Doctor:I am Dr XXXXX
User:I have problem of sinus, and had gone for CT scan and blood tests
User:My blood test says that my ESR is 18 mm/Hr and my Total IgE is161 IU/ml
User:this seems to be beyond normal range
User:and hence would need opinion if this is what my sinus problem may be related to
Doctor:IgE is an immunoglobulin that helps fight some infections.
User:could you please give your opinion on results ?
Doctor:It is usually raised to fight skin infections from a particular bacteria called as staphylococcus.
Doctor:When a skin infection is present ,that time it is called as Hyper-IgE syndrome.
Doctor:It may be raised for some time to fight allergies and hypersensitivity reaction.
Doctor:That time it is only found for some time and then it may go away.
Doctor:Your ESR looks ok.
Doctor:It is not raised.
Doctor:Thus you may not be having an infection but an allergy may be there.

User:I had been suffering from common cold and mild ashthma since childhood
User:then the problem was gone for last 10 years
User:lately I shifted to Bangalore, and after an year I again started having severe cold, blocked nose, and mild respiratory problem
Doctor:Asthma is a hypersensitivity reaction.
User:I was told to take fluticasone nasal sprays which was corticosteroid and Montek-LC which relieved me from the problem for some time till I took these medicines
User:When I stopped taking these medicines, I again started having blocked nose and sneezing problem, along with respiratory problems after a month
User:In my CT scan, it shows DNS and left maxillary sinus .. I took few medicines like Medrol, Glemont F and Becozinc for 10 days, and was relieved
User:now my blood test report came and shows this IgE value of 161 .. hence needed an opinion on what should be the future action
Doctor:Allergy and asthma are similar conditions and the treatment can also overlap.
Doctor:One thing I want to tell you is that DNS is normally present in most individuals.
Doctor:But it can become a discomfort in blocked nose.
Doctor:Your sinusitis is most probably being caused by allergy.
Doctor:Thus you may take fluticasone nasal spray as and when needed to get rid of blocked nose.
Doctor:It may help you with sinusitis as well.
User:ok, am still using it once a day
Doctor:You may take pain-killers for the pain of sinusitis.
User:ok, I would like to avoid surgeries and would want to correct this problem through medications only
User:is there anything I should do for the allergy problem ?
Doctor:If you get symptoms of asthma, the you may take inhaler bronchodilators
Doctor:I think the montelukast you were taking may help you.
User:ok, I normally use Aerocort inhaler when I have symptoms of asthma, and it relieves me
Doctor:Otherwise it is taken care as and when it happens.
Doctor:I think you are doing fine on medication.
Doctor:As you know how to use it.
Doctor:Talk to your physician about taking montelukast.
User:ok, so I should use these medicines as and when the problem occurs
Doctor:Yes that is the best way to go.
Doctor:which surgery are you talking about?
User:Few ENT related surgeries like septoplasty, sinuplasty
Doctor:You may go for it if the DNS is very uncomfortable.
User:as of now it is bearable, the only problem is the sensation of sneezing most of the times
Doctor:Otherwise it is not sure that you will get cured with a septoplasty.
Doctor:Yes sneezing comes with allergy.
User:and lot of mucus that comes along with cold, which now I understand is reaction of allergy
Doctor:It is best taken care of with anti-allergics.
User:ok, so better I stay for some more time on medicines and see the result
Doctor:Yes it is your call.
