I already dilated to 4cm and 80% eff, and baby is at -1/0 level. after that my cervix is not changing much. last day also I had pains for 6-7 hours but cervix hasn't changed. I need help on this.                                                                                                                   User :   Hi
Doctor :   hi
User :   Hi Doctor
User :   I need your help, can please find my question
Doctor :   let me read what you have typed
Doctor :   when was your last scan done? what was the report?
Doctor :   is the dilatation and effacement improved since yesterday?
User :   i think around 2 weeks back, whe nI was in hospital, everything is fine and baby is ahead
Doctor :   is this your first pregnancy?
User :   today, I didn't go for checkup, I have next app on coming tuesday
User :   no second doctor
User :   And I am continuously getting contractions.
Doctor :   if the effacement is progressing well, but dilation not is happening, then it coul dbe due to cervical dystocia
Doctor :   where cervix fails to dilate
Doctor :   have you been monitoring baby's heart rate?
User :   yes, i am checking at home also, doing fine
User :   13-140 per min
User :   sorry 130-140
Doctor :   in second pregnancy, usually labor is of short duration, cervical dilatation should not take long time if effacement is going on fine.
User :   and i fell some baby head or something feel on my cervix, but not sure
User :   ok
Doctor :   ok do these come down with the contractions?
User :   didn't get doctor
Doctor :   is the heart rate of baby dropping when you have the contraction?
User :   no
User :   2 weeks back I had pain and contractions and it was 3cm and 80% eff, and after 2-3 days 4 cm and 80%, after that no change
Doctor :   ok. when have you last checked the heart rate while having contraction?
Doctor :   how long does each contraction last ?
User :   last day, i had good pains and contractions for 6-7 hrs and i went hospital for chekup but no change in cervix
User :   30 -60 secs
Doctor :   ok.
Doctor :   you seem to be in active labour with cervical dilatation not happening.
User :   if i get one contractions and next one i am getting within 1-2 mins and 3 rd one 4-5 mins, this is recycling
User :   yeah
Doctor :   i would recommend you to go to a gynecologist or discuss over phone with your treating doctor
Doctor :   about cervical dystocia chances
User :   i don't want to get induced or something, I want natural labor doctor
Doctor :   and if something can be done to ripen cervix and make it dilate
User :   who can get "cervical dystocia"? doctor
Doctor :   what is the expected birth weight of the baby?
User :   when they checked 2 weeks back it was 5 pounds
User :   and baby is active all the time
User :   what is "cervical dystocia" about?
Doctor :   anyone can get cervical dystocia, it is a response of cervix that is altered
Doctor :   and can happen in anyone
Doctor :   aslong as you do not get huge dips and highs in heart rate of baby with your contraction, it should be fine
Doctor :   you can wait for your next appointment day
User :   ok
User :   Can I try natural ways for cerivx ripen/dilate?
Doctor :   but in case you start getting any abnormality in heart rate, then it will be emergency to do a cesarean to save the baby
User :   I am getting sick/depression because of this doctor
Doctor :   yes, if it is case of cervical dystocia, your doctor will first give you injecatable and topical medications to help it dilate
Doctor :   *injectable
User :   ok
Doctor :   if it fails totally to dilate and in the meantime baby starts getting compromised, then there will be no other option
Doctor :   than to do emergency cesarean
Doctor :   so be in constant touch with your doctor
User :   oh ok, so normal heart rate, should be 120-160, so other than that I need to be worry?
Doctor :   do not be depressed, it will lead to decrease in baby's heart rate too
Doctor :   no, you do not need to worry, the heart rate should not go beyond 160 or below 120 with your contractions
Doctor :   or anytime just after the contractions.
User :   contractions will hurt the baby doctor?
Doctor :   but cervical dystocia chances should be ruled out by an examination, and if found timely intervention will help prevent complications
User :   how they can find about Cervix dystocia?
Doctor :   during contraction the blood supply to uterus gets cut ,momentarily, so the baby might have suffocation as evidenced with change in heart rate
Doctor :   it can be found with your history and duration of labour and a careful examination
User :   ok, if heart rate doesn't change, then it is fine?
Doctor :   yes it is safe until it happens, but when it happens, it will mean that the baby has already tried to be fine, but now it is not able to be fine
User :   other than medical usage, can you help me to try natural things for cervix dilation?
Doctor :   no, it is normal body process that happens, i will not advise anything natural, it can sometimes be harmful also.
Doctor :   and lead to infection risk to baby
User :   ok
User :   can have sex at this time?, just want to make sure
Doctor :   wait and watch, be alert and in constant touch of your gynecologist
Doctor :   it might help release hormones to dilate cervix, but can also be a risk to cause rupture of membranes
Doctor :   and lead to complications
User :   ok
Doctor :   since you are 4 cm dilated, it will be better not to have sex at this time.
User :   ok
User :   how they can find about Cervix dystocia? and symptoms?
Doctor :   your history of labor and cervix dilation findings along with an examination will help to find it out as i said earlier
Doctor :   also the babys condition has to be looked upon.
User :   how can I find, if my cervix is changed or not by now?
Doctor :   you will feel more pressure of the head pressing down
Doctor :   and feel like going for a bowel movement
User :   I felt these things when I had pain with contraction
User :   but when doctor checked she said only 4, is there chance that doctor measured wrong?
Doctor :   it could have increased by now if you are having strong contractions
Doctor :   but if it has remained the same
Doctor :   then there are chances that it is due to the cervical dystocia
User :   but pain got reduced, i don't have pain, but have contractions
User :   if I get contractions/pain, Cervix has to change?
Doctor :   ok
Doctor :   yes usually due to the contractions baby is pushed down, and the pressure will help cervix dilated and be taken up.
User :   I do found the mucus discharge from 2 days
Doctor :   it could be due to the mucus plug coming out since cervix is dilated to 4cm now
User :   my first labor was very cool doctor and normal but i am not sure, now why it is happening like that, due to I am on bedrest, does walking helps?
Doctor :   walking does help. why were you put on bedrest? was there any specific reason for it?
User :   Doctor told me to take bedrest
User :   my blood pressure getting low sometimes
User :   my blood pressure always 100/60 are high is 110/70 and this pregnancy, 90/50 sometimes 80/40 also
User :   and now it is better, i didn't have good food also due to gastric
Doctor :   ok. then do not walk much now have someone with you always if you do walk
User :   yeah, what would be your final suggestions doctor?
Doctor :   my final suggestions would be: Doctor :   1) be in constant touch with your doctor , inform about contraction and baby heart rate every 2 hours or so
Doctor :   2
Doctor :   2) monitor contraction timing and gap iun between contraction along with baby's heart rate
Doctor :   note it down and report to your doctor
Doctor :   3) watch for signs of cervical dilation or rupture of membranes
Doctor :   4) if any drop in heart rate of baby anytime, report to hpspital immediately
User :   ok
Doctor :   5) discuss about cervical dystocia chances
Doctor :   and early intervention for it
Doctor :   6) do walking in between, but with assistance
