Doctor :   Hi
Doctor :
How can I help you?
User :   Hallo. Yes but please bare with me as I fill you in on the details
Doctor :   Sure

User :   Great. It concerns my father who is 65 yrs old and has suffered with high blood pressure and gout for over 15-18 years now. He has been diagnosed with diabetes a year ago but has since then lost 10 kg and stop taking his pills against diabetes because they have caused him problems with his kidneys
Doctor :   Let me read it
Doctor :
Ok
User :   Anti-inflammatory and pain killers against his gout has led him to have very elevated kidney levals. Besides that, he has some kind of mincus injury around/in the knee-cap. He has several times been in the emergency becuase of the gout and the doctor's have extracted fluid from his knee.
Doctor :   Ok
Doctor :
In gout it is commonly seen
Doctor :
Called as Gouty arthritis
Doctor :   It affects the toes as well
User :   A few days ago he was admitted to Reumatology and and the doctor gave him a cortison shot and said he would be better but he still can walk or stand
Doctor :   Ok
Doctor :
Once the fluid is cleared, it would be better
Doctor :
It takes around 7-10 days time atleast
Doctor :
So I ask you to advise him that it would resolve and needs to bear with it for some days
User :   The docor said that my father does NOT suffer from arthritis so now my father is not sure if its the gout or the minsic injury. The ortopedic doctor has advise my father not to go along with a operation since he was diabetes, gout, high blood pressure. He has it is very risky
Doctor :
Yes
Doctor :
Operation is not required
Doctor :
Medicines are more than enoug
Doctor :
But he has to show compliance
Doctor :
by regular follow ups and
Doctor :
blood tests
User :   Yes but he has been in bed for almost 2 months now aside from being in the hospital for his problem. Well his blood test are much better than 1-2 months ago. The acid levels in his urine and the gout have lowered immensly but he is still passive
Doctor :
Well, that is really a good improvement
Doctor :
He has to follow dietary advice as well
User :   But why does the fluid keep returning and the swelling as well? He can't stand or add pressure to his leg and when he has to walk, lets say to the bathroom he uses crutches
Doctor :   It is because of the accumalation of uric acid in the blood
Doctor :
the fluid retention will be there
Doctor :   in the subcutaneous tissues
Doctor :
most prominent in joints
User :   He takes Zyloric 300 mg aginst the gout.
Doctor :
Ok
Doctor :
It helps in better excretion of Uric acid
Doctor :
Maintaining adequate fluid intake helps prevent acute gout attacks.
Doctor :
Adequate fluid intake also decreases the risk of kidney stone formation in patients with gout.
Doctor :   Should avoid alcohol as well
Doctor :
Are you following?
User :   He doesnt drink or even smoke. Yes I am thanks for asking. So if I fallow you correctly Dr, its the gout and not the minsic or what its called?
Doctor :   Yes
Doctor :
They are acute attacks of gout
Doctor :
Dietary changes can help to a great extent
User :   Ok but he has never had such acute attack of gout before its been nearly 2 months and he still cant walk around with out help
Doctor :   Ok

Doctor :
It will take sometime to regain the function
Doctor :
Cant tell the precise duration
Doctor :
It is purely subjective and gradual
User :   I understand. So the injection of cortison has helped or not do you think?
Doctor :
It helps in decreasing the inflammation of the joint
Doctor :   It is advisable and was given rightly

User :   That's reasurring to hear. So avoid alcohol, eat healthy what else can he do? Any major no no?
Doctor :
Regarding
Doctor :
Is he following precautions in his diet
Doctor :
Abstain from seafoods, shellfish and organ meats such as liver, brains, kidneys, and sweetbreads.
User :   The gout, I mean his doctor has told him that due to the anti-inflammatory drugs and pain-killers, his kidney levels are a bit high and that its in-irresversible
Doctor :   Yes, but care should be taken to avoid its progression

User :   Can they kindeys get worse?
Doctor :   No need to anticipate such things
Doctor :
Let him monitor it with regular blood tests
User :   Ok. How about poultry and meat, are they accceptable for him?
Doctor :   No red meat
Doctor :
Others are fine
Doctor :
Organ meats, I have mentioned it earlier
User :   Here's a stupid question but please bare with me, what exactly is RED MEAT?
User :   rare meat?
Doctor :
Like beef and mutton
User :   Oh, got cha.
Doctor :
Like the meat which is red colored when raw
Doctor :   No questions are stupid
Doctor :
It is always better to get it clarified
User :   *Smiling* I understand thanks. True...
Doctor :   Is there any thing else I can help you with?

User :   Well I would like to ask you about now. I'm 32 and was diggnosed with diabetes october 2008. I was heavily over-weight, eat poorly and was physically passive
Doctor :   Ok
User :   I was prescribed Glucophage 500 mg once a day and Levaxin 25 mg against thyroid
Doctor :   Ok
User :   Today eating healthy and have shed over 20 kg in 4 months. My sugar levels are around 5-6.0 before breaka st. I'm not taking my medicine only Levaxin,
Doctor :
Ok
User :   I learned that taking them can damage your kidneys, so my question is, should I continue with them or not?
Doctor :   You need to continue it
Doctor :
There is nothing like kidney damage
Doctor :
when you are taking it for thyroid
Doctor :
The hormone has to be supplemented
User :   We have diabetes in the family but since I'v lost so much weight I thought I didn't need it anymore
Doctor :
So, you should continue
Doctor :   No

Doctor :
Should not decide like that
Doctor :   If your sugars are under control, then you can stop it after getting your doctor's opinion
Doctor :
But not abruptly
User :   So I do have diabetes even if my levels are stable, or did I get it temporarily because of me being over-weight
Doctor :
It can be due to overweight and thyroid
Doctor :
so check your sugar levels once again
Doctor :
and decide from the levels
Doctor :
Keep monitoring your thyroid levels as well
User :   How can a doctor tell if its temporarily or solid diabetes. He has said, my doctor I mean that I dont need to take it but have to take a medicine aginst
Doctor :   By checking it once more your thyroid is normal
Doctor :   So, now since you are on treatment for thyroid, check it once more

User :   fat in my blood, not sure it that is the correct medical term but its against fat
Doctor :
Ok
Doctor :
Cholesterol
User :   Exactly... True
Doctor :
Yes, even that has to be controlled
Doctor :
Very important for Cardiac health
Doctor :
That is also dependent on thyroid hormones
Doctor :
So, even that will be ok
Doctor :
if thyroid is in recommended values
User :   Yes so the medicine will help me, the cholesterol won't vanish with the weight?
Doctor :
No
Doctor :
It can do it directly
Doctor :
Keep a check on that as well
Doctor :
So the tests you need to undertake
Doctor :
Thyroid profile, Lipid profile and Blood sugars
User :   I see. Another question, Levaxin is it dangerous if taken for a long period of time, that's to say if one has a low thyroid function??
Doctor :
No
Doctor :
Dangerous if you dont take it
User :   Really, how so?
Doctor :   Bcos your thyroid imbalance will reproduce all the issues once again
Doctor :
overweight, high cholesterol and high sugars
User :   So I need/have to be on Levaxin (thyroid medicine)
Doctor :
yes
User :   I see. Good to know. They have helped me with my weight.
User :   Shedding kg
Doctor :   Yes
Doctor :
Thats rigth
Doctor :
Is there anything else you want to ask?
User :   I'm always very thristy and the minute I drink something I need to go the ladies room. Is that the diabetes?
Doctor :
Not really
Doctor :
You will come to know after the blood test, accurately
Doctor :
So, no tension till them
Doctor :
'then
User :   tension?
Doctor :
Tension regarding Diabetes I mean
Doctor :
Have you got the feedback form?
User :   My dr told me that my blood sugar levels are "satisfactory" but that I need to take medcine against cholestorol
User :   That's all I know.
Doctor :   Yes, if it is under control without medications then you need not take it
Doctor :
You can just keep monitoring it every month
User :   so is it full-blown diabetes???
Doctor :
No
Doctor :
I have explained previously
Doctor :
that it could be due to overweight and thyroid problems
Doctor :
So, it means that it is not full-blown one
User :   Yes you have. Thank you for your time and good advice. Have a wonderful day and take care... *Smiling*
Doctor :
Please fill the feedback form say done when done.
