User :   I have a question about contraception.
Doctor :   hi
User :   Hi
Doctor :   Dr. XXXXXXXX here
Doctor :   please proceed with your health query
User :   My girlfriend and I were having sex and afterward we found the condom had slipped off and was sitting on the bed about 2 inches below her vagina.
Doctor :   ok
User :   She takes ortho tri cyclen for birth control
Doctor :   ok. is she regular on it? since when she is taking them?
User :   however she has only taken 3 pills since her period
User :   she has been taking them for over a year
User :   last month she took two or three in the morning instead of at night when she usually does
Doctor :   ok
Doctor :   the important thing is whether she is regular on it?
Doctor :   if she is not missed any of her pills then there is no need to worry
Doctor :   when was this unprotected intercourse? (when condom slipped)?
User :   we were concerned about it since shes only taken three pills so far and i went and got her a plan b pill
User :   there was semen on the bed and in the condom so I think it slipped off during witdrawl
User :   * withdrawal
Doctor :   ok. she can take the plan B pill as additional contraception, but since she is already on BCP, it would not be necessary (if she is regular with it)
User :   thats what i thought but she was really worried so i got her the plan b
Doctor :   and she can take two pills of the BCP each 12 hours apart from the time of the intercourse
Doctor :   this too will give her additional protection even if she does not take plan-B
User :   she took the plan b pill about 3 hours after intercourse and plans to take the ortho tri cyclen as normal every night
Doctor :   yes, that will be the right thing to do,
Doctor :   let her continue with the usual BCP
Doctor :   as she takes it normally and not miss any dose
Doctor :   in case she does forget to take 1 pill,
Doctor :   she can take it withing 12 hours or as sson as she remembers, and take that days pill too
Doctor :   *soon
Doctor :   have you followed what I have said?
User :   yes
Doctor :   is there anything else you would like to ask me?
User :   how concerned should we be about this?
User :   shes losing sleep over it
Doctor :   since she is on regular BCP and has taken plan -B too, let her be regular with her BCP now
Doctor :   and wait for her periods whil eshe is on th eplacebo pills
User :   what are the chances of her getting pregnant?
Doctor :   there is no need to worry as far as pregnancy is concerned
Doctor :   if she is regular with the BCP's then there is no chance of pregnancy
Doctor :   as BCP's work by inhibiting ovulation(egg release)
User :   thats what i thought
Doctor :   and the plan-B works by not letting the fertilized egg get implanted in uterus
Doctor :   so she has taken both of them, so no need to wory now
User :   she thought there would be a problem because she only took 3 BCP so far
User :   that didnt make sense to me
Doctor :   plan-B works the best if taken within12 hours of the unprotected intercourse, and she has taken it 3 hours later, as per your history.
Doctor :   if she was regular with her pill in her previous cycles, there wound not be any problems
Doctor :   even if she has taken only 3 pills in this cycle
Doctor :   she can continue taking the rest of then as usual now
Doctor :   have you followed what I have said now?
User :   yes
Doctor :   is there anything else you would like to ask me?
User :   so basically
User :   even if semen somehow got in her
User :   her normal bcp will prevent ovulation anyway
Doctor :   yes, very much
User :   and even if it didnt the plan B would prevent the egg from becoming implanted
Doctor :   and even if the semen got into her, and she was not regular with her normal BCP
Doctor :   and somehow the egg got fertized
Doctor :   the plan-B will take care
Doctor :   by not letting implantation occur
User :   great thanks
Doctor :   so chances of pregnancy as of now for her is nil
User :   so where are you at location wise?
Doctor :   I am presently located in XXX
User :   cool
User :   how long have you been a doctor?
Doctor :   more than 6 years now
Doctor :   with experience mainly in OBG
Doctor :   is there anything else you would like to ask me XXX?
User :   im going to e-mail her this chat thats why i asked
User :   i dunno i paid for 30 min ..
Doctor :   ok
Doctor :   if you have any other query, you can proceed by asking me
User :   what are some of the causes of a paralyzed vocal cord?
Doctor :   you still have about 10 minutes more to go
User :   my grandma has one of her vocal cords paralyzed
User :   she cut her leg a few months before and waited before going to the doctor at it was pretty bad
Doctor :   ok. the causes could result from repeated infections, voice exertion, injury, and cancer.
User :   she had to get some really strong anti-biotics to get rid of the infection from the wound
Doctor :   the exact cause can be made out by careful examination and also by looking at the functioning via endoscope
User :   would the infection in her leg cause the paralyzed vocal cord?
Doctor :   infected would can lead to many problems, and the choice of antibiotics depends on th organism affecting it and the extent of the infection.
Doctor :   no, it wouldnt. the vocal cords paralysis is something very different
Doctor :   has it been thoroughly checked by ENT specialist?
User :   but could the infection have lingered in her after her leg healed?
User :   she got an MRI today but i havent heard what the results were
Doctor :   no, the antibiotics given will work systemically and fight infection anywhere in the body
User :   her ENT doc wasnt very good
Doctor :   get the vocals cords reexamined by some other ENT specialist and also get a endoscopy done
User :   so you think the likelihood of a prior infection causing it is small?
Doctor :   to see the functioning of it
Doctor :   yes.
User :   her bloodwork was all normal too
Doctor :   ok
User :   also doesnt have any appetite
User :   forces herself to eat
Doctor :   ok
User :   so what are possible diagnosis?
Doctor :   could you describe her symptoms in detail?
User :   not like she could
User :   and i know she needs to go see a good ENT and get an endoscopy
Doctor :   what symptoms does she have at present?
Doctor :   could you describe her symptoms in detail?
User :   she has more energy
User :   was very weak at first
User :   one vocal cord paralyzed
User :   voice sounds different
User :   very little appetite
Doctor :   when and how this vocal cord paralysis diagnosed?
User :   infection history that was treated with antibiotics
Doctor :   ok
User :   her ENT doesnt know
User :   i dont even think he was able to do an edoscopy
User :   *endoscopy
User :   thats the think they stick up your nose right?
Doctor :   ok. then the only way is to get a repeat examination by some other ENT surgeon
Doctor :   and see what has hapened to her vocal cords
Doctor :   the lack of appetite could be due to gastritis induced by previous antibiotics
User :   what is the most common cause of vocal cord paralysis?
Doctor :   it should subside in sometime, if not, she would require acid suppressors
Doctor :   paralysis is usually due to the damage of to nerve supplying it
Doctor :   so the cause for that has to be found out
User :   thanks for your time
User :   :)
User :   its appreciated
Doctor :   and first and foremost, it needs to be confirmed whether it is paralysed or not
Doctor :   Please fill the feedback form say done when done
Doctor :   Thanks for consulting me. Bye for now. Please chat with our doctors 24/7 for medical queries. Please refer your friends to our service
