User :   Hi I have done several hiv tests, duo at 31 days, antibodies and antigen test at 69 days. I have also done insti tests at 3,4,5,6 months all negative. My question is that quite a few of my cuticles are disintegrating and the lunula seems to be white in quite a few fingernails. I also have a coated tongue which goes when I drink water. Also I have loose stools on and off. I am really confused as to what is going on. I have lost weight. Do you think I have hiv?
Doctor :   Hi
User :   Hi
Doctor :
you said have taken hiv tests, what type of tests have you taken?
User :   Hiv Duo, P24 antigen combined with antibody, in Uk twice, as well as INSTI which I believe is 3rd generation.
Doctor :   ok
Doctor :
have you anytime taken dna pcr test for hiv?
User :   No never.
Doctor :
Doctor :   you have disintegration of the cuticles since when?
User :   quite early on since the incident about, 2months approximately
Doctor :
is it happening in all the fingers or only in few fingers?
User :   A few, 3 fingers, plus both thumbs
Doctor :
loose stools since when?
User :   Since about week 3/4. But on and off. I used to get really constipated after eating lamb, now I am okay. I have never really suffered from loose stools
User :   since now
Doctor :
do you have history of unprotected sexual exposure?
User :   Just once, on a stag do in , I was so drunk I can't be sure? Sorry for being so vague. My cousin's stag/bachelor party
Doctor :   when did this happen?
User :   End of Hune 2009
Doctor :
so approximately 6 months back.
User :   Yes that's correct
Doctor :   the symptoms you have described like cuticle loss, whitening on lunula, coated tongue all indicate malnutrition
Doctor :
specifically to multivitamin and mineral deficiency
Doctor :   the loose stools and loss of body weight may be part of the malnutrition,
User :   What about weight loss? I am eating normally. I have a full blood count done end of August, which was fine except raised cholesterol level. So you don't think it is hiv. What do you suggest Dr. And what is your name and where are you based?
Doctor :   i don't think it is HIV,
Doctor :
my self drfrom
Doctor :   many of your tests for hiv are negative
User :   So what do I do, I have lost 6 months of my life going crazy. I lost both my parents one in 2007 & 2008, and I am really devastated for my wife and daughter. I am totally at fault and now don't know what do do? I had another test done at a Gum clinic, now awaiting results. I should have used PEP but I was so confused.
Doctor :   it is very unlikely that you are suffering from the hiv
Doctor :
all te tests till date done are negative
User :   What do I take for the deficiency that you mentioned?
Doctor :
further i suggest you to undergo the DNA PCR analysis for HIV
Doctor :
if this tests is negative than it means you are not having HIV infection and you need not worry about the hiv any more
User :   If its positive, my life is over. What are the treatments like in India, is it expensive to get treated there?
Doctor :   i dont think it will be positive
Doctor :
as many of the tests you mentioned are very sensitive it is very unlikely that they have missed
Doctor :
it is the final confirmation to say that you are not having hiv
Doctor :   regarding the treatment option
Doctor :
right now what you require is multivatimin tablets
Doctor :
and consumption of protein diet like egg and meat
User :   any types in particular? Will the egg off set any diarrhoea? Should I stop sweets
User :   Dr you still there?
Doctor :   no egg will not affect the diarrhea part
Doctor :   do you have active diarrhea now?
User :   I wouldn't say that exactly. I pass motions once daily, but they seem very soft and mushy. Sorry for the detail. I sometimes get fresh blood, possibly haemorrhoid?
Doctor :
Doctor :   may i know your diet pattern
User :   Breakfast, hardly anything. Lunch maybe a sandwich, chicken or ham with a packet of crisps along with some water. I try to drink 3/4 cups of water per day. Dinner a proper meal, say pasta/chicken/ homemade soup with vegetables etc
Doctor :
Doctor :   was this your regular diet pattern or it is changed in the recent times
User :   I have probably cut out junk food. I would say I eat a bit less than normal. I would say initially after the incident I wasn't as interested in food as before.
Doctor :
yes this change in the attitude towards food is responsible for the present situation
Doctor :
this has caused the malnutrition and altered constipation and soft stools at time
Doctor :
when it is hard stools there might be blood in stools also
User :   Dr I haven't hard stools for such a long time. I miss being constipated in a funny way. That was normal for me!!
Doctor :   blood in stools can happen when the stools are hard and they tear the delicate mucosa of the rectum when they are passed
Doctor :
it is ok for you to have a soft stools
User :   Dr I forgot to mention I always seem to get a hot back for the last few months, post the incident. My wedding ring does not fit anymore. I probably don't deserve to wear it. My wife is trly my XXXXXXXX I love her and I can't bear to lose her
Doctor :   mistakes will happen it is humanly, but what as a man you should do is try to overcome the mistake
Doctor :   life cannot be stagnant thinking of some mistake happened on the past
Doctor :
this will not only affect you it will also affect your wife and daughter
Doctor :
do you want that to happen?
User :   No way, point taken. I used to be so bubbly and outgoing. I have taken a step back, but you are right, this depressive attitude of mine is affecting everyone close, namely my immediate family. Any inference about my hot back?
Doctor :   once you set up your tempo and start thinking positive, you will see that the hot back will disappear by itself
User :   Is it anxiety or what, come on surely it can't last for over 4 months?
Doctor :   it is not anxiety but a mild depression,

User :   How do I overcome this, any suggestions? Surely it can't be HIV?
Doctor :   you are depressed because of two reasons, one you think you have cheated your wife. second you think you may have infected with the HIV
Doctor :   hiv i dont think you are infected with,
Doctor :
as the array of tests are negative
User :   I forgot to mention after that fateful day I developed a rash on my hand about 10 days after. It went away on its own, is this indicative of the infection?
Doctor :   you can close the chapter of thinking hiv bu getting the DNA PCR Analysis. i am sure you will be negative in that
Doctor :   i dont think so, it may be because of different reason
Doctor :
regarding the feeling of guilt that you have done mistake and you have cheated your wife
Doctor :
best thing is donk keep anything in you, disclose the fact to her and ask apology
User :   Are these tests cheap to get? Can I ask a GUM clinic specifically for that. I don't want to ask my GP as I don't want it on my record. I feel so awful about being unfaithful to my wife. I apologise profudely to her always.
Doctor :   this may be very much not acceptable to you
Doctor :
yes it can be got done at the GUM clinic, if they dont have it there ,they will provide you the center where it will be done
Doctor :   and i am sure that it will be negative
