User:hello doctor ...
User:Gud evening... :)
Doctor:Gud evening
Doctor:How can I help you?
User:we have 6 months age baby girl..
User:she is having some problem in her foot..
User:today we have consulted pediotric neurologist
User:his name is XXXXXXX XXXXXXX aroor in bangalore...
User:he has suggested the test called nerve conduction test
User:we got the result as welll
User:he told us tat my baby has neuropathy problem..
User:motor neuropathy..
Doctor:did he tell you the cause?
User:he told us genetical disorder..
User:but any of our family members from myside or my wife side does not have this kind of problem..
Doctor:can you tell me the exact diagnosis, XXXXXXX?
User:test name is : neuro conductive test
Doctor:Not the test name
Doctor:What does your child have
Doctor:There are loads of genetic motor neuropathies
User:his findings on the report is : Motor neuropathy moderate to severe in upper limb & severe in both lower limb.
User:he has suggested the physiotheray...
User:could you please tell us what is motor neuropathy.. ?
Doctor:actually XXXXXXX
Doctor:Its a condition where the motor nerves do not function properly due to an abnormality in their structure or function
User:physiotherapy suggested is : galvanic stimulation..
Doctor:In different kind of neuropathies, different kind of defects are present
Doctor:Physiotherapy helps since the nerves are stimulated which improves their function
User:wat is galvanic stimulation ?
Doctor:Its like using current to stimulate the nerves
Doctor:A small amount of current is used
Doctor:so it helps to stimulate the nerves
User:i have one basic question..
User:why does the nerves abnormality happens ?
Doctor:for a nerve to function properly, many proteins are required
Doctor:when there is a genetic problem ( which may not always be manifested in relatives)
Doctor:some of the proteins are not ok
Doctor:This causes the nerve function to decline
Doctor:most of these disorders XXXXXXX are autosomal recessive
Doctor:which means you will hardly have any relatives who have the problem\
Doctor:But since both you and your wife have 1 defective gene for this
Doctor:these 2 combined in your child to cause a problem
Doctor:There is no problem with you
Doctor:because you have 1 normal gene as well which produces enough protein
Doctor:Is there any other concern that I can address?
User:for the problem which my daughter has how severe it is ?
User:is it curable by physiotherapy ?
User:what could be the impact of this problem in her growth ?
Doctor:The severity and long term outlook can be decided only if I know the exact condition XXXXXXX
Doctor:While physiotherapy should help
Doctor:If its hereditary its unlikely to be curable
Doctor:As I said , there are lots of hereditary motor neuropathies
Doctor:As far as the growth is concerned,
Doctor:She should attain her normal height and weight
Doctor:But depending on the condiion
Doctor:moving around can be a problem
Doctor:So a part which is not used, typically doesnt develop very well
User:i will write down the procedure what they did it in hospital...
Doctor:Do 1 thing XXXXXXX
Doctor:Ask your doctor
User:yes tell me
Doctor:the exact diagnosis
Doctor:then we will be able to discuss this better
Doctor:Use these words
Doctor:Tell him you want to discuss with another doctor
Doctor:so the technicals are required
User:i will write down the technicals what they mentioned in the report.
Doctor:when can I see you back
User:just check & let me know is this helpful to discuss this better
User:i will give u now itself
User:MOTOR conduction and F wave studeis perfromed on right median, ULNAR and bilateral common peroneal[CP], Posterior Tibial [ PT] nerves and sensory conduction performed on right median ulnar and both sural nerve
User:this the the technicals mentioned in report
Doctor:what is the treating doctor's impression
Doctor:Dr XXXXXXX Rao's
User:results : Encl: Tracings of all recordings with relevant values. EMR amplitude severely reduced in right median and ulnar nerves and not elicited from both side CP, PT nerve
User:conclusion : findings suggest motor neuropathy moderate to severe in upper limb & severe in both lower limbs..
Doctor:did u show these finding to dr suresh?
User:this is what the doctors impression as well ..
User:we did
Doctor:what did he say
User:he said go for physiotherapy & galvanic stimulation and consult him after 10 weeks..
