User :   I have recently benn diagnosed with multinodular goiter. Im a little concerned as to what I should be doing about it. The nodules are heterogeneous measuring 8mm 15mm 14mm and 16mm. There is also a fluid filled cyst that measures 13x17x19mm. The ultrasound comments are: Bilateral benign multinodular goitre, with a cluster of solid nodules and a fluid filled cyst in the lower right pole accounting for the patients palpable mass. What should I be doing in regards to this diagnosis how should I proceed. Im a single mother of 6 and am a little worried.
Doctor :   Hi,
Doctor :   Let me read your query for few minutes to be able to have a good discussion here
User :   hello
Doctor :   ok, the nodules can be either begnin or malignant ,
Doctor :   th scan has mentioned them being begnin
Doctor :   but i would ask you to get a radioactive iodine(isotope) scan
Doctor :   which will differentiate the nodules into hot ( malignant ) and cold (begnin ) nodules
Doctor :   the treatment form where differs
User :   Im a little worried because Ive been feeling a little pain in that area lately
Doctor :   its surgery followed by radiotherapy for a malignant one and medicinal treatment for the begnin type
Doctor :   pain could be due to pressure , pain do not indicate the prognosis of the nodules
User :   my doctor told me to see an endocrinologist and I cant afford one right now
User :   What should I do?
Doctor :   also you may have to get thyroid profile( 5thyroid hormone estimation ) so that if the nodules are begnin the right medicinal treatment modality can be chosen
Doctor :   thyroid hormone*
Doctor :   its very necessary to visit a endocruinologist
User :   I had a blood test done but did not fast would this affect the results?
Doctor :   no , may i know what blood tests did you go for ?
User :   A thyroid function test?
Doctor :   good , once you get the reports come on to the chat we can have a discussion and decide about the necessity of the endocrinologist
User :   The test results came back as normal thyroid function
Doctor :   i still am suspecting it to be begnin and non dangerous one
Doctor :   very much fine then ,
Doctor :   May i know the values please ?
User :   I dont have the results with me but my doctor said they were within normal range
User :   Should I be going back to her to discuss the endocrinologist and further tests?
Doctor :   ok, i would consider no problem in waiting for another 2 months to visit the endocrinologist
User :   These tests were done in September last year
Doctor :   if there is any increase in the size of the nodules ( testd by self) you may have to rush to the specialist
User :   should I go back immediately then?
Doctor :   i am sorry , i missed to ask the date ,
User :   September 23 2010
Doctor :   for now repeat the test and come back on to the chat with the report
User :   Ok thank you
Doctor :   lets have a detailed discussion over it
Doctor :   and may i know when was the scan done ?
User :   Ok I just worried because recent pain
Doctor :   pain as i said could be due to pressure
User :   the scan was september 23 2010
Doctor :   ok,
User :   pressure from what?
User :   the nodules?
Doctor :   no need of repeating the scan till you get the thyroid test ( hormonal)
User :   another blood test?
Doctor :   pressure by the nodules onto the surrounding tissue and also due to thyroglobulin fluid filling up in the nodule
User :   will that get worse?
Doctor :   yes, the test done in sep 2010 is old repeat again
Doctor :   no , the nodules can get bigger giving symptoms
User :   Ok thank you so much Ill book another appointment with doctor for test repeat
Doctor :   nothing else it could affect
Doctor :   yes,
User :   Good bye doctor have a nice day
Doctor :   get a thyroid test every 6 mon5hs p0nce to make sure that the hormone levels are noit altered by the nodules
Doctor :   months once*
Doctor :   not*
Doctor :   Bye for now]
Doctor :   Thanks for consulting me
Doctor :   Kindly provide your feedbac
