User :   22 month old baby, high fever-102, very high tlc-28.6thousands, very high neutrophil count-0000, throat swab test-scanty squamous epithelial cells, URT flora seen. CRP positive for bacterial infection. SGOT-287, SGPT-380. On physical examination - congested throat,
Baby is tugging on his right ear a lot, rubbing his right eye often.
He was given augmentin duo for 7 days. reacted well. fever was over after 3rd dose.
Questions
what are these scanty epithelial cells in throat?
Can this be ear infection as well?
Is it sure that fever was due to throat infection?
Please guide

User :   is it normal to have squamous epithelial cells in throat of a baby?

User :   he has been a liver patient since birth
Doctor :   It is very common to see some lining of squamous which is nasopharynx.
Doctor :   Usually in the esophagus, it is columnar epithelium.
Doctor :   But the report says Flora seen, which says that there can be an Infection in throat.
User :   he was suspected to be CMV positive so he was given valcyte for one whole month.. i hope it has nothing to do with the side effects of that drug
User :   coz i just read on the net that valcyte is carciogenic and the presence of squamous epithelial cells seems scary
Doctor :   It is very rare to see side effects such as neutropenia and thrombocytopenia, pancytopenia,
Doctor :   But here we have seen a reversal and we see neutrophilia .
Doctor :   So this may be due to some bacterial flora in thr URT, which might have caused.
User :   shud we get another throat swab done?
Doctor :   Have they cultured and found the sensitivity in your previous throat swab test ?
Doctor :   As they identify the bacteria.
Doctor :   in throat swab cultures.
User :   they didnt get blood culture test done coz at that time he was already on augmentin
Doctor :   How about throat swab culture ?
User :   they havent ruled out any bacteria

User :   in his throat swab test they just wrote-scanty squamous epitthelial cells, URT flora seen

In that case I would have suggest another swab test if it was done before Augmentin, but now there would be an alteration in the throat culture , so no point now.

User :   no

User :   he is ok started eating well
User :   but stays irritable and falls off often
User :   we are giving shelcal, zincovit, etc
Doctor :   Could you elaborate on falls off ?
User :   along with udiliv for liver
User :   yeah sure
User :   he has a history of falling off very often.
User :   his eeg didnt seem normal to his neuro peadiatric... but MRI shown mildly prominent temporal region

Doctor :   Has he been diagnosed with any type of Petit mal seizures ?
User :   his neuro Paed said nothing to worry , just watch, acc to her he should overcome it with time. May be due to liver disorder (undiagnosed cause) his muscles are weak

User :   no he nvr had seizure
User :   we hv givn him lot of vita D AAAAAAA

User :   but overall he has weak muscles and bones..... thats wat we feel

User :   oh yes... he complains of back ache after playing in park, he bends his wrists often while playing bit rough on bed, he falls off often coz he doesnt put his legs and feet properly on floor ... he lacks balance

User :   he eats well but actually he is a bad eater

User :   yes it was higher earlier

User :   we got it tested 3 times
User :   same figure every time

Doctor :   Have you tested him for Pes cavus or Pes talipus ?

Doctor :   I mean any foot arch deformoties due to frequesnt falls ?
User :   not yet

User :   who does it....ortho
Doctor :   I would advise you to visit a Ped.Ortho
User :   or plain x ray

User :   fine i will do it

User :   he already has a history of 3 incidents of ear infections, severe AOM....

Doctor :   Acute otitis media ?
User :   wat i feel is that due to throat infection the infection has travelled to ears as well
User :   yes acute otitis media
Doctor :   It might be due to the throat infections which pass on to ears through eusctachian tubes.
Doctor :   And the ear infection can cause throat infection too.
User :   now though the throat infectin is gone but he is still tuggin on his ears and eys
