Doctor :   H
Doctor :   ok
Doctor :   may i know your age and the duration of your diabetes ?
User :   i am 60 yrs and 20 years diabetic
Doctor :   Can u tell me your HBA1C levels ?
User :   it is 8.5
Doctor :   your A1c IS VERY HIGH
Doctor :   and accordingly your sugar levels are high
Doctor :   diabetic control - has 3 important needs
Doctor :   1. diet control
Doctor :   2. lifestyle changes and physical excercise
Doctor :   3. medications taken regualarly
User :   yes i want to bring it down and i walk 30 minutes and have a perfect diet control. I get my stomack inflammated after eating. I take moza 5 or pantacid
Doctor :   when all 3 aspects are takn care of, the balance will help to control the sugar levels
User :   Can i take januvia? is it safe. presently my creatinine is 1 and urea is 25
Doctor :   HOW often do u check ur blood sugars ?
Doctor :   yes
User :   twice daily at home
Doctor :   januvia (sitagliptin) is a good medication and can help
User :   other drugs I take are listril 5, korandil , irovel 150 and clopilet. will glucored and januvia go together with novomix 30?
Doctor :   In addition, the insulin regimen has to be titrated depending on your daily sugar levels
Doctor :   haw long have u been on the same regimen ?
User :   januvia and janumet are costly and is any other drug of the same family which is cheaper? about 10 yrs I am taking the insulin
Doctor :   for effective control, the insulin dose needs to be titrated every 1 or 2 weeks
User :   what is the meaning of titrated
Doctor :   both are new drugs and cheaper versions are not yet produced
Doctor :   titrated = dose changed regularly depending on sugar levels
Doctor :   how often do u see your doctor ?
User :   can i try daonil with janumet or glucored forte with januvia? I am seeing doctors very often but not helped much and even this week i have been to three doctors and I am sorry that i am not helped and one suggested jalra 50 , other volix and other pioz and all these tried for two or three days but not successful.
User :   will it help if take insulin thrice than twice and what dose?
Doctor :   please understand that u need at least 1-2 weeks so that the medication can work
Doctor :   constant change of medications will not help you controlling your sugar levels
User :   that is a good point and tell me doctor which medicine i can take from today for a week or two
Doctor :   can u tell me how long have u been on the current regimen of novomix 30 and glucored forte ?
User :   among all these medicines which one can i take - pioz or jalra 50 or volix and i want to take combination which will not affect kidney or other strong side effects
User :   I will do if you will tell me the best medication without much of side effects
Doctor :   most medications are effective and each is different for different patients
Doctor :   in your case, long standing Diabetes could be associated with insulin resistance and hence you may need higher doses
Doctor :   glucored forte helps to control sugar levels very well
Doctor :   in case you have been on it for a shorter time, you should continue on it for a longer time so that the drug can work well
Doctor :   over time, if needed the dose can be increased to help you
Doctor :   but it is wrong to change the drug altogether itself
User :   See I am asthmatic ( only inhaler) hypertension under control and diabetic. You think volix will do good and what is the dosage and gulcored forte itself is not helping along with insulin 30 units am and pm. I am taking glucored for the past some years. I want to be helped
Doctor :   are u following my advice ?
User :   I do see that i should not change the drugs and take for a long time. my problem is that sugar level is not getting controlled.
Doctor :   whats the dosage of glucored forte u r on currently ?
User :   Just because I had to stop rosecon 2 because govenment banned my problem started. I am take glucored twice.
Doctor :   volix comes in 0.2 mg & 0.3 mg tablets.
Doctor :   Voglibose (VOLIX) is indicated for improvement of postprandial hyperglycemia in diabetes mellitus only when diet and/or exercise or oral hyperglycemic drug or Insulin preparation in addition to diet and/or exercise, does not result in adequate glycemic (Glycemic) control, which is the case with you
User :   Please tell me the dosage of volix and which one .3 mg and how many times i should take apart from glucored forte twice and inslulin twice 30 units
Doctor :   But before this medicine can be started, we have to have your sugar levels, checked 3-4 times in a day, so that we can know when u need the proper dosage
User :   Last night my reading was 150 after 2 hours of meal but this morning it went up to 250 why?
Doctor :   Without having the details of your sugar charting, for over a week, it will be unethiical and illegal for doctors to prescribe the dosage online
User :   last night i took volix, glucored forte and insulin as usual
Doctor :   the reason for this is coz, u might be needing a lower dose of the medicine
Doctor :   when u take a higher dose at night, your sugar levels tend to become lower at night, when u sleep (around 4 am)
Doctor :   at that point the body releases hormones that increase sugar release
Doctor :   and this appears as a high sugar levels in the morning
User :   I am giving you the charts - 150 - 190 fasting and 260 after food. what is COZ please
Doctor :   i meant because
Doctor :   i would advice you to do the following
User :   what to do to stop to get the hormones not to shoot up
Doctor :   maintain a log of your sugars for a 2 weeks
Doctor :   check your sugar levels 4 times in a day
Doctor :   1 - early morning before breakfast
Doctor :   2. 2 hrs after break fast
User :   go ahead please
Doctor :   3. pre and post lunch sugar levels
Doctor :   and finally
Doctor :   4. pre and post dinner levels
Doctor :   chart these levels for 2 weeks
Doctor :   and once we have these levels , this chart will guide us on further change of your medications
Doctor :   also i need more info , on what your diet consists of and your total calorie intake as well
Doctor :   have u followed my advice
