little endometrial thicknening.
Doctor:therefore your doctor have prescribed this estrogen pill to you.
User:understood tht
Doctor:Since your follicles are being monitored by the doctor
Doctor:you can take this pill.
Doctor:Ideally follicle would rupture to release healthy ovum when they reach size close to 18-20mm
User:am apprehensive bcoz i read in google specifically saying progynova to be not used whn planning pregnancy
User:does it like hamper?
Doctor:if this doesn't happen your doctor can prescribe drugs the counteract progynova effects and release the ovum.
Doctor:So as long as you are being monitored, you can continue to use progynova as instructed.
Doctor:Continued used of progynova without monitoring might impair ovulation.
Doctor:But since you are monitored by your doctor, you can take them.
Doctor:Hope it is clear to you.
User:k, so no harm
Doctor:In your case, you need not be worried.
Doctor:Do you have any other query?
User:because last month also everything went off well
User:everything folllicle ruptruing, right lining, right timing to try
User:but it didnt happen
User:argipreg also does the same thing?
Doctor:Are you on assisted fertilization or are you trying naturally?
Doctor:As I see Argipreg are supplements and vitamins.
Doctor:They are not known to have any side effects
User:follicular monitoring is assisted right?
Doctor:Follicular monitoring helps your doctor time the right days of ovulation.
User:she has prescribed me argiperg, ecosprin and progynova for lining
User:since my lining was 4mm only
Doctor:When I meant assisted, I meant whether are you trying naturally or have your doctor suggested IVF or any other alternative techniques?
Doctor:Your doctors seem to be treating you rightly.
Doctor:please continue the measures.
User:no just advising on right daya with FM and supported by some tabs
User:last month she had advised me to continue progynova till my next cyscle
Doctor:It is not uncommon to have a failed result even when all the factors are right - I mean optimum endometrium, optimum ovum release and all.
Doctor:And we only encourage you to keep trying.
Doctor:If you don't succeed after repeated trials, say after 6-8 trials, then you can discuss with your doctor about other assisted techniques.
User:and can u ensure good quality ovum by taking some measures?
Doctor:At this point, I would say your doctor who has been recording your progress, knowing your health completely would be the right person to treat you.
Doctor:My opinion would be biased since I only have partial information about your problem.
Doctor:I can only say that you are being rightly treated now based on your follicular study.
Doctor:Please continue the treatment.
Doctor:if follicle rupture doesn't happen
Doctor:your doctor can prescribe you drugs that counter the effects of progynova
Doctor:and release the ovum.
Doctor:Hope it is clear to you.
Doctor:Do you have any other questions?
User:n use of progynova in continuation even after trial till the next cycle or conception is also fine?
Doctor:As i said earlier, I concur with the use of progynova as the current endometrium is not optimal.
User:am saying after
Doctor:Yes. That is fine.
Doctor:You are welcome.
Doctor:Do you have any more questions?
User:with this lining or follicle it will not make sens to try tomrw the 12th day?
Doctor:You need to wait for some more time.
Doctor:Follicle size at 12mm is not likely to rupture now.
Doctor:It should grow few more mm;
Doctor:it would rupture as it reaches close to 18mm
Doctor:Proceed with the follicular scan
Doctor:and you will know the right time to try.
User:ya because tomrw eing sunday, there shall be no fm
User:hence i was asking
Doctor:Do you have any more questions?
User:after my apprehension doc also removed the progynova
User:and said tht we will see by Monday if lining is good by thn otherwise we can add progynova on 13th day that is monday as well
User:one day progynova at tht time can help?
Doctor:You can work with your doctor and plan the treating rightly.
Doctor:As a physician, I do not have enough experience to comment if one day of progynova would be sufficient or would you need more.
Doctor:I can only say that if endometrium isn't optimum and your follicles are still immature
Doctor:then you can safely continue using progynova
Doctor:There are drugs available to counteract progynova
Doctor:which can be prescribed to you when follicles become mature.
Doctor:Hope it is clear.
Doctor:Do you have any more questions?
User:that means progynova blocks rupturing as per u?
User:counteractimg progynova whn follicles mature means?
Doctor:It has tendency to prevent ovulation
Doctor:*I mean it has potency to prevent ovulation.
Doctor:We call follicles after 18-20mm as mature follicles.
Doctor:Mature follicles contain healthy ovum.
Doctor:Do you have any more questions?
User:hucog is wht counteracts progynova?
Doctor:Hucog is a gonadotropin.
Doctor:It helps to grow and mature follicles.
Doctor:It is prescribed to ladies who have problems with follicles.
User:so even if i dont take progynova, i might hv to take hucog?
Doctor:it depends.
Doctor:If ovulation isn't happening, your doctor might choose to prescribe you hucog or another drug called estrogen receptor modulator.
Doctor:I think you should be looking at it right now.
Doctor:*you should not be looking at it right now.
Doctor:Please continue the medicines prescribed to you. Visit your doctor again as instructed
Doctor:she can advice you whether we can use progynova or can it be withheld.
User:she gives me hucog after 12th day if follicle increases to 20mm in every cycle
User:issue is tht its too much of drugs
User:why shud hucog be given in every cycle after follicle increases beyond 20
Doctor:I am sorry that you are being subjected to too many medicines.
User:cant it be left to natural rupturing
Doctor:But as a second doctor, without complete knowledge of what is happening with you, what are your current medical problems and tests that is done
Doctor:I wouldn't be the right person to judge whether you are being rightly treated or not.
User:ya i understand
Doctor:There are cases where ovulation has happened without medications
Doctor:and there are cases where people needed medications to release ovum.
User:but is hucog necessary or not, how do we know
Doctor:Since the chances of ovulation can be unpredictable in some cases
Doctor:your doctor is most likely not leaving it to chance.
Doctor:And as a second doctor, more importantly as a general physician
Doctor:I would not like to comment whether or not you should take those extra drugs.
Doctor:You may discuss about the whole concern with another specialist nearby and take a call.
Doctor:I hope you understand.
Doctor:If there are no more questions, can we end this chat.
User:last query pls
User:also read progynova increases chances of strokes and breast cancer in women
Doctor:It has problems on the long run.
Doctor:If you have no previous history of strokes and deep vein thrombosis or family history of cancer
User:it does?
Doctor:you can take them briefly in this case.
User:my father has undergone a by pass
User:maasi had breast cancer
Doctor:I would put across this way
Doctor:people who have risk factors for heart illness and breast cancer need to be screened regularly for illness.
Doctor:A brief use of prognova rarely raises those risk factors to significant levels to cause such problems now.
Doctor:I would be concerned if you are going to be on this drug for a long time.
User:by long time u mean?
Doctor:Even for this matter, I would be very concerned if you are going to be on any other estrogen containing pills for long time - months and years together.
Doctor:Short duration use as in this case hardly raises those risk bars.
Doctor:so I would not prevent you from using it for few days.
Doctor:Do you have any more questions?
User:thats all
Doctor:Thanks for consulting me.
Doctor:Bye for now and Good luck
