User :   I was wondering about differential diagnosis for ctcl and plaque psoriasis in a patient on a TNF inhibitor experiencing new plaques and swollen lymph nodes..
User :   what would the difference be in plaque appearance?

User :   new symptoms include fatigue, alopecia, larger plaques w a diferent apearance... headaches, .. ctcl= cutaneous t cell lymphoma
User :   does this warrant a lymph node aspiration and skin bx?
Doctor :   The diagnosis of a new plaque skin or changes always need a biopsy for a definitive diagnosis
