User :   I got implanon 6 months ago and I didn't stop bleeding so i went to the doctor and he gave me 'primolut n tablets' which is norethisterone for 3 times a day. he told me while i'm on the pill for 10 days, i need to have condom while i'm having sex. i forgot it and i had sex without condom. do i have to get morning after pill???
Doctor :   Hi,
User :   Hello!
User :   this is long story...
User :   I got implanon 6 months ago..
User :   and i didn't stop bleeding for 6 months so i went to doctor and she gave me normal pill.
User :   and it didn't stop and next time, another doctor gave me stronger pill.
User :   he told me to take it 3 times a day.
User :   and i lost it. so i went to another doctor (because he was booked and it was urgent)
Doctor :   Ok,
User :   anyway before that, that male doctor who gave me stronger pill
User :   he told me to have another contraception such as condom while i'm on the pill even though i have implanon
User :   anyway after 2 days of having it, i lost it.
User :   so i went to another doctor and he gave me 'primolut' which is norethisterone.
Doctor :   Ok,
User :   not sure if it's same one that previous doctor gave...
User :   and i had a sex without condom today.
User :   shortly, i have implanon on my arm, on norethisterone pill.
User :   do i have to have morning after pill??
User :   or is it safe??
User :   i feel like i'm putting too much hormone in my body and i don't feel quite safe
Doctor :   I do not see any need for a morning after pill
Doctor :   yes , do agree with it , do avoid hormones over what you are taking
User :   but then why the second doctor who gave me stronger pill told me to have another constraception while i'm on the pill??
Doctor :   Since you are under the cover of implanon , i see you safe
User :   no but he told me while i'm on this pill, the implanon won't work.
Doctor :   he would had thought that the implanon is not sufficient in you and hence added with more progesterone to strengthen the contraception
Doctor :   and since the Primolut takes time to attain steady concentration in the blood he would have asked for extra cover
Doctor :   But , i think the implnon is ver efficient in providing contraception
Doctor :   very*
User :   that's what i think too.
Doctor :   the failure rate with it is 0.05% according to the records
User :   but what he said was while i'm on these pill, implanon won't work which implies that implanon will stop work from the day i'm on the pill.
User :   implanon itself is sufficient. he said this PILL will stop the effect of implanon.
User :   and one more question here. if implanon is made of progesterone, how come putting more progesterone in my body makes my bleeding stop???
Doctor :   Its via feedback inhibition
User :   so i'm back to zero.
Doctor :   I will explain it to you
User :   okay. thank you.
Doctor :   the excess of norethisterone will block/use all the receptors ( the site at which the hormone acts ) leaving none for progestin( etonogestrel) present in the implanon nullifying its effect
User :   i'm sorry i'm not quite getting it, are you saying norethisterone is nullifying implanon's effect?
User :   if that's true, i need to have morning after pill, right?
Doctor :   May be he wanted to treat you for the continuous bleeding you had (which could be a expected sideeffect of implanon ) by putting you on a different kind of progesteron ( norethysterone) and inhibit the effects of the progestin in implanon and lessen the sideeffect
Doctor :   I am sorry to say nullify ... its minimize the effects what is right word to use
User :   i went to him for that, to stop bleed.
Doctor :   The purpose of the norethysterone he had on you , is explained to you by me i think
User :   yeah.
User :   i'm just reassuring it.
User :   so... since this pill minimized implanon's effect,
Doctor :   and also i would say that the effect of the implanon might have been minimized to a extent not to have sideeffect like bleeding , which may not hamper its prime function of contraception
User :   yes. i understand that. but since these pills minimized the effect of implanon,
User :   i could get pregnant as well if i don't use another contraception.
Doctor :   No , the effect of implanon would have minimized but not to the extent that the contraceptive activity of it is nill
User :   oh you explained it already. sorry. i didn't read properly.
User :   then why would he want me to have another contraception??
User :   that's the part i don't get.
Doctor :   just to be on safer side
User :   so are you 99% sure that implanon will do it's main function as contraception in my body right now??
Doctor :   very rarely in conditions where the norethysterone levels get higher than expected the inhibition of implanon may be more resulting risk on you
Doctor :   i would say thats a rare situation to encounter
Doctor :   yes, i am 98% sure
Doctor :   Bye
Doctor :   Take care
