Was it in your previous chat
Doctor :   Ok

Doctor :
Proceed
User :   ok
User :   Well its about my father, gout, diabetes, high blood pressure etc. He
Doctor :   Yes

Doctor :
I could see that
User :   had a minor procedur and placed a kind of tube in his bladder/penis becuase he head kidney stones in his bladder which were 5 minimillers
Doctor :   Ok

Doctor :
Urethroscopic procedure
User :   Correct, tomorrow they are going to take it out
Doctor :   Fine

How is he tolerating it?
User :   Well he has a very low treshold of pain but it went suprisingly well
Doctor :   Great

User :   he was also given an antobiotic because he had a infection, the stone caused it the doctors said
User :   So now my question is this, how will the gout react to this. Becuase he has bedridden for 2 months now and feels much better
Doctor :   Ok

User :   will his kidney levels be okey
Doctor :
This is a complication of gout, most likely I mean
Doctor :
Uric acid stones
Doctor :
The other treatments have to continue thesame
User :   You mean Zyloric?
Doctor :   The same medications would continue

User :   he has a very dark urine
Doctor :   no changes in that
User :   I see.
Doctor :   Did they tell what type of stone it is?

Doctor :   like calcium/uric acid

User :   I believe it was kidney stones
Doctor :   Yes, that I got

Doctor :
Fine, the other details are better said if it is removed out
Doctor :
Do enquire about that
User :   about what exactly?
Doctor :
the stone content
User :   His kidney levels were very high 2-3 months ago 400 something and now they are a bit over 100
Doctor :   Ok

Doctor :
Let him undergo the procedure
Doctor :
as that is of major concern now
User :   It was caused my anti-inflammatory medicins, gout and infection in the bladder
Doctor :   Ok

User :   You mean to extract the tube?
Doctor :   No
User :   Then what, please elaborate?
Doctor :   The procedure of removing the stone
Doctor :
extracted through the tube
Doctor :   It is an endoscopic procedure

User :   oh I father just told me that they already extracted the stones and tomorrow they are going to remove the tube
User :   Yes...
Doctor :
and is safer compared to normal conventional operations
Doctor :
Hence faster recovery
User :   my father (I meant to write) above
Doctor :   Ok

Doctor :
So the procedure is over then
Doctor :
Fine
User :   I understand.
Doctor :
Congrats for that
Doctor :
having completed it
Doctor :
as any procedure is a tension to the recepient
Doctor :
and his family
User :   Thank very much. (Smiling ) So how will his gout correlate to this?
Doctor :   Gout increases the chances of stones

User :   He coudn't walk and used crutches and was in bed all the time, now he can press down on his foot and use one crutch
Doctor :
the uric acid accumalated forms crystals
User :   yes.
Doctor :
which ultimately forms stones
User :   Yes, I see.
User :   But his urine is very dark, is that alarming?
Doctor :
No
User :   Ok, that's reassuring
Doctor :
there could be bleeding in the region
Doctor :
slightly I mean to say
Doctor :
which alters the color
User :   I see.
Doctor :
the minor injuries have to heal
User :   I understand
Doctor :
The same diet which I had advised you the last time
Doctor :
when had a chat is recommended
Doctor :
We should do the best as we can
Doctor :
the rest is left to the body and soul to react accordingly
User :   Oh so you and I have chatted before (smiling)
Doctor :
even in spite of taking best care, things do happen
Doctor :
yes, once I have done
Doctor :
wherein I recommended some dos and donts in the diet
Doctor :
I mentioned about alcohol
Doctor :
and you said that he doesnt take
Doctor :
Do you remember?
User :   My father is a bit of a hypocrodiac so he often asks me to consult with you
Doctor :   No problem

User :   No no alcohol what so ever. Only on holidays or big events
Doctor :
Yes, I know that
Doctor :
You had told me the same in my previous chat
User :   Laughing
User :   Ok
Doctor :
again I repeat not to have organ meat, red meat, shellfish, mushrooms, asparagus, dried peas and beans
User :   Yes I remember
Doctor :   Is there any thing else I can help you with?

User :   Yes.
Doctor :   Sure ,proceed with that
User :   My twin sister has gallstones, plus cirrhosis and hep C
User :   She was in the E.R and got painkillers
Doctor :
Ok
User :   Spasofen, with codine and morphine
Doctor :   Yes given for pain control

Doctor :   high end drugs to be specific
User :   She now she is very tender in the abdomen area, is very gasy and hard in the stomach also has white spots in the urine
Doctor :
Ok
User :   cant belch
Doctor :
What is your query regarding it?
User :   Well she has these pains very sporadically, she has lost a lot of weight these last 5 months over 25 kg, so is on a well balaced diet but does pig out every now and then
Doctor :   Ok

User :   in the past when she took painkillers it helped her after 20 min, now this last time she took 3 Spasmofen and the pain didnt pass
Doctor :   Ok

User :   she had to go the E.R, there is no inflammation they said as they did an ultrasound
Doctor :   Ok

Doctor :
When was she diagnosed with cirrhosis?
User :   should she operate the gallbladder
Doctor :
Is the pain mainly in abdominal area?
User :   a month ago
Doctor :   Ok

User :   she doesnt drink, smoke, no blood transfusion or any tattoos
User :   she is going to start treatment for Hep C in January 2010
Doctor :   Ok

Doctor :
Now regarding the operation, I am sorry that I cannot comment on that
Doctor :
as there is a viral infection
Doctor :
and also cirrhosis
Doctor :
risks and benefits have to be weighed
Doctor :   and only then a decision could be taken regarding that

Doctor :   What is her age?

User :   So a operation might be risky? The doctor said that since the pain didnt relent this last time she should do a operation as the gallbladder will produce new stones. She is 32 yrs old
Doctor :
Ok
Doctor :
If it is advised, then she should go for that
Doctor :
to determine the feasibility I would like to have details like her liver function tests, ultrasound report, other blood test reports, endoscopy(if done)
Doctor :
to give a clear picture regarding the same
User :   Well now she is perlexed. Well her liver fuction is very good the doctor said
Doctor :
Send the details to YYYY@YYYY
Doctor :
Our panel will discuss on it and reply you via mail
User :   they said her blood samples were also very good, the doctor gave her the news about cirrhosis after the biopsy
Doctor :   Ok
Doctor :   But for a better comprehension I would require the details
User :   on ?
Doctor :   the things I mentione before

Doctor :
liver function tests, ultrasound report, other blood test reports, endoscopy(if done)
Doctor :   Our panel will discuss it as it is slightly complicated for individual decision making
User :   her ultrasound was also very good, they said she has gallstones which she has for 10 yres now
Doctor :
Need to take extra care in such cases
Doctor :
Ok
Doctor :
Hope you got my idea
Doctor :
and the reason
Doctor :
But one thing is for sure, Medical line of treatment is the first choice
User :   yes I understand
User :   Well thanks for this time and have a great day.
Doctor :   Sure

Doctor :
Will you be sending the details?
User :   I will certainly try but its Christmas soon so I will do my best to get the details before
