User :   my girlfriend had tb when she was 17 y/o, she underwent treatment then. she's now 25 y/o, she started coughing blood a few days ago, is TB latent and infectious at this point?, am I at risk of infection? what should I do to prevent?
Doctor :   Hi,
User :   hello
User :   Hello Dr.
User :   my question is above
Doctor :   Ye,s there are high chances of TB to relapse
User :   can i be infected at this point?
Doctor :   Is she has hemoptysis , it could be a possibility thats its a open TB now ( contagious )
Doctor :   yes, you are at risk
User :   what should i do?
Doctor :   before that i would ask you to get her tested for the same
Doctor :   get a sputum analyisis and consult a pulmonologist
Doctor :   Analysis*
User :   what about the skin test?
Doctor :   no that is not reliable now
User :   is sputum analysis a blood test?
Doctor :   it will be anyway positive because of memory antibodies created by old infection
Doctor :   Sputum analysis is gold standard here
Doctor :   PCR test with blood sample is very sensitive , better than sputum
Doctor :   and accurate too
Doctor :   Take her to a pulmonologist after these tests
Doctor :   if the tests are positive then she must have to be restarted on treatment again
Doctor :   and you may have to be put on single drug prophylactic treatment
User :   what is that about?
Doctor :   Prophylactic treatment is to prevent infection in people exposed to infection
User :   so i should get a blood test first?
Doctor :   the skin test is ok for you
Doctor :   but she has to get a sputum analysis and a PCR test to confirm the relapse
Doctor :   Do you follow me ?
User :   yes, so what about during her treatment?
User :   what is the risk?
Doctor :   during the first two months of the treatment she may be infective later after that the risk is very low
User :   infective by air, contact?
Doctor :   By air
User :   ok Dr, thank you very much
Doctor :   you are welcome
Doctor :   Thanks for consulting me ,kindly provide your feedback
User :   any last recommendation?
Doctor :   do not quarantine her
User :   i wont. Thanks
Doctor :   though she is infective but the risk is not too high
