User :   My son becomes very sick whenever he eats food. He lives on the medical formula Elecare. Our GI has ruled out EE and a whole list of other GI disorders. They are suggesting we go to a peditric in patient feeding clinic....but he's only sick when he eats. This happens 4 hours after eating.
Doctor :   Hi
Doctor :   Let me read youe query
Doctor :   Ok
Doctor :   His age?
User :   He's two (June 12, 2008)
Doctor :   ok
Doctor :   What other feeds is he comfortable with
User :   He sometimes licks a pickle or will bite and chew a cracker - however, spits these out and doesn't swallow
Doctor :   Ok
User :   He enjoys licking chocolate, but again does not swallow
Doctor :   I will suggest you to reward him after he consumes a normal meal
User :   I think he's learned that when he swallows = he gets sick. Diarrhea, vomiting and many times aspiration pnuemonia from vomiting
Doctor :   We should rather encourage him to have some extra feeds othe rthan elecare
Doctor :   Ohh
User :   Yes - he receives feeding therapy three times a week
Doctor :   So Elecare is wise to continue
User :   but - I must have you understand he is only sick when he has eaten something
Doctor :   Then he will require extreme caere
User :   Last Thursday in therapy he took 5 bites of pizza crust
Doctor :   What about liquid diet
User :   We just came back home from the hospital last night
User :   I do have a juicer and he accepts sweet juices
Doctor :   It usually seen in 10- 15% of individuals
User :   Is there a name for something like this?
User :   Is there an enzyme missing?
Doctor :   the condition he faces should be gradually adjusted from day care and medical help
Doctor :   Yes, rather a dificulty in digestion
Doctor :   These are more of development related issues concerning his GI tract
Doctor :   Which usually resolves by self with slow practise and training to adapt normal food
User :   We have an important doctor visit on Monday with a developmental pediatrician. This doctor has been very kind and helpful. I'm just wondering what our next step should be? More testing? Travel out of state to a specialty hospital? Start a new therapy or medication? Can you advise?
Doctor :   Is your son lactose intolerant?
User :   Milk Protein Intolerant
Doctor :   Ok
User :   yes
Doctor :   He would require more testing and workup
User :   What type of testing and workup?
Doctor :   On his GI related to causes for in acceptance for food
Doctor :   It more specifically in the normal functioninig of GIT rather morphology
Doctor :   Its usually governed by development studies
Doctor :   Was he diagnosed of any syndromes other than lactose intolerance
User :   We've had two endoscopes, biopsies, ph probes, colonoscopies, upper GIs, lower GIs, colonoscopies, bloodwork, urine tests, swallow studies, motility testing.....What is this testing you are referring to?
Doctor :   Oh
Doctor :   All the tests are already done
Doctor :   I thought only basic workup was done
User :   Yes - we've had these tests already Dx with severe GERD and esotropia
User :   in left eye (surgery corrected that)
Doctor :   What were the inferences
User :   had a hernia surgery too
Doctor :   ok
User :   the above scopes only showed some damage in mid-esophogus which cleared after switching him to elecare
User :   The other testing was all negative
Doctor :   He then requires help from a specialist Developmental pediatrician.
User :   That's the appointment we have on Monday
Doctor :   As in GERD we have the findings suggested
Doctor :   I
Doctor :   in your case i have a very few advises to share
