User :   Great, Doctor, my mother was to be shifted from ICU today, but unfortunately, serum creatinine level has increased now to 3.5 and breathlessness has increased with both hands becoming swollen

User :   Is it possible to advise what could be the possible reason and if it is possible to control the creatinine levels

Doctor :   Both kidney functions and heart functions are regulated by few common factors.
Doctor :   One of them is the the blood volume and blood pressure.

Doctor :   The cardiac output and the blood pressures can be affected due to an acute heart event like an myocardial infarctions.
Doctor :   This along with fluid changes may have brought about the worsening of the kidney functions.
Doctor :   Another theory may be due development of infections.

Doctor :   Myocardial infarction is the medical term for heart attack.

Doctor :   Since I am unable to perform a physical examination, i cannot predict which of the 2 causes have brought in the worsening.

Doctor :   This will depend on the cause.
Doctor :   She may need oxygen to assist breathing, diuretic drugs to remove excess fluid that may have accumulated in the lungs, treatment of infections if any.
Doctor :   She will have to be monitored - both heart and kidney functions( creatinine, blood urea and potassium levels).
Doctor :   Dialysis is the option if feasible and if there is worseining.
