User :   hi Dr. XXXXXXXX this is Axxxxhere.
User :   i have 5 months old son ..he had thermal burn yesterday with hot iron touch
User :   he has burn at left hand and left eye..
Doctor :   ok. please describe the symptoms he is having in detail
Doctor :   please describe the appearance of the area
User :   in left eye , water dropout is happening..lower and upper eyelid are inflamed.(red and swollen)
User :   i am applying neospory eye ointment
Doctor :   is there swelling of eyelids?
Doctor :   is there blistering of skin present?
User :   shall i start antibiotic or first consult specialist ?
User :   no blistering but discoloration specially below lower eyelids
Doctor :   you should first consult a pediatrician with examination of the burns to know the extent and whether oral antibiotics would be needed to avoif secondary bacterial infection
Doctor :   if the eye and eyelid lesions are serious, then he will need an ophthalmologist opinion too
User :   there is no discoloration in eyes and he is able to open 80% of his eyes.
Doctor :   how did he get the burns?, was the hot iron in him for long time?
User :   just a second touch , long time
User :   thr is improvement from yesterday but swelling is thre
Doctor :   s there swelling of eyelids? is there blistering of skin present over eyelids?
Doctor :   ok, then it will take about 72 hours for redness and swelling to go away
User :   swelling of lower eyelids is there..but no blisters of skin present over eyelids.
Doctor :   Burnol ointment can be applied to the skin (except eye area)
User :   Is neosporin not okay?
User :   shall we wait for 72 hrs or go for examination ?
Doctor :   neosporin ointment you are applying over eyelids or left hand burns?
Doctor :   is the redness better since yesterday?
User :   neosporin eye ointment for eyelids and neopsorin skin ointment for left hand burn
User :   yes better.
Doctor :   keep the infant out of reach of dangerous objects, take special care
User :   ys..feels like we have to be more careful
User :   can u tell us why tears are coming from left eye ? is it okay ?
Doctor :   then continue to use them 2-4 times a day. if there is no improvement or if the child develops fever, itching, blistering of skin, any serious or pus discharge
Doctor :   then he will need to examined
Doctor :   the watering from eye indicates conjunctival inflammation
Doctor :   it should subside within 48 hours of the incident
Doctor :   if it continues, then it will signal ongoing infection due to burns
Doctor :   and will need antibiotic drops (pediatric ones) to be used to resolve the problem
Doctor :   did you follow what I have said?
User :   do u have any recommendation for antibiotic drops ?
User : wife is a dentist so she is able to understand the medical terms
Doctor :   no, I will not prescribe it as it is prescription medication which should be given when exact cause and need is made out after examination

Doctor :   to avoid further untoward incidents in furture
