User:Good Evening
Doctor:How can I help you?
User:May I know .... if you are physician?
User:or a specialist?
Doctor:I am a general Physician.
User:I have few questions
Doctor:Sure. Please proceed.
User:Once it happened that I fell down due to giddiness
User:and loo also
User:while I was unconsious
User:it has happened thrice before this
User:My family doc told me to see a neurologist
User:it happened in my home town
User:but due to time constraint
User:and couldnt see a doc there
User:he asked to get some tests done
User:like CT scan, EEG and ECG
User:i got everything done
User:but when I showed the reports
User:he said that its normal
User:he wrote on the prescription that it might be either seizures or syncope
User:he asked my questions
User:if my body shakes when it happens
User:I tolld him no
User:finally he said that he cant start any medicine as the symptoms I told him was not enough for him to diagnose
User:he told me to get the video if it happens again
User:and then only he would be able to do anything
User:would you help me
Doctor:Let me guide you regarding this.
Doctor:May I know your age?
Doctor:let me ask you a few questions.
Doctor:During the past three episodes, was there anyone near you when this happened?
Doctor:During the past three episodes, was there anyone near you when this happened?
User:my mom was there
Doctor:What was her description about the event?
User:for 2 episodes
User:and for 1 I was alone
Doctor:What was your mom's description about the event?
User:She said that I was sweeating like a pig
User:and my pulse rate gone down
User:I was totally Unconscious .. in a way that even when i was sitting , I fell down
User:and then Loo happened
User:and thats it
User:after sometime I was back
Doctor:Did she say about any shaking or trembling of the body?
User:she never mentioned that
Doctor:How long did you remain unconscious? And then, what got you back to consciousness?
User:she said .. it looked as normal as it happens when someone has that giddiness thing due to weakness and all
User:I was unconscious for about 3-5 mins
User:and then automatically I am back to normal
Doctor:Immediately after waking up, did you feel extremely thirsty or hungry?
User:I used to have that giddiness problem right from my childhood
User:but loo never happened
User:It happened first when i was 21-23
User:I dont feel that
Doctor:Now, let me try to explain from your history.
Doctor:The symptoms that you have mentioned: Giddiness, unconsciousness, sweating, pulse falling and loo points towards a parasympathetic activity increase.
User:what is Parasympathetic
Doctor:Sympathetic and parasympathetic systems are the neurological control systems in our body.
Doctor:I agree with your Doctor saying that, it could be a syncope.
User:So what does it mean when you say Parasympathetic activity Increase
Doctor:In that case, as your doctor said, it could be a syncope.
Doctor:Other possibility as he said is Seizures. But you did not have any shaking. And the EEG has come out normal.
User:My father also had the same problem .. he underwent a treatment for 3-4 years.. he used to take Tegrital tablet
User:EEG was normal
Doctor:So, I would say, you need to consult another Neurologist with all these details. he may start you on anti seizure medication such as tegrital.
Doctor:At the same time, another control system of the body is the hormonal system.
Doctor:You need to do hormone test including Thyroid profile, Cortisol levels, Insulin levels, estrogen and progesterone levels etc.
Doctor:If there is any imbalances, they need to be corrected.
User:ohh Okay
Doctor:If both the hormonal systems and neurological systems are controlled, then you should not be having any more of these episodes.
User:ohh Okay
Doctor:have you followed all my suggestions?
Doctor:Is everything clear?
User:Yes Doctor
User:I would first go to another neurologist
User:could you please suggest me someone
Doctor:Surely, I can help you with that.
User:That would be very helpful
Doctor:I hope you need to find a neurologist right?
Doctor:Can you confirm your mobile number?
Doctor:One of our customer care executives will assist you further in finding a good neurologist
Doctor:You will be receiving a call soon regarding the same.
User:Sure.. thanks so much
User:I have one more question doctor
Doctor:Please proceed.
User:Its gync related
User:What are the possibilites of female getting pregnant, if she had the intercourse 2-3 of her periods ends?
Doctor:You mean 2-3 days during the period or before the period?
User:after it
Doctor:Chances of pregnancy are less. But, if there is any cycle variation during that time, then pregnancy may occur.
User:and what if she has taken an emergency contraceptive pills the third of intercourse?
Doctor:if an emergency pill has been taken, then pregnancy chances would be much lesser.
Doctor:You can say there is a failure rate of 15-20% for emergency contraceptives too.
User:when they say to take it within 72 hrs.. does that mean that it has to be perfectly 72 hrs or it means third day
Doctor:No. it means that the success rate of the pill decreases as the duration increases. it means that, take it as soon as possible within 72 hours.
Doctor:That is the time within which it has higher success rates.
User:and also is it necessary to have the periods again once you take any ipill
Doctor:Yes. Usually, when the pill is taken, there can be a breakthrough bleeding within one week.
Doctor:But, if it is taken during the early part of the cycle as you said,
Doctor:the layers of uterus might not have formed enough for a bleeding to occur.
User:and how much gap should be given to get the firmness ot Vagina back
Doctor:There is no particular time to be given for that.
Doctor:It gets back to normal within a few days.
Doctor:But, the initial tightness would be lost.
User:what if someone who is unmarried had sex with her partner and dont want her fiance to know about it
Doctor:It would be hard to explain. It depends on the fiancee's experience and knowledge of the condition of a vagina which did not have an intercourse.
Doctor:Have you followed everything?
User:Yes doctor
Doctor:Is there anything else I could help you on?
User:But it is also said that if a gap of 1 year or so is given the vagina get a little firm than before
User:is it correct anywhere?
Doctor:Yes, it gets firm with time, if it remains unused.
User:Thanks for the guidance doctor
User:I will see another neurologist
Doctor:Is there anything else I could assist you on?
Doctor:You are welcome.
Doctor:Hope I had been helpful to relieve your concerns.
User:This was very very helpful doctor
User:Thanks again!
Doctor:You are Welcome again.
Doctor:Thanks for consulting me.
Doctor:Have a Nice Evening!!
User:You too Doc !
Doctor:Bye for now.
Doctor:Thank you.
