User:I have pain
Doctor:Since when do you have pain?
User:Hello doctor
Doctor:and where is the pain located
Doctor:Is there a concern I can address?
Doctor:Is there a question that I could help answer?
User:Dr I have a low back pain on my left side. I underwent an uretic stone surgery in the year 2007. I am taking Zyrolic one tab for the last 4 years. What do you suggest for me
User:Doctor are you there ?
Doctor:I am here
Doctor:I understand
Doctor:there are a few things that you should be doing
Doctor:First, is taking plenty of water
Doctor:Here, we would be talking about 3-4 litres of water in a day
Doctor:Secondly, it would be avoiding certain foods
Doctor:These would include non vegetarian items, very high protein things like milk, cottage cheese
Doctor:Also, spinach, nuts etc should be avoided
Doctor:These measures will help reduce the recurrence of stones
Doctor:Thirdly, you should also be taking about 1 cup cranberry juice daily
Doctor:Since people with stones are prone to infections of the urinary tract
Doctor:the juice helps prevent infections
Doctor:Also, if your uric acid levels are normal
Doctor:We can consider weaning you off the drug
Doctor:Actually Zyloric does nothing but reduce the uric acid levels
Doctor:So if we stabilize the uric acid levels by other means
Doctor:The need for the drug reduces
Doctor:Do you follow?
User:Please find my observations:
User:Pleae find my observations - I have advise not to take water more than 2 litres a day. Also, not to take any high ptotein food i.e chicken, meat, etc. I am also suffering from cardiomyopathy and also takes other medicines.
Doctor:Did they specify the cause for the cardiomyopathy?
Doctor:Also, have any investigations been done for you like Echocardiogram etc?
Doctor:If yes, whats the ejection fraction?
Doctor:Are you there?
User:yes, 67%, it is due to overweight
Doctor:The ejection fraction is normal
Doctor:If there is such an issue, the water intake needs to be restricted
Doctor:But other measures need to be followed
Doctor:also, regular medication for pain control like diclofenac etc
Doctor:which is often taken for pain in ureteric stones
Doctor:needs to be taken only under supervision
Doctor:It interferes with the compensatory mechanisms that kick in during such an issue
