Doctor :   Hi
Doctor :
How can I help you?
User :   this is regarding my wife pregnancy
User :   I am at my Full term pregnancy. as per the latest sonography i show all most 40 weeks which is considered as full term in INDIA. i am 29 yrs old and have no past medical sickness. in the last sonography i showed slightly excess water and everything else was fine. The doppler also showed that the blood supply and oxygen to the baby is fine. The doctor has also done a stress test which shows that the baby is fine and he is doing a stress test every alternate day along with doppler test. My doctor has done an internal examanation and my cervis is not ripe. My baby is already 3.8 kgs as per last sonography. the baby is still floating with head facing down. the doctor wants to induce pain but the possibility that i will deliver normally seem less afyer inducing and she says that if nothing happens on inducing pain we will have to operate and do a C-Section. Is this the best choice i have at pesent? Can we continue to wait and monitor and let things happen naturally, meaning wait for some sign to happen naturally before inducing? my due date was 20th January, 2010 and i am already overdue. I want to go through a normal pregnancy but not at the cost of any risk to me and the baby and i am afraid that my gynecologist prefers a C-Section. is it safe to wait. we have spoken this to the doctor but she feels it is not worth waiting? do you also feel that this is the best option we have?
User :   you can read what my questions and doubts are as above and let me know?
Doctor :
Ok
Doctor :
Let me read it
User :   okay waiting
Doctor :   Since you say everything is normal as checked by the Doctor
Doctor :
It is worth to wait for one more week till 2nd of February
Doctor :
Or else induction of labor with cervical ripening can be done
Doctor :
if you do not prefer the Cesarean
User :   but some say it will cause stress to the baby, although the NST (STRESS TEST) we have done shows all normal
User :   how can induction of labor with cervical ripening done? will the doctor know this? will she let us know this?
Doctor :
A pregnancy is usually completed in 38 to 42 weeks.
Doctor :
so you have two weeks to reach 42 weeks
Doctor :
Is this the first pregnancy?
User :   yes this is the first pregnancy
User :   however if we go ahead with the induction what are the chances of getting labour pain even if the cervic is not ripe and the baby is still floating
Doctor :   The Doctor who is dealing with pregnancy

Doctor :
will know how to ripen the cervix
Doctor :   As long as the mother feel the kicking movement

Doctor :
or the moving movements of the baby
Doctor :
the baby is not at risk
User :   yeah the bay is moving at all times
Doctor :
Since the amniotic liqor is adequate
Doctor :
and baby is head on position
User :   can be talk to our doctor about cervic ripening? will she feel offended?
Doctor :
you can wait and ask for labor induction
Doctor :
You can talk
Doctor :
After all it is for the benefit of the mother and baby she should work
Doctor :
Many doctors induce labor if a woman is 2 weeks past her due date.
User :   we are going to the hospital 2moro and doctor will be inducing pains 2moro morning? do you think we have a good chance that the labour pains will start even when the cervic is not ripe and the baby is floating with head down?
Doctor :   Here there is one more problem
Doctor :   she induces the cervical ripening not the pains
Doctor :
Since the baby is big, 3.8 kg
Doctor :
the vaginal delivery is not easy
User :   when her doctor examined her cervic internally yesterday she said that in her practical experience she does not feel that my wife will have a normal delivery so that is why we feel she prefers a C-Section when we have told her that we want to try for normal
Doctor :
Where are you from?
User :   yes vaginal delivery is not easy but i guess an episiotomy can be done which the doctor says she will in the case of a normal
User :   i am from mumbai
Doctor :
Ok
Doctor :
Yes episiotomy is done
User :   we are going to the hospital 2moro and doctor will be inducing pains 2moro morning? do you think we have a good chance that the labour pains will start even when the cervic is not ripe and the baby is floating with head down?
Doctor :
The Doctor gives Oxytocin which contracts the Uterus to push
Doctor :
the baby out
User :   oxytocin also ripens the cervix?
Doctor :
The other way is to break the waters open
Doctor :
Oxytocin shall induce contractions
Doctor :
There are cervical ripening agents called prostaglandins
User :   okay and what would ripen the cervix?
User :   oh! okay that might be helpful. will ask my doctor that
Doctor :
Now the Doctor is worried about the labor initiation
Doctor :
which had not taken place yet
Doctor :
that is why she is more inclined towards C/S
Doctor :   Thgere are 4 stages of labor
User :   yeah guess that could be one reason for her to suggest a C-Section
Doctor :
and the first stage has not started yet, I guess
Doctor :
and moreover the first pregnancy
Doctor :
with big baby
Doctor :
These reasons are suggesting her to go for C/S
User :   the thing is that in my wifes family everyone has had a normal delivery so that why so many doubts? it feels like a waiting game and we are confused what is the best thing to do. we want a normal but not with the risk of causing stress to the baby and my wife
Doctor :
Yes
Doctor :   Since the 41 weeks are not completed yet
User :   yeah you are right in your own way....the doctor is quite experienced and well known so i am sure she knows her job just that she prefers C-Sections from what we have heard
Doctor :
wait for few days and enter the 42nd week
Doctor :
let her know the progression of the labor
User :   where are you from? which part of india
Doctor :
if not then go for C/S
Doctor :
I am from
User :   progression meaning?
Doctor :
Labor progressing
Doctor :
The dilatation and thinning of cervix
User :   don't understand what labor progression means??
User :   you mean to say how the pregnancy is proceeding
Doctor :   It is near to labor now
Doctor :   Did the doctor say that head is floating still and not engaged?
User :   do you think we should induce 2moro??
Doctor :
there is something called engagement of the head
Doctor :   did it happen?
User :   the doctor wants us to come to the hospital and induce? what are the chnaces in the present conditions for a normal delivery if she induces. the doctor said that the head is still floating
User :   head is still floating and head has been down since the past 4 weeks almost now. my wife has been walking regularly for 1 hour everyday
Doctor :
Ok if the head is floating then she has one more reason that the pregnancy goes over due
User :   she has also been doing excerises in an anti natal class
Doctor :
Ok that is good
User :   i did not understand what you meant by "the head is floating then she has one more reason that the pregnancy goes over due". does this mean this is why she wants to try for C-SECTION if inductions fails
Doctor :
yes
Doctor :
You have one more week to go for a normal deliver
Doctor :   to happen the Doctor feels that the chances are very less examining the mother

User :   hmmm...okay. so what to tell my doctor. she has called 2moro for inducing pains
Doctor :
because the head is still floating
Doctor :   Let her take a decision by informing you properly about the sequence of events
Doctor :
You can say, since till 42 completed weeks we have time, can we delay
Doctor :
and wait
User :   so from what i understand you are saying that sinvce the head is still floating, since the cervix is not ripe and since the is the first pregnancy the doctor seems to think that in her practical experince a C-Section is what will eventually happens. is this what you trying to say?
User :   okay will speak to my doctor and see then.
Doctor :
You understand what I am trying to say
Doctor :
Yes correct
