User :   i have had problems with e-coli in my urine for the past decade. i do everything right believe me - wipe front to back, never have anal sex (can't even if i wanted to - hurts too badly), always urinate after intercourse plus drink a full glass of water, and i use 2 different wash cloths for my urinary/vaginal area and my anal area. so i have NO CLUE how in the world this kept happening to me with the e-coli. however, the past 2 cultures tested positive for klebsiella. i am on septra since i found that macrobid didn't work against the klebsiella (for me anyhow). so, my doc told me after intercourse to not only urinate, drink a full glass of water, but take a shower plus a macrobid pill. i just fear there is an underlying problem. what am i doing wrong? more importantly, what tests can i ask for to find out? fyi, i've had just about every blood test known to man CBC, blah, blah...and everything comes back it's not like I have a low immune system according to the usual tests. i've been to a urologist many years ago, who put scope in my urethra - found nothing. this is so frustrating and worrisome. i would love some guidance.
Doctor :   Hi
Doctor :   Dr
Doctor :   let me read your query
Doctor :   ok
Doctor :   from the history provided it seems that you are doing everything by the book
User :   yes b/c i hate these uti's
Doctor :   but the occurance of UTI recurrently is a matter of concern
Doctor :   have you done any pelvic ultrasound ?
Doctor :   or a CT scan ?
User :   the last pelvic ultrasound would have been about 3 years ago. i've never had a CT scan on my pelvic area.
User :   are you thinking it could be in kidneys??
Doctor :   did the condition worsen after the last ultraosund ?
Doctor :   yes, the chances that the foci of infection could be in the kidney is high
User :   i had an ultrasound to see if i was ovulated, to rule out polycystic ovarian disease
User :   so i don't know if that was technically a "pelvic ultrasound" LOL
Doctor :   you actually had a pelvic ultrasound, by the description
Doctor :   you may have to get a KUB ultrasound (Kidney , ureter, and bladder )
Doctor :   this might reveal any foci of infection
User :   but okay, so would a urologist be able to determine if it was in my kidneys? or should I skip to the urologist and just ask my GP to order a KUB ultrasound AND a CT scan or just the KUB you think?
Doctor :   also, once the culture is positive for klebsiella, a prolonged duration of therapy for about 4-6 weeks might be necessary
Doctor :   first you can ask your GP to do it
Doctor :   then if there are any findings that need a urologist for help, then your GP CAN refer you one
User :   okay i will ask for the KUB ultrasound, should I also ask for a CT scan?
Doctor :   first get a Ultrasund
Doctor :   then if needed a CT may be done
User :   i certainly will, thank you. forgive my ignorance, but how will an ultrasound determine an infection? will they fill me full of some fluid and see how it digests or something??
Doctor :   yes, it will find any abnormaility or a cyst , in the kidney
Doctor :   this might be a foci of infection
Doctor :   they wont fill u with any fluid
User :   i see. are you a man or a woman?
Doctor :   I am a Male, doctor, Why ? Does it bother you ?
User :   not at all. we southerners like to say thank you sir.
Doctor :   ok ,
User :   i wasn't sure which to say
Doctor :   thanks you, and you are most welcome
User :   one more thing. let's say that it's not my kidneys. is there anyway that it could be caused from my boyfriend's semen?
Doctor :   no
User :   for one thing, i don't even know where on Earth klebsiella comes from or how i got it.
Doctor :   there is no way that semen can cause a UTI
User :   dang, i wanted to blame him! lol
Doctor :   i understandd the feeling
Doctor :   sorry about that
User :   so if it is an abnormality in my kidney, then my guess would be that klebsiella bacteria is found in everyone, but their kidney's filter it or something, but mine doesn't?
Doctor :   Is there anything else i can help you with ?
Doctor :   no its not about the kidney to filter the bacteria
Doctor :   the bacteria are very small and can easily pass thorugh the kidney
User :   but what is wrong with me that i have that bacteria in the first place reaching my bladder?
Doctor :   there should be a abcess within the kidney
User :   ah so the abcess is producing the bacteria and passing to my kidney
Doctor :   yes
User :   perfect thank you sir!!!!!!!!!!!!
Doctor :   you are welcome
