Doctor :   Hi
User :   hello
Doctor :   How can I help you?
User :   what is your specialty
Doctor :   I am a general physician.
User :   i am a 21 year old female with urinary issues
User :   my doctor isnt helping me
Doctor :   Can you explain your symptoms?
User :   First of all thank you for helping me with this. I took both a urine test and a pelvic exam at the doctor because I had been experiencing constant urges to urinate. I was negative for yeast infection and I thought the gyno had told me that I had a slight bladder infection, and she did prescribe me antibiotic pills and also a cream to help with the itching if I recall. The thing is, I called her today because the symptoms had subsided here and there but definitely returned the day I took the last pill. She told me that when they had checked the culture, I was actually negative for a UTI. I don't know why she still gave me the meds though, maybe just in case and because I had the symptoms. But that one day I had the tests she still said that I had a bit of a bladder infection and that there was a little blood in the urine. I am going to tell you a little backup information that might be of use. I actually had to get the urine sample twice. The first time I took it but had forgotten to use the sanitary wipe prior. I do not know if they used this sample, but when I looked at it, I didn't see any blood. I later had to take another sample (for which I used the wipe) which was rather a small amount obviously, and this one didn't seem to have apparent blood in it either. But the thing I mainly wanted to note was the first time I went in the restroom to get the first sample, there was fresh blood on my pantiliner. I couldn't tell if it was from the urethra or vagina. Don't think the doctor really was able to tell either, although I think it might've been residue blood from my last period a couple weeks prior (I guess that's normal). So I'm not sure if this helps any like if it is relevant, but there you go. Aside from the urges (which I THINK can be described as a sort of pressure in the bladder to "go" and sometimes a feeling in, I believe, the urethra area), I get this strange feeling oftentimes when I experience somewhat intense feelings. I get some sort of tingling feeling "down there", I cannot tell where exactly it is from (such as the urethra or any other area). If anything it is around there, though, and doesn't seem around the vagina. Examples when I feel this would be when I am startled by something, burn my mouth with hot food, feel other varieties of rather "sharp" pains (maybe accidentally touch an injured sensitive cuticle, etc.), even possibly when I get sudden itches or something, but not all the time. Another strange incidence is when I clean my ear with one of those wooden ear cleaner things. It's very sensitive in the inner ear though, which probably accounts for the feelings. I'm not sure if the feeling I get is the same every time it happens, but I know it is something down there. Very unusual, I've never had this symptom before. Other than these symptoms there isn't really anything else that I believe I need to mention, at least for now. So the gynecologist hasn't really been able to identify what I have, for now. But it apparently isn't a UTI, which unsettles me. I am going back there this Thursday to take another urine test to make sure. But still, what if it isn't anything? What am I going to do? Thank you for the help. My mind will never rest... Please ask if you have any other questions. oh and the night before i finished the pills my gyno gave me i had one of those wet dreams..

User :   nope that i know of
Doctor :   Ok
Doctor :   Let me explain to you.
User :   ok
User :   i appreciate your help doctor
Doctor :   * I mean let me analysis your issues.
User :   my gyno is not helping me
User :   ok
Doctor :   You seem to have multiple issues
Doctor :   1. Increased urge to urinate - this is probably related to an UTI.
User :   my gyno said no uti though
Doctor :   Urine culture test can be negative, if the sample of urine was taken with aseptic precautions.
Doctor :   The fact that your symptoms were better with antibiotics is one proof of it.
Doctor :   Hence this can be possibility.
Doctor :   The other conditions of increased urge to urinate are as follows.
Doctor :   a. Stress and anxiety
User :   ok so maybe i should get tested again
User :   do u think i should go to my regular family doctor instead of my gyno
Doctor :   b. Uterine conditions such as retroverted uterus - which is ruled out by pelvic examination
Doctor :   c. Medical conditions such as diabetes.
Doctor :   So along with a repeat urine test you probably may also need to check your blood sugars.
Doctor :   Have you followed?
User :   yes
User :   ok
User :   so
Doctor :   Now your next symptoms
User :   my friend is a physician assistant
Doctor :   The on and off itching -
User :   he said what u said
User :   about the antiobitoc
User :   and that maybe the doctor prescribed a wrong antibiotic and could give a different one
Doctor :   If your symptoms are better with the antibiotics prescribed, then you the antibiotics given are definitely effective.
Doctor :   Most often we do see these in clincical practice.
User :   my doctor gave me ciprofloaxin
User :   and my friend said thats not a good "first line" for utis
Doctor :   Antibiotics proved ineffective in a culture and senstive test can be effective when used in the treatment.
User :   do uknow of something else i should ask for
User :   now for the itching it sort of went away lately
Doctor :   Yes, ciprofloxacin isn't the first line of drug, for UTI. A broad spectrum antibiotic, or tetracycline group of antibiotics are generally used.
Doctor :   However since you report of feeling better,
Doctor :   One cannot disagree that ciprofloxacin has worked effectively in your scenario.
Doctor :   Hence do not worry.
User :   ok
User :   so
User :   for everything all these problems
User :   what should i do
User :   what is ur reccomenation
Doctor :   The repeat test is basically needed to see if you need further antibiotics.
Doctor :   So go ahead and get a repeat urine analysis done and also get your sugars checked.
Doctor :   That should solve your first/primary concern.
Doctor :   Now on and off itching in the genitalia can occur due to muliple reasons too.
Doctor :   The commonest of these are
Doctor :   1. Excessive vaginal dryness
Doctor :   2. Stress and anxiety factors
Doctor :   3. Medical conditions such as diabetes, hypothyroidism
Doctor :   4. An infection.
User :   ok
User :   so
User :   shoul di tell my doctor
User :   not to use aseptic test
Doctor :   Since I am unable to physically examine you, I cannot pinpoint the exact cause of that could be causing the on and off itching and tingling sensation.
User :   so the tingling could that be related to the uti
Doctor :   When collection the urine sample, the perineal region needs to be cleaned with aseptic solutions.
User :   ok
Doctor :   you need to collect the mid stream urine and whilst you seperate the labia.
Doctor :   This urine sample are the best samples for the diagnosis of UTI.
Doctor :   A negative report on this sample either means that you are presently not infected
Doctor :   or you didn't have the infection at first place and probably the other conditions may have been the likely cause.
Doctor :   Have you followed what I have suggested?
User :   wait
User :   the hwole area?
Doctor :   Yes.
