Doctor :   hi
Doctor :
how can i help you?
User :   Hi
User :   36 hours ago I had protected sex, but the condom broke
User :   8 hours later I took Plan B
Doctor :
ok
User :   I have a mild case of Crohn's and only later realized that this may be an issue
User :   only a small portion of my terminal ileum is affected
Doctor :
ok
User :   how will this impact absorption?
Doctor :
if only the terminal part of ileum is affected
Doctor :
the absorption will not be hampered very much
Doctor :
minimal loss of absorption wil be there
Doctor :
which plan-B tablet did u take?
User :   i took the one step
Doctor :
the one having 1 oil or one having 2 pills?
User :   it is only one pill
Doctor :
ok
Doctor :
when was your last menstrual period?
User :   on the 21st of January
Doctor :
and how many days menstrual cycle do you have? 28 days? 30 days? or more or less?
User :   i have actually been quite irregular
User :   anywhere between 24-28 days
Doctor :
as per the calculation you have had unprotected intercourse on the 20th or the 21st day of your cycle
Doctor :
your ovulation time/fertile period lies between 29th jan to 5th feb
Doctor :
during which and unprotected intercourse can result in pregnancy
Doctor :
so in this scenario
Doctor :
the chances of pregnancy for you is very less
User :   ok
Doctor :
since you have already taken plan-B one step pill
User :   that is good to hear
User :   i was only worried about the absorption of the pill
Doctor :
the effect of it will be there
Doctor :
so do not worry
Doctor :
if you do not get your periods on time
User :   would you say that the pill is mostly absorbed before reaching the terminal ileum?
Doctor :
wait until another 10 days
Doctor :
it should come by then
Doctor :
yes
Doctor :
if not, then visit your gynecologist.
User :   ok
User :   thank you
Doctor :
is there anything else i can help you wioth?
Doctor :
*with?
User :   is there anything else I should be doing in the mean time?
User :   or can i trust that the pill had effect?
Doctor :   you can trust that the pill has its effect
User :   ok
Doctor :
also the chance that you are pregnant is also minimal
Doctor :
if your cycles are of 28 days
Doctor :
or less than that
Doctor :

Please fill the feedback form say done when done.
User :   i was also taking Pentasa
User :   is it alright to continue taking pentasa or will it affect the pill's reaction in the body?
User :   i discontinued the use just in case
Doctor :
you can continue to take it as you take it usually
User :   ok thank you
User :   also, my periods have been lasting longer than usual
User :   they usually last five days but have been lasting for about a week
Doctor :
it can happen due to the Pentasa use
Doctor :
you can discuss this with your doctor
Doctor :
and see if the medication can be changed
Doctor :
to some other alternative
Doctor :
Is there any other query you want to ask me?
User :   yes
User :   in general, just so that I am aware
User :   where are pills absorbed in the digestive system?
Doctor :
ok
User :   i understand that each medication is different
Doctor :
it depends on teh type of pill that you take
User :   but in general
Doctor :
most of them get absorbed in the stomach being acted upon by the gastric acid
User :   ok
Doctor :
few get absorbed in duodenum, and small intestine
Doctor :
and there are few sustained release ones, that get absorbed slowly until they are into the large intestine also
User :   also if this does occur again, I have read that inserting the pill vaginally works as well?
Doctor :
but the largest area for teh absorption is teh small intestine in general terms
Doctor :
in case you are sexually active and want to avoid pregnancy, please discuss with your gyne regarding the contraception use
Doctor :
as you are suffering from crohns disease
Doctor :
so teh absorpion will matter
Doctor :
also, the vaginal pills
Doctor :
can be used, but they are usually prescription medicines
Doctor :
and should be taken after examination by a doctor
Doctor :
as they have potential to cause severe uterine contractions and cramps
Doctor :
so do not take anything yourself
Doctor :
if you have completed your family, go in for a IUD (intra-uterine device) that is more effective than all these things
Doctor :
if you have not completed your family,
Doctor :
then it is advised to go in for regular BCP or depo shots
Doctor :
for adequate contraception
User :   right
Doctor :
such that your crohns status will not affect their efficacy
Doctor :   have you followed what i have said?
User :   yes of course
Doctor :   is there anything else you want to ask me?
User :   at this time however, because it was an emergency situation
User :   is it safe to assume that the pill was absorbed
User :   and did have an effect?
Doctor :   yes, i understand, and hence i am advising to such that these situation do not arise again
Doctor :
see, as per your history you told me that your cycles vary between 24-28 days
Doctor :
so based on that, the emergency situation arised on teh 20th or teh 21st day of the cycle
Doctor :
which does not fall in ovulation period or teh fertile period
Doctor :
when egg is released in your body
Doctor :
assuming that teh first day of bleeding of last menstrual period was given corect
Doctor :
so in this scenario, the chances of pregnancy itself is minimal
Doctor :
however if due to various reasons if we assume that the ovulation occurred late, and the unprotected intercourse happened when the egg was stil inside your body
Doctor :
then yes, the pill wil have its effect, as only the terminal ileum is affected in your case of Crohns
User :   ok
Doctor :
as the maximum absorption takes place in small intestine, the drug in th epill
Doctor :
would almost be absorbed already by the time it reaches the affected area
Doctor :
*pill
User :   perfect
User :   that is exactly what i wanted to know
User :   very little information about this online
Doctor :
i hope this clears your doubts now
User :   yes it does
Doctor :
ok
User :   and i will consider your advice
User :   thank you
Doctor :   is there any other query you would want to ask me?
Doctor :   have you filled the feedback form?
User :   yes i did
