Doctor :   hi
Doctor :
how can i help you?
User :   My mom was detected for sugar of 400 - 2 days back, she has had BP for the past 3 yrs
Doctor :
ok
User :   she panics every time the BP machine is used - She was recommended Cordase 5 mg, Amaryl 1 MG
Doctor :
ok
User :   In the morning she had both the tablets - The BP again is 150/105 twenty minutes back
User :   she had the tablet at 9.20 am i.e 2.5 hrs before
Doctor :
what was the time gap between taking the tablets and taking BP?
Doctor :   ok

User :   why does her BP rise - All the other tests, kidney, liver CT scan, Dopplers on Leg and Carotid were good
Doctor :
ok
Doctor :
one reason for her BP rise could be her panic affect itself
Doctor :
on seeing the BP machine
Doctor :
try to check her BP with a digital machine if you are cheking regularly with the conventional one
User :   ok - the digital one has not been used
Doctor :
then try using that
Doctor :
but do not tell her that it is to measure her BP
User :   ok - will try to get her to do that
Doctor :
else you can try a counseling session for her to help her out of this panic state on seeing the BP apparatus
User :   why else could her BP go up?
Doctor :
another reason could be that the medicines are not working
Doctor :
and they will need a change
User :   but how can it happen that the medicine stopped working suddenly
Doctor :
one more reason could be that her sugars are not controlled which will also indirectly affect her hypertension
User :   ok
User :   they have reccommended
Doctor :
so get her sugars controlled
Doctor :
and then see for BP control
Doctor :
try to see her sleeping BP
User :   nicardia 30 mg
Doctor :
if that comes out normal
Doctor :
then the BP rise will be purely due to panic attacks
User :   ok will get the BP when she is sleeping
Doctor :
is there anything else i can help you with?
User :   how does sugar effect hypertension
Doctor :
i will help you in this
Doctor :
diabetes affects end organs, like kidneys, heart and brain
Doctor :
which control BP to large extent
Doctor :
so any damage to these will result in increased BP
User :   but these were normal yesterday - these were reviewed
User :   all the parts liver, kidney, brain & heart
Doctor :
if her tests are al normal, with no diabetic changes in end organs, then it will be due to high sugar levels
Doctor :
high sugar levels are known to cause damage
Doctor :
to kidneys and peripheral vascular systems
User :   ok - she has started amaryl since yesterday
Doctor :
ok
User :   or does she have to take insulin injections
Doctor :
you can wait to see if Amaryl is controlling BP in next two to three days, meanwhile practice diet control for sugar along
Doctor :
with diabetic medications
User :   diet she has contrlled
Doctor :
and try and check her sleeping BP
Doctor :
which will provide the main clue
User :   we are going for a second opinion for sugar tomorrow - to decide the need for injection
Doctor :
and if BP remains high in sleep also, then she will need medication change
User :   ok - her sleeping BP shall be tested
Doctor :
that will be good
Doctor :
is there anything else i can help you with?
User :   nothing else - thanks
Doctor :
