User :   I've had an ectopic episode back in January which hospitalised me for two days, pulse 120 . Since coming out I had a high puse of 110 ( occasional ) and problems at night breathing ( but only in the night ) its like someone has swithced off my breathing I awake from a deep sleep totally unable to breath at all and after about 4/5 seconds its clicks back in...when I take my BP the lowest its been is 85/ soon as I get up Im fine.....its like im losing conciousness (this is not anxiety) cardiologist says although they have patients with these symptoms, but my echo is totally normal.....tilt test normal and corsitol nrmal, bicarbonate low....I can live with strong PVC's although I have no idea why suddenly they would start....but the night time breathing issue is worrisome....though it stops when I put on weight.... im five foot four.I was 7 st 7 pounds whn I had the episode and when I go up to eight stone 3 I sleep through the night..........the other worrisone item is the cardiologist said I had missed beats and a few prolonged beats......but then didnt explain.....I asked the registrar when I went back and he didnt know and couldnt see why the cardiologist said this, obviously with long qt syndrome it worried me, but if that was a worry would they have picked up on it???
Doctor :   Hi Dr xxx here
Doctor :   How can i help you ?
User :   I wonder if you can shed light on the above questions
User :   hello by the way, sorry
User :   hello?
Doctor :   Hi
Doctor :   sorry for the minor technical error
User :   dont know what happened there
Doctor :   let me read your query
User :   no worries
Doctor :   ok
Doctor :   from what i have gained from your history, you may be a tachycardia syndrome
Doctor :   and your doc, has suggested it could be long QT syndrome , is this right ?
User :   no , definately not.....he said I had a few prolonged beats as a throw awy comment after the missed beats and extra beats and then never mentioned it again
User :   its was only afterwards that it started to conern me. hes a good cardio
User :   ??
Doctor :   ok
Doctor :   you are defintely not over weight , as per your height and weight ratio
Doctor :   have u ever done a sleep study?
Doctor :   Do u smoke ?
User :   can you tell me if a bp of this level is normal at night
User :   no dont smoke
User :   no sleep study
Doctor :   for your weight and body ratio, a BP of 100/ 60 WOULD Not be low and it would be adequate
Doctor :   but sometimes, while sleep or at rest the BP can be a bit lower as you have
User :   ok, so what are your thoughts on 85/36
User :   becasue thats almost halve
User :   half
Doctor :   so 85/ 40 would be ok,.... unless you have symptoms like dizziness, black outs, sudden unconsciousness , etc
User :   but its at night and its waking me unable to breathe????
User :   not during the day....what causes this???
Doctor :   but if you are symptomatic, then, a tilt table sdudy will be beneficial, to know the cause of your low BP as it will be helpful to rule out autonomic neuropathy related causes
User :   tilt test normal as stated above
Doctor :   Do you wheeze or have dyspnea at rest ?
User :   no
Doctor :   ok
User :   i was pretty fit until the ectopic episode and ive eased off the exercise some what...could the pvcs be causing the low night time P
Doctor :   this symptoms of high pulse at night
Doctor :   yes , definitely,
Doctor :   \when you have PVCs, the heart runs faster
Doctor :   so as to cover the low intensity PVCs , and to make the blood flow normal
Doctor :   in this situation, the high PULSE, may give you discomfort
Doctor :   and your produce your symptoms
Doctor :   are you taking any medicines right now ?
User :   hi pulse not a problem it subsides and never above 110 since I have been in hospitak where it was 120....I get pvcs when normal resting pulse of 73
User :   no medicines apart from pill
Doctor :   ok,
Doctor :   i would recommend a sleep study, to evaluate it more
Doctor :   also, a EPI electrophysiology study, may be helpful further as well
Doctor :   are u following me ?
Doctor :   PVCs - are smaller heart beats, in addition to normal ones
