User :   Hi, I am a 25 year old male with Gastro oesophagal reflux, and have a beer or two drinks of whiskey on the rocks everyday. I also smoke 10-15 cigarettes a day. I have sharp pain in my lower back - somewhere between the last two ribs, which eventually also appears in my left shoulder. I have had this two times in the last 3 days during the night. Everything is fine when I wake up in the morning. Is it a gastric problem or something serious? the pain is quite sharp and increses when I breathe or move.

User :   not exactly the back i think more from the center to the middle of the back

User :   not a lot of stress

User :   im pretty sure it is gas

Your pain, the discription, onset, the time that you get the pain
Doctor :  
are all more in favor of gastritis and reflux symptoms.
Doctor :   The other probable cause is definitely a heart problem.

But your age, absence of risk factors except for drinking and the reports are

not in favor of it.

So it is most probably a gastritis.

Are you on any medications?
User :   none

I will suggest you a few measures then.
Doctor :  
1. You can take Syp Mucaine gel 2 tsp before food twice a day.
Doctor :  
2. Cap Omex 20 mg tablets once a day for 1 week before breakfast.

3. Reduce your caffeine consumptions.
Doctor :  
4. While sleeping raise the head with two soft pillows.
Doctor :  
5. Drink plenty of water before you sleep.
Doctor :  
6. Do not hit the bed immediately after a meal.

7. Avoid hot and spicy food.\
Doctor :   8. Reduce your alcohol consumption.
Doctor :  
The first two measures are for the treatment of the symptoms.

The rest of the measures are for preventing the episodes.

9. Lastly i would suggest you to get an endoscopy done to see if you have an

In which case surgery may be needed.

User :   The pain is nowhere near the chest
User :   it is more toward the side/back of the body right abouth the place where the last rib bone would be there
Doctor :   One more thing if your are obese or over weight weight reducing measures may be helpful too.
User :   no i am 182 cms tall and weigh 76 kgs
User :   are you sure it is not bladder, pancreas or kidney issues?

If the pain is on the right side and it is referred to the right shoulder, then there is a possible chances that you may be having gall stones.

User :   yes it is
User :   ive had it 3 times and all three times it has been on th eleft side
Doctor :   Kidney and bladder causes pain much lower and not near the ribs.

so it is unlikely to be those.

And a pain from pancreas is very severe and is usually following heavy alcohol consumption,
Doctor :   Your description hence was more in favor of the gastritis.

You may get an endoscopy with an ultrasound examination, if you still doubt the presence of other causes.
