User :   there is nothing wrong and ir surprises me much more for the condition to get so chronic

Doctor :   I would still suspect it to be Tuberculosis Since the Montaux Tes would not be reliable
Doctor :   hence would recommend you to get the Bronchoscopy done for the Sputum Sample and get that tested for Tuberculosis..
User :   does a BCG not work then why do we vaccinate children for
Doctor :   Raised ESR and X-ray findings of URTI
Doctor :   BCG efficacy you would Know that is in the Range of 30-80% only so.
Doctor :   since it has not Resolved to Augmenting and Clarithromycin
