Doctor :   Hi
Doctor :   How can I help you?
User :   hi Doc
User :   My daughter age - 21 months is having cough and cold
Doctor :   Ok
User :   She was referred Kosat for cough earlier, but now she is not liking the medicine
Doctor :   Ok
User :   What should we do?
Doctor :   Since when is she having the cough and cold?
User :   Its been 3 days, but a bit severe from last night
Doctor :   Is she running fever?
Doctor :   Does she have nasal congestion, difficulty in swallowing?
User :   very little warm, but no high fever
User :   yes
User :   today she was crying while she was having a banana
Doctor :   Ok
Doctor :   Does she have any difficulty in breathing?
User :   yes
User :   She has just recovered 2 weeks back from gastrointitis
Doctor :   Since when have you noticed the difficulty in breathing?
User :   yesterday night
Doctor :   Ok
Doctor :   Let me brief you.
User :   ok
Doctor :   The commonest cause for cough and cold in young are allergies or
Doctor :   upper respiratory tract infections.
Doctor :   Alllergic causes will settle down on its own, however if she has fever, it can mean
Doctor :   that the cause is most likely an infection.
User :   what should we do? what medicine can I give her instead of Kosat
Doctor :   Hence if her temperature is high (more than the normal body temperature),
Doctor :   I suggest you to consult a pediatrician.
Doctor :   In the mean time you can try saline nasal drops for nasal congestion.
User :   we have used nasomist
Doctor :   I would suggest you not to use nasomist,
User :   her temp was 97.8
Doctor :   Nasomist can cause rebound nasal congestion once the effect of the drug comes down.
User :   what happened, is it not good
Doctor :   Instead saline nasal drops would be a better choice.
Doctor :   Have you followed what I have suggested?
User :   but nasomist says it is saline nasal drops
User :   not actually
Doctor :   Oxymetazoline is a content in some nasomist drops,
Doctor :   Oxymetazoline can cause rebound increase in the nasal congestion,
Doctor :   So if the drops contain this ingredient, than my suggestions is to change it for a plain nasal saline nasal drops.
User :   this contains isotonic and buffered solution of sodium chloride I P 0.65% with Benzalkonium Chloride as preservative in Purified water
Doctor :   Then its fine.
Doctor :   You can use it.
User :   what should we used instead of Kosat
User :   for baby's cough
Doctor :   Since you said, she is having fever, I suggest you to consult a pediatrician for evaluation and treatment,
Doctor :   She may need antibiotics too.
Doctor :   Have you followed what I have suggested?
User :   but her temp is 97.8 which is normal isn't it?
Doctor :   Ok
User :   she is been active through out
Doctor :   If her temperature is normal, then you can try robitussin pediatric cough syrup 2.5 ml twice a day.
User :   I had consulted her pediac.. he asked to take the same medicines, but my concern is she is not liking Kosat. Secondly, she has fallen sick thrice earlier and the symptoms are similar, she gets normal cold/cough and then it turns out to be some viral
Doctor :   Monitor her temperature regularly,
User :   ok
Doctor :   it looks more like a viral illness.
Doctor :   However it can soon turn bacterial, which is indicated by presence of high fever.
Doctor :   hence watch her temperature,
Doctor :   if you find her temperature is going high, then she may need antibiotics.
User :   ok
Doctor :   Apart from this also look for decrease in activity, hurried breathing - these also are an indication that you need to consult a doctor.
Doctor :   Have you followed what I have suggested?
User :   I will keep a tab on her temp
Doctor :   Do you have any other query?
User :   and if its high then I will take her to Dr. or if the cough doesn't subside in couple of days
User :   Dr. also wanted to know if there is any solution for PSP - variation of Parkinsons as yet - My Dad is suffering from PSP.
Doctor :   If you are looking for a permanent cure, then till date, there are none.
Doctor :   for PSP.
Doctor :   The medications that are given for it are for control of symptoms alone.
Doctor :   However research is on for finding a cure by means of stem cell therapy.
Doctor :   The result of this will take time, and we may have cure for this condition in the future.
Doctor :   If you are interested in trying this, you can discuss with your neurologist.
Doctor :   Have you followed?
Doctor :   Are you there?
Doctor :   Are you there?
Doctor :   Are you there?
User :   ok, system is very slow
User :   he is on syndopa, but his condition isgetting worse, difficulty in eating food etc.
Doctor :   PSP is a progressive condition and the medical drugs have to be modified as the condition varies.
Doctor :   He may need addition of few more drugs if necessary for getting a desired result.
User :   he is on maximum dose of syndopa.. every 4-6 hrs
Doctor :   Similarly are the need for taking the medical drugs regularly without fail.
User :   How will I know once the stem cell therapy is available..also how much wud it cost approximately
Doctor :   If the drug is not producing the desired results, then you can discuss with your neurologist for the possibility of addition of another drug into the treatment.
Doctor :   The stem cell therapy as mentioned is in the research stages.
Doctor :   Few hospital which also has research centres may have this facility.
Doctor :   Again, i suggest you to discuss with your neurologist about this.
User :   Is there also a way by using dopamine reactor?
Doctor :   Since it is still on trial the effect of this therapy is still unknown.
Doctor :   Dopamine reactor?
User :   ok, I will consult his Doctor on the medication.
Doctor :   The antiparkinsonism drugs act by three modes.
Doctor :   1. One which increases the dopamine production - like the class of syndopa.
Doctor :   I mean 2 modes.
Doctor :   2. One drug that produces opposite effects of dopamine.
Doctor :   called as anticholinergic group of drugs.
Doctor :   You can discuss about the need for the second drug with your neurologist.
Doctor :   Have you followed what I have suggested?
User :   K. thanks a lot for ur assistance Doc..
User :   Whats ur name Doc?
Doctor :   Dr XXX
Doctor :   Do you have any other query?
User :   no, thanks
Doctor :   Thanks for consulting me. Bye for now.
