User:My daughter is 7 yrs 4 month and has slight cough since day before yesterday afternoon. i.e 3rd march, Yesterday she came back from school at 3PM she had 103 f fever. Gave crocin DS 10 ml at 3:30 and then at 8:45 before she went to bed. Gave Dilo BM 7.5 ml and allegra 5 ml also alongwith. Monring 8 AM temp was 99.2 not given crocin. Gave Dilo BM 5ML and allegra 5 ml. Now also she looks activeand fine , apparently no fever apart from slight cough. Should we continue medication

Doctor:The medicines that you used are symptom relieve medicines. It has no role in treating the cause of her symptoms.
Doctor:Apparently if she has no symptoms now, you need not continue it.
Doctor:Dilo BM has antitussive properties. If the cough is disturbing her, she can continue these for few more days.
Doctor:I don't see any need for allergra though.
Doctor:Crocin can be used only if she has fever again.

User:when you say fever .. is it above 100 degree f ?without crocin?
Doctor:Temperature measurement is to classify the grade of fever. Temperature above 100 means high grade fever which should be managed appropriately.
Doctor:I recommend my patients to get back to me even if low grade fever persists for more than 3-5 days.
User:ok doctor what should be the ideal doese of dilo bm for her- she is approx 26 kgs
Doctor:The recommended dose is about 1.2 to 1.6 mg/kg body weight.
Doctor:We titrate the dose depending on the concentration of drug in one ml.
Doctor:Usually that is mentioned on the label.
User:and just to inform you that she has allergic rhinits which keeps surfacing every now and then and gets under control with metaspray , otherwise every night her nose starts blocking.. ( not always though)
Doctor:I understand she has allergic symptoms. However without an opportunity to examine the child, I wouldn't be able to figure out if there is an element of allergy involved.

User:ok.. but continuing metaspray and xylomist - p shound be an issue now?
User:and do you advise allegra.. now , if yes than what should be the dose?

Doctor:I only meant that physical examination is mandatory to confirm a diagnosis and advice the right treatment.

Doctor:In a 7 year old child the sudden appearance of cough and fever may be from viral infection or allergy with super-added bacterial infection.
Doctor:Treatment of both these condition differs.
User:i undersntand that Doctor .. diff betn viral and bacterial infections .. but my question was related to uses of metaspray and allegra , both do not treat any infections
Doctor:Metaspray is a steroid which should be used along with antibiotic cover when there is high grade fever.
Doctor:Allegra has antihistamine properties which can be very essential if her symptoms is allergy related.
Doctor:Hence in the current context with just fever and cough as the symptoms, crocin and ambroxol (dilo BM) can suffice.
Doctor:Need for antihistamines and antibiotics can be determined after evaluating the child.
Doctor:At this point if the child is stable and has reduced symptoms you can continue crocin as and when fever spikes over 100 deg F
Doctor:and dilo BM twice daily.
Doctor:Monitor the child for next couple of days.
Doctor:If symptoms don't improve, earnestly visit the doctor.
